目的:观察复方灵芝健肾汤对糖尿病大鼠肾脏及尿血管紧张素原(AGT)和胰岛素样生长因子-1(IGF-1)表达的影响,探讨其肾脏保护作用及机制。 方法:采用链脲佐菌素(STZ)诱导制作糖尿病肾病大鼠模型,随机分为模型组、给药组,另设空白组。给药组予以复方灵芝健肾汤灌胃,余大鼠给予等量蒸馏水。8周后,观察大鼠一般情况、生化指标,免疫组化及病理切片观察肾脏组织改变,酶联免疫吸附法(ELISA)检测尿AGT和IGF-1的变化。 结果: ①与空白组比较,模型组和给药组大鼠体质量均显著下降(P<0.01),肾/体重指数、生化指标及尿AGT和IGF-1水平均显著升高,差异有统计学意义(P<0.01)。镜下可见大鼠肾小球肥大、系膜基质增生、基底膜增厚,肾小管可见蛋白管型及上皮细胞变性,肾脏AGT和IGF-1表达增加。②与模型组比较,给药组大鼠体质量增加(P<0.01),24 h尿蛋白、血肌酐、尿素氮及尿AGT和IGF-1水平均显著降低,差异有统计学意义(P<0.05);病理改变较模型组明显减轻。 结论:复方灵芝健肾汤对早期糖尿病肾病大鼠有肾脏保护作用,其机制可能与降低肾脏及尿AGT和IGF-1表达,抑制肾脏RAS系统活性有关。
Objective: To observe the clinical efficacy of compound “Lingzhi Jianshen Decoction” on expressions of AGT and IGF-1 in rats with diabetic nephropathy (DN), and to explore its mechanism. Methods Rat models with DN were established by injection of streptozotocin (STZ) and divided into the model group, treatment group and blank group. Treatment group was treated with compound “Lingzhi Jianshen Decoction”, and the others were treated with the same dose of distilled water. After 8 weeks, the general condition and biochemical indexes were observed, and the renal tissues were observed by immunohistochemical and pathological section. Levels of AGT and IGF-1 were detected by ELISA. Results: ① Compared with the blank group, the body weight of the model group and treatment group were reduced markedly (P<0.01), and ratio of kidney wight to BMI, biochemical indexes, levels of AGT and IGF-1 were increased significantly (P<0.01). Pathological examination of kidney in rats showed glomerular hypertrophy, mesangial matrix proliferation, glomerular basement membrane thickening, tubular epithelial cell degeneration, protein cast formation in some renal tubules. The levels of AGT and IGF-1 in the kikdney were markedly increased. ② Compared with the model group, the body weight of the treatment group was remarkably increased (P<0.01), while the levels of 24 albuminuria, BUN, SCr and AGT and IGF-1 were reduced markedly, with significant difference between the two groups (P<0.05). The pathologic changes were much more alleviated in the treatment group than that of the modle group. Conclusion: Compound “Lingzhi Jianshen Decoction” can protect the kidney function of diabetic nephropathy rats. Its mechanism may be association with the reducing expression of AGT and IGF-1 and inhibting activity of RAS.
