目的:观察刺山柑果风湿止痛贴对类风湿关节炎(RA)大鼠血清TNF-α、IL-1β、VEGF水平以及滑膜组织MMP-9蛋白表达的影响,探讨其对RA软骨保护的作用机制。 方法:40只大鼠随机分为正常组、模型组、阳性药组和刺山柑组,每组10只。除正常组外,其余各组大鼠足跖底部注射完全弗氏佐剂复制RA模型。阳性药组和刺山柑组分别以伤湿止痛膏和刺山柑果风湿止痛贴外敷,模型组以同样方式予以空白凝胶膏剂外敷,正常组不干预;各组均连续干预21 d(3个疗程),观察大鼠 AI指数评分、足爪容积和热痛阈,ELISA法检测大鼠血清TNF-α、IL-1β和VEGF水平,Western blot法检测滑膜组织中MMP-9蛋白表达。 结果:与正常组比较,造模后大鼠均出现炎症表现和热痛阈降低,模型组大鼠血清TNF-α、IL-1β和VEGF水平明显升高(P<0.05,P<0.01),MMP-9蛋白表达显著增高(P<0.01)。与模型组比较,阳性药组和刺山柑组的关节炎症和热痛阈敏感均明显改善,且刺山柑果风湿止痛贴的消肿作用起效更快(7 d),并能显著降低大鼠血清VEGF水平和滑膜组织中MMP-9蛋白的表达(P<0.05),而阳性药组均无明显变化(P>0.05)。 结论:刺山柑果风湿止痛贴对类风湿关节炎大鼠的消炎止痛作用可能与降低血清TNF-α、IL-1β、VEGF水平,抑制滑膜组织MMP-9蛋白表达有关。
Objective:To observe the effects of Capparis Spinosa fruits pain gel paste on serum levels of TNF-α,IL-1β,VEGF,and the expression of MMP-9 protein in synovium in rheumatoid arthritis (RA) rats and to explore its mechanism of anti-inflammation and cartilage protection in RA. MethodsA total of 40 rats were randomly divided into normal group, model group, positive drug group and Capparis Spinosa group with 10 rats in each group. The rat model of RA was established by injecting complete Freund’s adjuvant into the sole of the foot. The positive drug group and Capparis Spinosa group were treated with Shangshi Zhitong Gao (adhesive plaster for removing dampness and relieving pain) and Capparis Spinosa fruits pain gel paste, the model group was treated with blank gel paste, and the normal group was not given any treatment. The continuous intervention lasting 21 days (3 courses) was conducted in each group. AI index score, foot volume and thermal pain threshold were observed. The levels of TNF-α,IL-1β and VEGF in serum were detected by ELISA, and the expression of MMP-9 protein in synovium was detected by Western blot. Results:Compared with the normal group, inflammatory lesions and thermal pain threshold decreased in all the rats after modeling, and the levels of serum TNF-α,IL-1β and VEGF increased significantly in the model group (P<0.05, P<0.01),and the expression of MMP-9 protein increased significantly (P<0.01). Compared with the model group, the arthritis and thermal pain threshold sensitivity in the positive drug group and Capparis Spinosa group were significantly improved, the anti-swelling effect of Capparis Spinosa fruits pain gel paste was faster (7 days), and the serum VEGF level and the expression of MMP-9 protein in the synovium of rats reduced significantly (P<0.05) while there was no significant change in the positive drug group (P>0.05). Conclusion:The anti-inflammatory and analgesic effects of Capparis Spinosa fruits pain gel paste on rats may be related to the decrease of serum TNF-α, IL-1β and VEGF levels and the inhibition of the expression of MMP-9 in synovium.
