目的:观察九子地黄汤治疗原发性干燥综合征的临床疗效 方法:将60例原发性干燥综合征患者随机分为治疗组与对照组,每组30例。对照组予硫酸羟氯喹片,治疗组予九子地黄汤。两组疗程均为3个月,观察比较临床症状积分、Schirmer试纸湿润长度及泪液细胞因子水平的变化情况 结果:①治疗前后组内比较,两组临床症状积分差异均有统计学意义(P,<,0.05);组间治疗前后临床症状积分差值比较,差异有统计学意义(P,<,0.05)。②治疗前后组内比较,两组Schirmer试纸湿润长度差异均有统计学意义(P,<,0.05);组间治疗前后Schirmer试纸湿润长度差值比较,差异有统计学意义(P,<,0.05)。③治疗前后泪液中细胞因子IL-2、IL-4、IL-10均无法测出。治疗前后组内比较,两组IL-6和INF-γ的阳性检出率差异均有统计学意义(P,<,0.05);组间治疗后比较,IL-6和INF-γ的阳性检出率差异有统计学意义(P,<,0.05) 结论:九子地黄汤治疗原发性干燥综合征,可明显缓解临床症状,增加泪液分泌,其机制可能为通过抑制CD4,+,T细胞的表达来调节免疫、控制炎症。
Objective:To observe the curative effect of “Jiuzi Dihuang Decoction” on primary desiccation syndrome patients Methods:60 diagnosed primary desiccation syndrome patients were divided randomly into treatment group and control group, 30 in each group. The patients of control group were administered with Hydroxychloroquine Sulfate Tablets, while the treatment group with “Jiuzi Dihuang Decoction”. Two groups were treated for 3 months. The changes of the clinical symptom scores, the length of Schirmer test and tear cytokine levels were observed Results:①Within group comparison before and after the treatment, the differences of the clinical symptom scores were statistically significant in both two groups (P,<,0.05); Compared in each group, the difference of the clinical symptom scores before and after the treatment was statistically significant (P,<,0.05). ②Within group comparison before and after the treatment, the differences of the length of Schirmer test were statistically significant in both two groups (P,<,0.05); Compared in each group, the difference of the length of Schirmer test between before and after the treatment was statistically significant (P,<,0.05). ③Tear cytokine IL-2,IL-4,IL-10 could not be detected before and after the treatment. Within group comparison before and after the treatment, the differences of the positive rate of INF-γ and IL-6 were statistically significant in both two groups (P,<,0.05); Between group comparison of the differences of the positive rate of INF-γ and IL-6 between before and after the treatment, the differences were statistically significant (P,<,0.05) Conclusion:“Jiuzi Dihuang Decoction” can both relieve clinical symptoms and increase lacrimal secretion. The mechanism may be modulating the immune system and reducing inflammation by depressing expression of CD4,+,T cell.
