目的:通过对颈部端提手法的动力学与运动学参数进行客观化描述,为端提手法的规范操作提供数据支撑。 方法:选择30例年龄18~36周岁的男性颈型颈椎病患者,由1名高年资推拿医师实施颈部端提手法操作;采用测力台及红外光点高速运动捕捉测试系统,采集施术者在端提手法操作过程中的动力学与运动学参数。 结果:①颈椎端提瞬间,施术者左右足力值差异具有统计学意义(P<0.05),施术者足底合力增加值与受试者足底合力减少值差异具有统计学意义(P<0.05)。②颈椎端提瞬间,施术者右肘关节合速度最大,其余依次是右肩、膝、腕关节;施术者右膝、肩、肘、腕关节合速度及合加速度比较,除膝、肩关节外(P>0.05),其余关节点两两比较,差异均有统计学意义(P<0.05)。 结论:①端提瞬间,施术者双足整体发力,以右足发力为主;②合加速度是导致施术者增加力与受试者减少力之间进行转换的影响因素之一;③颈部端提手法是以肘部“端提”为主,膝、肩、肘、腕关节整体协调发力的手法;④端提操作前,施术者右肘、肩关节牢牢环绕固定受试者颈部,可实现整体发力。
Objective:Through the objective description of dynamic and kinematic parameters of cervical lifting manipulation, to provide data support for standardized operation of lifting manipulation. Methods30 male patients aged 18-36 years old with cervical spondylosis of neck type were selected. The cervical lifting manipulation was implemented by a senior massage physician. The dynamic and kinematic parameters of the performer during the lifting manipulation process were collected by force platform and high speed motion capture test system for infrared light spot. Results:①At the moment of cervical lifting,there was statistically significant difference on the force value between the left and right foot in performer(P<0.05),and there was statistically significant difference between the increase value of plantar force in performer and the decrease value of plantar force in patient(P<0.05). ②At the moment of cervical lifting,the resultant velocity of right elbow joint in performer was maximum,followed by right shoulder,knee and wrist in turn. For the comparisons of resultant velocity and resultant acceleration in performer,there were statistically significant differences between the right elbow and other joints,right wrist and other joints(P<0.05),but there were no statistically significant differences between the right knee and other joints,right shoulder and other joints(P>0.05). Conclusion:①At the moment of cervical lifting,the performer makes the force on both feet and mainly depends on the right foot. ②The resultant acceleration is one of the factors that affect the conversion between the increase force of performer and the decrease force of patient. ③The cervical lifting manipulation mainly depend on the elbow and combine with the knee,shoulder and waist joint to make overall coordination force. ④Before the lifting manipulation,the neck of the patient is firmly fixed by the right elbow and shoulder of the performer,which could make the overall force.
