      摘要:Emotional stress can stimulate the invasion and metastasis of malignant tumors through various ways. Traditional Chinese medicine (TCM) has accumulated rich clinical experience in preventing and treating emotional diseases, and has formed a complete and unique theoretical system. The advantages of TCM in regulating emotional stress to prevent and treat recurrence and metastasis of malignant tumors are analyzed based on a systematic review, and intervention strategies for emotional stress in patients with malignant tumors are proposed in this paper, aiming to provide reference for further research related to the integrative medicine prevention and treatment of the recurrence and metastasis of malignant tumors.

      摘要:Holistic integrative medicine (HIM) is people-oriented, which facilitates the organic integration of knowledge, theories and practical experience from various fields of medicine based on social, environmental and psychological realities. HIM is developing into a new medical system that is more conducive to human health and disease treatment, and is an important direction for future medical development. The “inflammation-cancer transformation” of chronic gastritis is a problem involving two specialties: gastroenterology and oncology, whose multi-causes, multi-mechanisms, multi-pathways, and multi-stage characteristics bring difficulties and challenges to the scientific prevention and treatment. HIM can help to solve this problem, providing directions and strategies to delay and stop the “inflammation-cancer transformation” in chronic gastritis and reduce the incidence of gastric cancer by integrating multidisciplinary techniques and information.The prevention and treatment of “inflammation-cancer transformation” in chronic gastritis is explored from the perspective of HIM, including the following aspects: ①Collaboration between doctors and patients for lifestyle management from various aspects; ②Dynamic prevention and treatment through the whole process management of “prevention-screening-diagnosis-treatment-rehabilitation”; ③Guidance on the prevention and treatment of the “inflammation-cancer transformation” in chronic gastritis from the integrated perspective of the “gastric microenvironment”; ④The multidisciplinary intersection promotes the new development of integrated prevention and treatment, including mutual promotion of integrated Chinese and western medicine, basic research and clinical diagnosis and treatment, as well as the introduction of modern science and technology.

      摘要:Chronic pancreatitis is an independent risk factor for pancreatic cancer. It is significant to attach great importance to the “inflammation-cancer transformation” of chronic pancreatitis and its prevention and treatment. Based on Li Dongyuan’s theory of yin-fire, this paper puts forward that yin-fire mainly refers to qi stagnation, fire stagnation and damp-heat caused by spleen-stomach qi deficiency, which is the key to the “inflammation-cancer transformation” of chronic pancreatitis. When practicing traditional Chinese medicine intervention in the process of “inflammation-cancer transformation” of the chronic pancreatitis based on the yin-fire theory, it is necessary to recognize its dynamic change and stick to the core mechanism of yin-fire. The maximum clinical benefits could only be achieved by strengthening the spleen and benefiting qi, adjusting treatment according to specific circumstance and attacking pathogens reasonably and appropriately.

      摘要:Surgery is the primary approach to eradicate early-stage non-small cell lung cancer(NSCLC). However, postoperative patients are prone to postoperative complications, lung function decline and other issues, which affect the prognosis and quality of life of patients. Therefore, postoperative rehabilitation of NSCLC is gradually being emphasized. In this paper, we put forward that postoperative rehabilitation of NSCLC can be divided into three stages: rapid rehabilitation, consolidation rehabilitation and rehabilitation for recurrence prevention according to the characteristics of each stage. In the stage of rapid rehabilitation, the pathogenesis encompasses phlegm-fluid retained in the lung, qi stagnation in the middle energizer, and stasis obstructing meridians and collaterals. The treatment should be resolving phlegm and expelling fluid retention, unblocking bowel movement and restoring transformation of the spleen, and removing obstruction in collaterals to relieve pain. In the stage of consolidation rehabilitation, the pathogenesis focuses on liver-lung dysregulation and toxicity due to treatment. The treatment should be regulating qi circulation and reducing toxicity and correcting deviation. In the stage of rehabilitation for recurrence prevention, the pathogenesis centers on stagnation of liver-qi and yin deficiency and yang hyperactivity. The treatment should be dispersing stagnated liver qi for relieving qi stagnation, and nourishing yin for suppressing hyperactive yang.

