      摘要:The formation of the tumor microenvironment (TME) directly mediates tumor progression and metastasis, exacerbating the risk of death in cancer patients. Targeting TME has become the key to the treatment of cancer. The significant clinical benefit of immune checkpoint inhibitors (ICIs) is a successful example of tumor metastasis suppression by remodeling the TME, but there are still challenges such as adverse events, drug resistance, and poor response rates in most cancer patients. Traditional Chinese medicine deals with tumor treatment from a holistic perspective. With multi-target, multi-component and multi-directional effects, traditional Chinese herbal medicines (TCHMs) can intervene in the entire process of tumor metastasis by reversing immunosuppression, reducing the inflammatory microenvironment and remodeling TME metabolism, and TCHMs have played a non-negligible role in overcoming drug resistance, preventing metastasis, improving quality of survival and prolonging life, etc.

      摘要:We aim to discuss the inflammatory-cancer transformation of pulmonary nodules and traditional Chinese medicine prevention and treatment strategies by reviewing the mechanism of pulmonary nodule development. The key to the inflammation-cancer transformation of pulmonary nodules is latent toxicity due to vital qi deficiency. Therefore, the important way to prevent the development of lung cancer is to support vital qi and expel pathogens. One proven case is provided.

      摘要:Based on the theory of “normal circulation of genuine qi in five zang-organs” in the Synopsis of the Golden Chamber, this paper further explained the theoretical basis of “smoothing circulation of body to cure tumor”. Furthermore, it advocated to maintain “tong state” in the body as the guidance, and deeply explored the advantages of traditional Chinese medicine theory in the cognition of malignant tumors, with the aim on breaking through the existing bottleneck and mastering the initiative of tumor prevention and treatment.

      摘要:The clinical research progress on classic famous prescriptions combined cold and heat in treatment of malignant tumors was reviewed. In the Treatise on Cold Pathogenic Diseases, there were many prescriptions combined cold and heat, such as Xiexin Decoction category, Chaihu Decoction category, Chengqi Decoction category, etc. Clinical practice had shown that classic famous prescriptions of traditional Chinese medicine combined cold and heat could improve the postoperative complications of patients in treating malignant tumors, in addition, those classic famous prescriptions combining with chemotherapy could increase the effectiveness and reduce the toxicity, improve the quality of life, and prolong the survival period.

      摘要:Epidermal growth factor receptor (EGFR) mutation is a common carcinogenic factor in non-small cell lung cancer (NSCLC). The emergence of epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs) has brought new hope to patients, but acquired drug resistance is a major problem that limits the efficacy. By summarizing the research on traditional Chinese medicine (TCM) improving acquired resistance to EGFR-TKIs in NSCLC patients, analyzing the characteristics and shortcomings of the current research, and exploring the underlying TCM pathogenesis, we propose that “toxic pathogen as the cause” and “healthy qi being in deficiency” are the key factors and we prospect about the future research work to provide diagnostic ideas and medication basis for the TCM treatment of acquired resistance to EGFR-TKIs in NSCLC patients.

