目的:观察十全大补汤及拆方四君子汤对BALB/c小鼠CT-26结肠癌原发瘤切除后转移瘤生长及自然杀伤细胞(NK细胞)功能的影响,并探讨可能的作用机制。 方法:构建BALB/c小鼠CT-26结肠癌皮下转移瘤模型,随机分为原发瘤未切除组、原发瘤切除组、十全大补汤组、四君子汤组,每组12只。术前5 d起给予原发瘤未切除组及原发瘤切除组0.9%NaCl溶液灌胃,十全大补汤组灌胃十全大补汤0.52 g/只,四君子汤组灌胃四君子汤0.18 g/只,连续2周。原发瘤切除组、十全大补汤组、四君子汤组给予切除原发瘤治疗。采用Supervision免疫组织化学法检测转移瘤中黏附分子CD49b的表达;流式细胞分析术检测外周血中表达CD49b及CD335的NK细胞在淋巴细胞中所占比例。 结果:转移瘤的发生率为原发瘤切除组75%,原发瘤未切除组58%,十全大补汤组42%,四君子汤组58%。术后2周,十全大补汤组转移瘤重(1.45±0.28)g,明显低于原发瘤未切除组(2.20±0.40)g及原发瘤切除组(2.72±0.32)g,差异有统计学意义(P,<,0.05),而四君子汤组效果较不明显。4组转移瘤体积中,十全大补汤组(1 546.79 ±242.13)mm,3,及四君子汤组(2 079.24 ±234.10)mm,3,均显著低于原发瘤未切除组(2 412.98± 230.42)mm,3,及原发瘤切除组(3 065.06 ±317.02)mm,3,差异有统计学意义(P,<,0.01)。原发瘤未切除组、原发瘤切除组、十全大补汤组、四君子汤组外周血中表达CD49b NK细胞所占比例分别为(20.63±2.99)%、(22.74±1.05)%、(34.34±3.77)%、(28.50±1.65)%,十全大补汤组及四君子汤组明显升高,差异有统计学意义(P,<,0.05);4组中表达CD335 NK细胞所占比例分别为(2.19±0.69)%、(1.90±0.21)%、(3.21±0.19)%、(2.36±0.26)%,十全大补汤组明显升高,与其他3组差异有统计学意义(P,<,0.05)。免疫组化检测显示,十全大补汤组转移瘤中CD49b的表达显著高于其他3组(P,<,0.05)。 结论:原发瘤切除可抑制NK细胞的功能,使黏附分子CD49b及活化受体CD335表达下调,有利于肿瘤细胞逃逸及生长。十全大补汤明显抑制原发瘤切除后转移瘤的生长,可能与其上调CD49b及CD335的表达水平有关。
Objective:To discuss the influence of Shiquan Dabu Decoction and its separated recipe Sijunzi Decoction on the metastatic tumor growth and natural killer(NK)cells in BALB/c mice transplanted with CT-26 colon carcinoma after resection of primary tumor,and study the possible mechanism. MethodsThe subcutaneous metastatic tumor model of CT-26 colon carcinoma was established in BALB/c mice. The mice were randomly divided into the un-resection group of primary tumor,resection group of primary tumor,Shiquan Dabu Decoction group and Sijunzi Decoction group,12 mice in each group. From 5 days before the operation,the mice in the un-resection group and resection group were treated with 0.9% NaCl solution by intragastric administration,each mice in the Shiquan Dabu Decoction group were treated with Shiquan Dabu Decoction at dose of 0.52 g,each mice in the Sijunzi Decoction group were treated with Sijunzi Decoction at dose of 0.18g,with a course of continuous 2 weeks. Except the un-resection group,the resection of primary tumor was performed in the other groups. The expression of adhesion molecule CD49b in metastatic tumor was detected by Supervision immunohistochemistry. The percentage of NK cells expressing CD49b and CD335 in peripheral blood lymphocytes were measured by flow cytometry. Results:The incidence rate of metastatic tumor was 75% in the resection group,58% in the un-resection group,42% in the Shiquan Dabu Decoction group and 58% in the Sijunzi Decoction group. 2 weeks after operation,the weight of metastatic tumor in the Shiquan Dabu Decoction group was obviously lower than that in the un-resection group and resection group ,with statistically significant differences(P,<,0.05),but no significant effect on the weight of metastatic tumor was observed in the Sijunzi Decoction group. The volume of metastatic tumor in the Shiquan Dabu Decoction group [(1 546.79±242.13)mm,3,] and Sijunzi Decoction group [(2 079.24±234.10)mm,3,] was significantly lower than that in the un-resection group [(2 412.98±230.42)mm,3,] and resection group [(3 065.06±317.02)mm,3,],with statistically significant differences(P,<,0.01).The proportion of CD49b expressing NK cells in peripheral blood was(20.63±2.99)% in the un-resection group,(22.74±1.05)% in the resection group,(34.34±3.77)% in the Shiquan Dabu Decoction group and(28.50±1.65)% in the Sijunzi Decoction group. The proportion was obviously increased in the Shiquan Dabu Decoction group and Sijunzi Decoction group,with statistically significant differences(P,<,0.05).The proportion of CD335 expressing NK cells in peripheral blood was(2.19±0.69)% in the un-resection group,(1.90±0.21)% in the resection group,(3.21±0.19)% in the Shiquan Dabu Decoction group and(2.36±0.26)% in the Sijunzi Decoction group. The proportion was obviously increased in the Shiquan Dabu Decoction group,the difference was statistically significant compared with the other groups(P,<,0.05).The results of immunohistochemistry showed that the CD49b expression in the metastatic tumor in the Shiquan Dabu Decoction group was significantly higher than that in the other groups(P,<,0.05). Conclusion:The resection of primary tumor can inhibit the function of NK cells and down-regulate the expressions of CD49b and CD335,which is beneficial to tumor cell escape and growth. Shiquan Dabu Decoction can significantly inhibit the growth of metastatic tumors after resection of primary tumor,which may be associated with the up-regulation of CD49b and CD335 expressions.