      摘要:Research progress on epithelial-mesenchymal transition (EMT) in drug resistance of ovarian cancer and intervention of traditional Chinese medicine was reviewed. In recent years, more and more studies have reported that EMT closely related to drug resistance in ovarian cancer, and its related mechanisms mainly involve signaling pathways, exosomes, miRNA, lncRNA, immunity and so on. Some traditional Chinese medicines and their active ingredients play anti-tumor effects and a good ameliorating role in intervening the drug resistance of ovarian cancer caused by EMT, which has potential application value.

      摘要:Breast cancer is a common malignant tumor in women, and the therapeutic effects of traditional Chinese herbal medicines (TCHMs) are satisfactory. By reviewing relevant literature, we summarized the mechanism of action of Chinese herbal monomers against breast cancer in terms of compound properties (flavonoids, terpenoids, alkaloids and others), with the aim of providing a reference for the research on traditional Chinese medicine for breast cancer treatment and the development of new drugs. The main mechanisms of Chinese herbal monomers against breast cancer mainly involve direct cytotoxicity, regulation of the tumor immune microenvironment, increasing sensitivity to targeted drug therapy, and suppor radiotherapy and chemotherapy. Some issues, such as the biological toxicity and developability of certain Chinese herbal monomers, need to be addressed. Research data on the mechanisms of action, development of derivatives, pharmacokinetics, administration methods, and drug safety of Chinese herbal monomers with development potential also need to be supplemented.

      摘要:ObjectiveTo investigate the effect and mechanism of Astragaloside Ⅳ (ASⅣ) in inhibiting colorectal cancer metastasis through regulating exosome secretion.Methods①Cell experiments: MC38 cells were treated with ASⅣ at low, medium and high doses, and cell supernatants were collected to extract exosomes. The morphology of exosomes was observed by transmission electron microscopy (TEM). Nanoparticle tracking analyzer was used to detect exosome density. Immunofluorescence assay and RT-qPCR were used to detect the effect of ASⅣ on the expression of nSMase2 and Rab27a, which were genes related to exosome production and secretion. The effect of ASⅣ on tumor cell migration was detected by cell scrath assay and Transwell assay. ②Animal experiments: A mouse model of colorectal cancer liver metastasis was established. The mice were randomly divided into model group, ASⅣ group and GW4869 group, and were sacrificed after 28 d to observe the liver metastasis. HE staining was used to observe the histopathological changes in the liver tissue, and immunofluorescence was used to detect the expression of nSMase2 and Rab27a in the liver metastases.Results①Cell experiments showed that compared with the condition in control group, ASⅣ intervention did not change the morphology of exosomes but could inhibit exosome secretion from MC38 cells in a concentration- and time-dependent manner (P<0.05); ASⅣ could reduce the mRNA and protein expression levels of nSMase2 and Rab27a in MC38 cells in a concentration-dependent manner (P<0.05); and ASⅣ could inhibit the invasion and migration of MC38 cells in a concentration-dependent manner (P<0.05). ②Animal experiments showed that compared with the condition in model group, ASⅣ significantly reduced the liver tissue mass (P<0.05), decreased the number of liver metastases (P<0.05), and inhibited the protein expression levels of nSMase2 and Rab27a (P<0.05) in the mouse model of colorectal cancer liver metastasis; Moreover, the inhibitory effect of ASⅣ on liver metastasis of colorectal cancer was comparable to that of GW4869, an inhibitor of exosome production (P>0.05).ConclusionASⅣ could inhibit colorectal cancer liver metastasis by regulating exosome production and secretion.