      摘要:ObjectiveTo establish and evaluate a nomogram prediction model for traditional Chinese medicine(TCM) syndromes of menopause-like syndrome in patients who have undergone endocrine therapy after breast cancer surgery.MethodsAccording to the standard of TCM syndrome differentiation, patients with menopause-like syndrome after endocrine therapy for postoperative breast cancer were divided into Chongren imbalance syndrome, liver-qi stagnation syndrome, toxic heat accumulation syndrome and qi and blood deficiency syndrome. The general data such as tumor TNM stage, sex hormone receptor types, endocrine therapy duration, related laboratory indicators, Karnofsky scores and Kupperman improved symptom scores were analyzed and compared. On this basis, a nomogram prediction model of TCM syndromes was established, and a model calibration curve and a receiver operating characteristic (ROC) curve were made to evaluate the model.Results①A total of 180 patients with menopause-like syndrome after endocrine therapy for postoperative breast cancer surgery were included, including 54 cases of Chongren imbalance syndrome, 34 cases of liver-qi stagnation syndrome, 44 cases of toxic heat accumulation syndrome, and 48 cases of qi and blood deficiency syndrome. ②There were significant differences in tumor TNM stage, sex hormone receptor types, endocrine therapy duration, serum estrogen (E2) level, Kupperman improved symptom total score, and each sub-score including urinary tract infection and sexual life status among patients with different TCM syndromes (P<0.05 ). ③The predictive variables of Chongren imbalance syndrome were TG and tumor stage. The predictive variables of liver-qi stagnation syndrome were E2 level and tumor stage. The predictive variable of toxic heat accumulation syndrome was endocrine therapy duration. The predictive variables of qi and blood deficiency syndrome were bone and joint pain, tumor stage and sex hormone receptor. The nomogram prediction model of each TCM syndrome was established by using the above predictive variables. According to the comprehensive evaluation of ROC curve and calibration curve of the prediction model, the above-mentioned prediction model of each TCM syndrome showed good predictive ability, calibration ability, and high diagnostic value.ConclusionsThe nomogram for predicting TCM syndromes of menopause-like syndrome after endocrine therapy for postoperative breast cancer can visualize the relevant predictive variables. The establishment of TCM syndrome nomogram prediction model based on predictive variables can predict the incidence of each TCM syndrome, and the nomogram has reference value for the diagnosis of TCM syndromes.

      摘要:ObjectiveTo observe the clinical efficacy and safety of combining chemotherapy with Sanleng Xiaoliu Mixture in the treatment of advanced gastric cancer of intermingled phlegm and stasis syndrome.MethodsNinety-two patients with advanced gastric cancer of intermingled phlegm and stasis syndrome were divided into treatment group (n=44) and control group (n=48) according to their wishes. The control group was given chemotherapy (fluorouracil as the basic regimen), while the treatment group was treated with Sanleng Xiaoliu Mixture in addition to the treatment measures of the control group. A cycle of chemotherapy is 21 days. The basic course of chemotherapy was 4 cycles in both groups, and the follow-up treatment could be continued to 6-8 cycles as appropriate. At the end of the basic course of treatment, syndrome therapeutic evaluation and solid tumor evaluation were conducted, and the changes in Karnofsky Performance Status (KPS) score were compared. The chemotherapy cycles were recorded, and the changes in TCM syndrome scores before treatment and at the 22nd and 85th days of treatment were compared between the two groups, as well as the incidence of toxic reactions during the basic course of treatment.Results①The total effective rate according to the TCM syndrome therapeutic evaluation was 77.3% in the treatment group and 50.0% in control group; the TCM syndrome therapeutic evaluation in the treatment group was better than that in the control group (P<0.05). ②The difference in the efficacy for solid tumors between the two groups was not statistically significant (P>0.05). ③In the treatment group, the scores of epigastralgia, nausea and vomiting, poor appetite, heavy sensation in the body and limbs and the total TCM syndrome score decreased on day 22 of treatment (P<0.05),while the total TCM syndrome score decreased in the control group(P<0.05).And the differences in the scores of epigastralgia, nausea and vomiting, poor appetite, heavy sensation in the body and limbs and total TCM syndrome score were statistically significant on day 85 of treatment between the two groups(P<0.05). ④After treatment, the KPS score increased in both groups (P<0.05); and the KPS score was higher in the treatment group than that in the control group (P<0.05). ⑤There was no statistically significant difference between the two groups in terms of the completion of chemotherapy cycles (P>0.05). ⑥The incidence of peripheral neurotoxicity in the treatment group was lower than that in the control group (P<0.05), but the difference in the incidence of hematologic toxicity and gastrointestinal toxicity between the two groups was not statistically significant (P>0.05).ConclusionSanleng Xiaoliu Mixture with chemotherapy has a good clinical effect on patients with advanced gastric cancer of intermingled phlegm and stasis syndrome as it can significantly improve TCM symptoms and quality of life of patients, and reduce the toxic reaction of chemotherapy.

