摘要

伤筋病是骨伤科临床常见病，其核心病机为筋出槽，临床以可触及筋结为特征表现，伴有局部压痛、活动障碍。筋结既是筋出槽局部结构性变化的病理标志，也是临床诊疗的重要靶点。对于筋结的诊治，石氏伤科强调“以手法为先，知其体相，识其部位”，通过触诊精准判定筋出槽的程度与范围，从而为治疗提供依据。詹红生教授在继承石氏伤科学术思想的基础上，结合多年临床实践与基础研究，系统提出“从筋论治”的学术理念，并采用针刺作为“解结”基本方法，取得了较好的临床疗效。

Abstract

Tendon injuries are common in orthopedic clinical practice， with the core pathogenesis being tendon off position， which is clinically characterized by palpable tendon knots， local tenderness， and restricted movement. Tendon knots not only serve as pathological markers of the structural changes in tendon off position but also represent an important therapeutic target in clinical diagnosis and treatment. In the diagnosis and treatment of tendon knots， Shi's traumatology emphasizes "prioritizing manipulation techniques， understanding the body structure and identifying the precise location". Through palpation， it enables accurate determination of the extent and range of tendon misalignment， thereby providing a basis for treatment. Professor Zhan Hongsheng， building upon academic principles of Shi's traumatology and integrating years of clinical practice and foundational research， systematically proposes the academic idea of "treating from the tendon" and uses needling as the fundamental method for "releasing the knot"， achieving significant clinical results.