      摘要:ObjectiveTo retrospectively analyze the effect of the Yiqi Yangjing (benefiting qi and nourishing essence) Prescription on the survival prognosis of patients with stage Ⅰ-ⅢA lung adenocarcinoma after radical surgery.MethodsWe included patients with stage Ⅰ-ⅢA non-small cell lung adenocarcinoma after radical surgery who were admitted to the Department of Integrative Medicine, Shanghai Chest Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 1, 2012 to December 31, 2015, and these study subjects were divided into traditional Chinese medicine (TCM) and control groups depending on whether they had received Yiqi Yangjing Prescription-based TCM treatment. The propensity score matching (PSM) was used to exclude the confounding factors and bias, the Kaplan-Meier method was used to compare the overall survival (OS) and disease-free survival (DFS) between the two groups, and the Cox Proportional-Hazards Model was used to analyze the prognostic risk factors.Results①A total of 357 patients with stage Ⅰ-ⅢA lung adenocarcinoma after radical surgery were included. After conducting a 1∶1 propensity score matching procedure, we got 204 successfully matched cases, with 102 cases in each group. ②Before PSM analysis, the 5-year OS rate was higher in the TCM group than that in the control group (79.7% vs. 64.2%, P=0.001), and the 5-year DFS rate was higher than that in the control group (60.6% vs. 47.2%, P=0.005); after PSM analysis, the 5-year OS rate was higher in the TCM group than that in the control group (68.6% vs. 52.5%, P=0.038), and the 5-year DFS rate was higher than that in the control group, but the difference was not statistically significant (49.0% vs 43.1%, P=0.176). ③The Cox univariate analysis after PSM showed that the presence of high-risk pathological factors in histological subtypes (P=0.009) and the administration of TCM (P=0.046) were significantly associated with OS; and the presence of high-risk pathological factors in histological subtypes (P=0.031) and TNM stage (P=0.002) were significantly associated with DFS. ④The Cox multifactorial analysis showed that the presence of high-risk pathological factors (P=0.002) and the administration of TCM (P=0.030) were independent prognostic factors affecting OS in patients with stage Ⅰ-ⅢA lung adenocarcinoma after radical surgery; while the presence of high-risk pathological factors (P=0.027) and clinical TNM stage (P=0.001) were independent prognostic factors affecting DFS in patients with stage Ⅰ-ⅢA lung adenocarcinoma after radical surgery.ConclusionThe treatment effect of prolonging survival can be achieved by using Yiqi Yangjing Prescription-based TCM treatment in addition to Western medicine conventional treatment in patients with stage Ⅰ-ⅢA lung adenocarcinoma after radical surgery, especially those with high-risk pathological factors in histological subtypes.

      摘要:Adolescent idiopathic scoliosis (AIS) is a disease characterized by a three-dimensional curvature and rotation of the spine, whose cause is not yet known. The therapeutic effect of traditional Chinese medicine (TCM) manipulation on AIS is certain, and its specific application should be guided by the concept of syndrome differentiation-based treatment, and follow the rules of holistic treatment, selection of appropriate manipulation according to the disease condition, and manipulation intervention together with Daoyin in the early stage of scoliosis. In addition, the popularization of spinal health knowledge should be boosted to guide adolescents to form good living habits.

      摘要:ObjectiveTo summarize and analyze the evolution history and modern application of rice preparing and processing methods for medicinal use, establish the research basis for rice being used as the medicinal excipient through literature review, and provide literature to support further scientific research, development and utilization of processing methods of rice and rice-washed water.MethodsVarious documentary materials including Chinese medicine classics of previous dynasties, as well as national and local specifications on preparing and processing of traditional Chinese herbal medicines (TCHMs) were reviewed, and related preparing and processing methods recorded therein were summarized and analyzed.ResultsThe application of rice as a medicinal excipient was first recorded in the Jin’gui Yuhan Jing (Classic of the Golden Chamber and Jade Sheath) of the Han Dynasty, which introduced 6 rice preparation methods, 5 rice-washed water preparation methods and many other preparing and processing methods. The current version of the Chinese Pharmacopoeia 2020 edition Ⅰ only contains fried Dangshen (Codonopsis Radix) and fried Banmao (Mylabris) with rice. The national and local specifications on preparing and processing of traditional Chinese herbal medicines only include the rice frying method and the rice-washed water maceration method, which are also found in many other documentary materials. In general, the modern research on rice as a medicinal excipient is considered less than sufficient.ConclusionsAbout rice preparing and processing methods for medicinal use, in ancient times, there were various methods including rice steaming, rice boiling, rice-washed water maceration, rice baking, rice roasting, rice-washed water steaming, rice frying, rice-washed water rinsing, rice grinding, rice juice steaming, rice calcining, rice-washed water stir-frying, rice decocting and starching with ground rice, etc. The modern methods are different from the ancient ones, focusing on the rice frying method and rice-washed water maceration method. Current studies put more emphasis on the preparing and processing techniques of TCHMs with rice. However, the research on rice itself as a medicinal excipient is relatively weak, and there is no mature quality standard available yet.