      摘要:This paper summarizes professor Li Xiangyun’s experience in treating uterine leiomyoma from the aspects of etiology, pathogenesis and experience of differential treatment. Professor Li puts forward the“three steps gradual method”(breaking blood stasis and dispersing knot, softening hard and dispersing knot, promoting blood circulation and diuresis) to treat uterine leiomyoma based on the method of analogy. He is good at distinguishing body and regulating week, using drug pair, and creating “Xiaozheng Decoction” to treat uterine fibroid, so the curative effect is remarkable. One proven case is presented.

      摘要:ObjectiveTo analyze the distribution and pharmacological properties of anti-inflammatory and anti-tumor traditional Chinese herbal medicines based on data mining.MethodsTotally 481 clinically used traditional Chinese herbs recorded in Chinese Materia Medica were included for research, and Chinese herbs with anti-inflammatory and anti-tumor effects were screened out through text data analysis and literature retrieval from Chinese and English databases. The database was established, and frequency analysis, analysis of herbal property, flavor and meridian tropism, and co-occurrence analysis were used to find the characteristics of anti-inflammatory and anti-tumor herbs. KH Coder 3.0 was used to visualize and analyze the features of word frequency, part of speech, and inter-word relationships in the text data.Results①A total of 368 anti-inflammatory herbs, 321 anti-tumor herbs and 286 anti-inflammatory and anti-tumor herbs were screened out. ②The classifications of herbs in the 3 groups were similar as each group involves 21 herbal categories. The proportion of herbs with cool/cold property was comparable to that of herbs with warm/heat property. Most herbs had bitter, sweet and/or pungent flavors and entered the liver, lung, stomach, spleen, and/or kidney meridians. The property-flavor combinations of bitter-cold, bitter-warm and pungent-warm were more in frequency. ③Visualization analysis revealed that there were different clustering correlations among the three groups of herbs.ConclusionsThere is a high degree of overlap between anti-inflammatory herbs and anti-tumor herbs, and there are multiple potentially relevant common pathways between the two groups of herbs. The different stages of inflammation-cancer transformation and syndrome-treatment characteristics of herbal medicines may be correlated.

      摘要:ObjectiveTo study the difference of autophagy histone code expression in myelodysplastic syndrome (MDS) patients with different traditional Chinese medicine (TCM) syndromes.MethodsAccording to the relevant criteria, MDS patients were divided into healthy qi deficiency and toxin accumulation (HQDTA) group, toxin exuberance and healthy qi deficiency (TEHQD) group, and healthy people were set as the healthy control group. The laser scanning confocal microscope was employed to observe and compare the autophagic vesicles of bone marrow mononuclear cells (BMMNCs) in each group, flow cytometry was used to detect the autophagic level of BMMNCs, and Western blot was used to detect the histone code expression of BMMNCs.Results①The number of autophagic vesicles of BMMNCs decreased in the HQDTA group and TEHQD group compared with that in the healthy control group (P<0.05). The number of autophagic vesicles of BMMNCs was further reduced in the TEHQD group compared with that in the HQDTA group (P<0.05). ②Compared with those in the healthy control group, the expression levels of methylation modifications (H3K27me, H4K16me) of BMMNCs increased (P<0.05) and the expression levels of acetylation modifications (H3K4ac, H3K9ac) of BMMNCs decreased (P<0.05) in the HQDTA group. Compared with those in the healthy control group, the expression levels of methylation modifications of different H3 and H4 sites of BMMNCs increased (P<0.05) and the expression levels of acetylation modifications (H3K4ac, H3K9ac, H3K27ac, H3K56ac, H4K16ac) decreased (P<0.05) in the TEHQD group.ConclusionsThe autophagy levels and histone modification levels of BMMNCs are different between MDS patients and healthy people. The high methylation and low acetylation levels at different sites of H3 and H4 participate in the development and progression of MDS by down-regulating the cellular autophagy levels.

