目的

基于乌梅丸对白细胞介素(IL)-10、IL-17细胞因子及闭锁小带蛋白-1(ZO-1)、闭锁蛋白(Occludin)等紧密连接蛋白的调控效应,探究其对寒热错杂型变应性鼻炎(AR)大鼠模型的作用机制。
方法

从108只Wistar大鼠中随机抽取空白组(
n
=16)大鼠,其余大鼠采用丙硫氧嘧啶与知母石膏水煎液交替灌胃联合卵清蛋白致敏法,构建寒热错杂型AR病证结合模型。造模成功后的大鼠再随机分为模型组(
n
=14)、氯雷他定组(1.2 mg/kg,
n
=14)及乌梅丸高(2.0 g/kg,
n
=16)、中(1.0 g/kg,
n
=14)、低(0.5 g/kg,
n
=14)剂量组。空白组和模型组灌胃给予0.9%氯化钠溶液,其余组别灌胃给予相应药物,每日1次,持续6周。治疗后记录大鼠行为学评分、酶联免疫吸附分析(ELISA)法检测大鼠血清中IL-10、IL-17水平,Western blot法测定大鼠肠组织及鼻黏膜组织ZO-1、Occludin蛋白表达,剂量效应分析明确乌梅丸不同剂量间的效应差异及最佳有效剂量。
结果

模型组行为学评分显著高于空白组(

0.01);乌梅丸各剂量组及氯雷他定组评分均低于模型组(

0.01)。与空白组比较,模型组大鼠IL-10明显降低,IL-17明显升高(

0.001)。经过乌梅丸高、中、低剂量以及氯雷他定的干预后,各治疗组大鼠血清IL-10浓度与模型组比较均明显升高(

0.001),且乌梅丸高、中剂量及氯雷他定组大鼠血清IL-17浓度与模型组比较均明显降低(

0.01)。剂量效应分析结果显示,随着乌梅丸剂量升高,大鼠行为学评分呈逐渐降低趋势,血清IL-10浓度及肠组织ZO-1、Occludin蛋白表达呈逐渐升高趋势,血清IL-17浓度呈逐渐降低趋势,其中高剂量组效应最优(

结论

乌梅丸可改善寒热错杂型AR大鼠的症状,其机制与下调血清IL-17、上调IL-10及肠组织ZO-1、Occludin蛋白表达有关。
Objective

To explore the therapeutic mechanism of Wumei Wan on a rat model of allergic rhinitis (AR) with the pattern of cold-heat complex based on its regulatory effects on interleukin (IL)-10, IL-17 cytokines and tight junction proteins including zonula occludens-1 (ZO-1) and Occludin.
Methods

Sixteen rats were randomly selected from 108 Wistar rats as the control group. The remaining rats were used to establish a cold-heat complex pattern AR model via alternating intragastric administration of propylthiouracil and the decoction of Anemarrhenae Rhizoma and Gypsum Fibrosum, combined with ovalbumin sensitization. After successful modeling, the remaining successfully modeled rats were randomly divided into the model group (
n
=14), loratadine group (1.2 mg/kg,
n
=14), and high- (2.0 g/kg,
n
=16), medium- (1.0 g/kg,
n
=14), and low- (0.5 g/kg,
n
=14) dose Wumei Wan groups. The control group and model group were administered 0.9% sodium chloride solution via intragastric administration, while the other groups received their respective treatments once daily for 6 consecutive weeks. After treatment, rat behavioral scores were recorded. Serum levels of IL-10 and IL-17 were detected by enzyme-linked immunosorbent assay (ELISA), and the protein expressions of ZO-1 and Occludin in intestinal tissues and nasal mucosa tissues were determined by Western blot. Dose-effect analysis was conducted to clarify the differences in effects among different doses of Wumei Wan and to identify the optimal effective dose.
Results

Behavioral score of the model group was significantly higher than that in the blank control group (
P
<
0.01); the scores of all Wumei Wan groups and the loratadine group were all significantly lower than that of the model group (
P
<
0.01). Compared with the blank control group, the serum IL-10 level of the model group decreased markedly and the IL-17 level increased markedly (
P
<
0.001). After intervention with high-, medium- and low-doses of Wumei Wan and loratadine, the serum IL-10 levels of all treatment groups were obviously higher than that of model group (
P
<
0.01,

0.001) and serum IL-17 in the medium-, high-dose groups of Wumei Wan and the loratadine group were significantly lower than that of the model group (

0.001). Additionally, the protein expressions of ZO-1 and Occludin in intestinal and nasal mucosa tissues of medium-, high-dose Wumei Wan groups and loratadine group were obviously higher than those of the model group (

The dose-effect analysis results showed that as the dosage of Wumei Wan increased, the behavioral score of rats gradually decreased while the serum IL-10 level and the expression of ZO-1 and Occludin proteins in intestinal tissue and nasal mucosa tissues gradually increased, and the serum IL-17 level gradually decreased. Among them, the high-dose group had the best effect (

Conclusion

Wumei Wan can improve the symptoms of cold-heat complex pattern AR in rats. Its therapeutic mechanism is related to downregulating serum IL-17, upregulating serum IL-10, and enhancing the protein expressions of ZO-1 and Occludin in intestinal tissues.