      摘要:This paper introduces the chief physician Yuan Canxing’s staged treatment experience for Parkinson’s disease (PD) with nourishing the liver and kidney as the main method. It is considered that Yin deficiency of liver and kidney is the basic pathogenic mechanism of PD, which influences the whole disease course. According to Hoehn-Yahr scale, PD can be divided into early, middle and late stages. In the early stage, the focus of nourishing the liver and kidney is to slow down the disease progression. In the middle stage, nourishing the liver and kidney should be accompanied by strengthening the spleen, resolving phlegm, dredging collaterals and removing toxicity for the purpose of increasing efficacy and reducing toxicity. In the late stage, the key point is to improve the quality of survival by nourishing both Yin and Yang. For the treatment of non-motor symptoms of PD, it is also necessary to nourish the liver and kidney. One proven case is presented.

      摘要:This paper introduces the clinical experience of Professor Lu Jin’gen in treating hemorrhoid bleeding. It is considered that hemorrhoid bleeding is often caused by downward flow of damp-heat and excessive heat causing frenetic movement of the blood. The treatment focuses on cooling the blood, clearing heat and drying dampness, which is further summarized as four methods: treating symptoms by cooling the blood to stop bleeding, avoiding future trouble by activating blood circulation to disperse stasis, solving the problem at the source by clearing heat and drying dampness, and strengthening and consolidating constitution by tonifying deficiency. One proven case is presented.

      摘要:This article synthesized the syndrome and treatment methods of limb arthralgia in Taiping Holy Prescriptions for Universal Relief, summarized its etiology, pathogenesis, clinical manifestations and treatment characteristics, and further explored the differentiation and treatment characteristics of limb arthralgia in Taiping Holy Prescriptions for Universal Relief. The book provided 6 kinds of limb arthralgia with prescription analysis. The etiology and pathogenesis emphasized both internal deficiency and external infection. Multiple evils invaded together, which were frequently accompanied by dampness evil, and then the theory of visceral weakness was developed. The pathogenesis and clinical characteristics of each limb arthralgia were discussed in detail. Through the examination of the causes, the complicated disease evidence became orderly. The drugs gradually transferred from relieving superficies and dispelling cold to warming inside and dispelling cold, activating blood, invigorating the spleen, tonifying the kidney. As other methods were used together, the application of insect drugs was emphasized. In Taiping Holy Prescriptions for Universal Relief , the essence of name of limb arthralgia caused by multiple evils, the rich clinical practice of examination of the causes, and the knowledge of treating bi by replenishing the liver and the kidney had very important enlightenment significance for modern rheumatology clinic.

      摘要:ObjectiveTo observe the effect of Chaihu Shugan Powder plus needling Zusanli (ST 36) on serum inflammatory factors and immune indexes in patients with acute pancreatitis.MethodsEighty patients with acute pancreatitis of liver depression and qi stagnation syndrome were randomly divided into the treatment group (n=40) and the control group (n=40). Patients in the control group were given comprehensive internal medicine treatment while patients in the treatment group were treated with Chaihu Shugan Powder plus needling Zusanli (ST36) for five days in addition to the conventional medical treatment. We observed the changes in the levels of serum amylase, serum lipase, inflammatory factors [tumor necrosis factor-α (TNF-α), white blood cell (WBC) count, neutrophil percentage (NE%), C-reactive protein (CRP), interleukin-6 (IL-6), interleukin-8 (IL-8), and interleukin-1β (IL-1β)] and immune indicators (CD3+, CD4+, CD8+, and CD4+/CD8+) before and after treatment (i.e. after 5 d of treatment, same below) in both groups.Results①After treatment, the levels of serum amylase and serum lipase decreased in both groups (P<0.05), and the levels of serum amylase and lipase in the treatment group were lower than those in the control group (P<0.05). ②After treatment, the levels of serum TNF-α, WBC count, NE%, CRP, IL-6, IL-8 and IL-1β decreased in both groups (P<0.05); and the levels of serum TNF-α, CRP, IL-8 and IL-1β in the treatment group were lower than those in the control group (P<0.05). ③After treatment, the serum levels of CD3+, CD4+ and CD8+, and the value of CD4+/CD8+ increased in both groups (P<0.05); and the serum levels of CD3+ and CD4+ in the treatment group were higher than those in the control group (P<0.05). ④During the trial, no significant adverse reactions were observed in both groups, and the treatment had good safety record.ConclusionThe use of Chaihu Shugan Powder plus needling Zusanli (ST36) in addition to conventional medical treatment can better reduce the inflammatory response, regulate the immune status and promote the recovery than the single use of conventional internal medicine treatment in treating patients with acute pancreatitis