      摘要:ObjectiveTo observe the effect of Shenling Baizhu Powder (SLBZ) on intestinal mucosal barrier in mice with xenograft model of colorectal cancer after chemotherapy.MethodsThirty-two SPF grade BALB/c mice were subcutaneously inoculated with CT26 colorectal carcinoma cells, and after tumor formation they were randomly divided into four groups (n=8 each): model group, oxaliplatin group, SLBZ group, and SLBZ plus oxaliplatin group. Mice in the model group were given distilled water by gavage (0.2 mL, once daily), mice in the oxaliplatin group were treated with intraperitoneal injection of 5 mg/kg oxaliplatin (0.2 mL, once every 3 days), mice in the SLBZ group were treated with 15.6 g/kg SLBZ by gavage (0.2 mL, once daily), and mice in the SLBZ plus oxaliplatin group were treated with 15.6 g/kg SLBZ by gavage (0.2 mL, once daily) and intraperitoneal injection of 5 mg/kg oxaliplatin (0.2 mL, once every 3 days). The treatment lasted for 18 days. At the end of treatment,8 mice in each group were selected for serum collection, and the enzyme-linked immunosorbent assay (ELISA) was used to detect serum diamine oxidase (DAO), D-lactic acid (D-LA), interleukin-17 (IL-17), intestinal alkaline phosphatase (IAP) and intestinal mucosal secretory immunoglobulin A (sIgA). The tumors and spleens of the mice were excised and weighed. Hematoxylin and Eosin (HE) staining and Immunohistochemistry (IHC) staining were used to analyze the effect of SLBZ on intestinal mucosa of mice in histomorphology, and flow cytometry was used to detect the changes of immune microenvironment in tumor tissues.ResultsCompared with the tumor growth condition in the model group, the tumor growth was inhibited in SLBZ group and oxaliplatin group, and significantly inhibited in SLBZ plus oxaliplatin group (P<0.05). Oxaliplatin treatment alone significantly decreased the expression of IAP (P<0.05), and significantly increased the expression of D-LA (P<0.05) and DAO (P<0.05). The results of HE staining showed that SLBZ could significantly repair the histological arrangement of colonic villi after chemotherapy and reduce the occurrence of inflammatory reactions. The results of IHC staining showed that SLBZ could significantly increase the expression of colonic tight junction proteins (claudin), occludin and zonula occludens-1 (ZO-1) in colon of mice after chemotherapy. Compared with the effect of oxaliplatin treatment alone, SLBZ plus oxaliplatin could alleviate the intestinal mucosa barrier damage, and repair the chemotherapy-induced intestinal mucosal barrier damage to a certain extent. Meanwhile, SLBZ could increase the CD8+ T cell infiltration in the tumor microenvironment.ConclusionSLBZ can protect the intestinal mucosal barrier after chemotherapy and improve the tumor immune microenvironment, thereby inhibiting the growth of subcutaneously xenograft tumors in mice.