      摘要:ObjectiveTo observe the clinical efficacy of Modified Qilong Quyu Mixture plus external treatment and conditioning in the treatment of lower limb venous ulcer of qi deficiency and blood stasis syndrome.MethodsOne hundred patients with lower limb venous ulcer of qi deficiency and blood stasis syndrome were randomly divided into treatment group (n=50) and control group (n=50). Patients in the control group were given external treatment and conditioning plus oral administration of Maixuekang Capsules, and patients in the treatment group were given external treatment and conditioning plus oral administration of Modified Qilong Quyu Mixture. The course of treatment in both groups was 8 weeks. The clinical efficacy was observed, and the changes in trauma area, topical traditional Chinese medicine (TCM) syndrome score and the visual analogue scale (VAS) score of pain were compared between the two groups before and after treatment.Results①During the trial, 4 cases dropped or got removed from the treatment group, and 5 cases dropped or got removed from the control group. Finally, 46 cases in the treatment group and 45 cases in the control group completed the trial. ②The total effective rate of the treatment group and control group was 76.1% and 66.7%, respectively, and the clinical efficacy of the treatment group was better than that of the control group (P<0.05). ③After treatment, the trauma area of patients became smaller in both groups (P<0.05); and the trauma area of patients in the treatment group was smaller than that in the control group (P<0.05). ④After treatment, the score in the items including peritraumatic pigmentation, lack of new epithelial cells, exudation, dull or dark purple granulation tissue, pus decay and peritraumatic swelling and the total score of topical TCM syndrome decreased in the treatment group (P<0.05), while in the control group only the score in exudation, dull or dark purple granulation tissue and peritraumatic swelling decreased (P<0.05); After treatment, the score in the items of peritraumatic pigmentation, lack of new epithelial cells, exudation and dull or dark purple granulation tissue, and the total score of topical TCM syndrome in the treatment group were lower than those in the control group (P<0.05). ⑤The pain VAS score decreased in both groups after treatment (P<0.05); After treatment, the pain VAS score was lower in the treatment group than that in the control group (P<0.05).ConclusionModified Qilong Quyu Mixture plus external treatment and conditioning has achieved a satisfactory therapeutic result in treating lower limb venous ulcer of qi deficiency and blood stasis syndrome since the combined therapy can effectively improve the topical traumatic symptoms, alleviate pain and promote wound healing.

      摘要:ObjectiveTo screen the exogenous regulators that can induce resistance and stimulate Ginseng Radix et Rhizoma plants to produce secondary metabolites under artificial planting conditions.MethodsBased on the inducible plant defense response, 2-year-old, 3-year-old and 4-year-old Ginseng Radix et Rhizoma plants were treated with exogenous substances such as salicylic acid, chitosan and amino acids. Ginseng Radix et Rhizoma plants were collected and the contents of ginsenosides Rg1, Re, Rb1, Rb2, Rb3 and Rd in their main roots were determined by ultra high performance liquid chromatography with diode array detector (UPLC-PDA). The changes of fresh weight and ginsenoside content of single plant before and after treatment were observed.ResultsThe growth/differentiation balance hypothesis (GDBH) was verified, suggesting that there is a contradiction between yield increase and high quality, and exogenous substances such as amino acids, salicylic acid and chitosan can induce plant defense response, start secondary metabolism and promote ginsenoside accumulation.ConclusionThe compound nutrition of amino acids, salicylic acid and chitosan is the optimal exogenous regulator to effectively improve the fruit quality of cultivated Ginseng Radix et Rhizoma.

      摘要:In this paper, the theoretical basis of tendon nodules is organized and discussed in terms of connotation, distribution and physiopathological characteristics, and the current clinical application of the theory of tendon nodules in the treatment of diseases of the motor, digestive, neurological and circulatory systems are reviewed to further summarize the pathogenesis of meridian-tendon diseases and provide reference materials for the clinical treatment of tendon nodules and related diseases.