      摘要:ObjectiveTo study the application rules of stimulation parameters of electroacupuncture and transcranial electrical stimulation (TES) for dementia with data mining techniques.MethodsBoth Chinese databases including Chinese National Knowledge Infrastructure (CNKI), VIP Database for Chinese Technical Periodicals (VIP), WanFang Data, China Biology Medicine (CBM) and English databases including PubMed, Web of Science, Embase, and Cochrane Library were systematically searched for clinical research literature related to electroacupuncture and TES for dementia which were published from the establishment of databases up to May 2022. The quality of randomized controlled trials in the included literature was assessed using the Cochrane 5.4 risk of bias tool. A database of electrical stimulation parameters was established and data mined using descriptive analysis, association rules, and cluster analysis.Results①Finally, 175 articles were included, of which 143 were randomized controlled trials, containing data from 179 groups. ②Overall, the frequently used pulse waveforms were continuous wave (57 times, 50.44%) and disperse-dense wave (44 times, 38.94%), and electrical stimulation frequency was 300 to 500 times/min (5.00-8.33 Hz) when using electroacupuncture and TES for dementia. The electrical stimulation intensity of electroacupuncture therapy was often determined by patient tolerance, and a single treatment session was 30 min. The stimulation intensity of TES therapy was 2 mA, and a single treatment session was 20 min. ③The commonly used stimulation parameters of electroacupuncture and TES for vascular dementia (VaD) were consistent with literature data in general, while those for Alzheimer’s disease (AD) were different from literature data: the frequently used pulse waveform was continuous wave, and the electrical stimulation frequency was 10/50 Hz. The electrical stimulation intensity was determined by patient tolerance for electroacupuncture, while that of TES was 2 mA, and a single treatment session often lasted 30 min for both electroacupuncture and TES. ④The most frequently selected acupoints were Baihui (DU20), Sishencong (Ex-HN01), Fengchi (GB20), Shenting (DU24), and Benshen (GB13), and the most frequently selected meridians were Du meridian, gallbladder meridian and stomach meridian. ⑤According to the association rule analysis, for electroacupuncture treatment of dementia, the most frequently used acupoint-pair was Baihui (DU20)-Shenting (DU24), 3-acupoint-combination was Sishencong (Ex-HN01)-Fengchi (GB20)-Baihui (DU20), and 4-acupoint-combination was Baihui (DU20)- Shenting (DU24)- Fengchi (GB20)- Sishencong (Ex-HN01). The cluster analysis also revealed that among the clusters formed by common acupoints, the pair of Fengchi (GB20) and Sishencong (Ex-HN01), and that of Baihui (DU20) and Shenting (DU24) were in two separate clusters.ConclusionsContinuous waves are commonly used at a frequency of 300 to 500 times/min (5.00-8.33 Hz) for electroacupuncture and TES for dementia. The electrical stimulation intensity of electroacupuncture is often determined by the patient tolerance, and a single treatment session lasts 30 min. The electrical stimulation intensity of TES therapy is 2 mA, and a single treatment session is 20 min. The frequently used acupoints for electroacupuncture and TES for dementia are Baihui (DU20), Sishencong (Ex-HN01), Shenting (DU24), and Fengchi (GB20), which were closely compatible with each other.

      摘要:ObjectiveTo analyze the prescription formulation rules of traditional Chinese medicine (TCM) and the molecular mechanism of relevant traditional Chinese herbal medicines (TCHMs) for the treatment of gout based on a data mining analysis and a network pharmacology approach.MethodsWith Shanghai Guanghua Hospital of Integrated Traditional Chinese and Western Medicine as the initiator, consultation data of gout patients from 22 hospitals in 10 provinces (including autonomous regions and municipalities directly under the central government) across China were retrospectively analyzed to extract information on the names, tastes, properties, meridian tropism and effects of traditional Chinese herbal medicines (TCHMs). Microsoft Excel 2019 was used for analysis of frequency and relative frequency, and SPSS Modeler 18.0 and SPSS Statistics 20.0 were used for association rules and systematic cluster analysis of high-frequency TCHMs. Based on the data mining results, the active components of TCHMs were obtained from databases including Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), Integrative Pharmacology-based Research Platform of Traditional Chinese Medicine (TCMIP), Encyclopedia of Traditional Chinese Medicine (ETCM) and Herb, and they were filtered thereafter. The Canonical SMILES format of active components of TCHMs was obtained from PubChem and then imported into SwissTargetPrediction database to predict the target genes of the active components. The disease genes associated with gout were retrieved from databases including GeneCards, Therapeutic Target Database (TTD), DisGeNET data, Online Mendelian Inheritance in Man (OMIM) and Pharmacogenetics and Pharmacogenomics Knowledge Base (PharmGKB), and then intersect analysis was conducted to obtain common genes. Cytoscape 3.7.2 was used to obtain the core genes for the treatment of gout with TCHMs, perform pathway analysis and association analysis, and construct a “herb-component-gene-signaling pathway” network.Results①In this study, clinical data of 2,688 gout patients were collected, and 659 prescriptions involving 266 TCHMs were obtained, including 36 high-frequency TCHMs [Tufuling (Smilacis Glabrae Rhizoma), Yiyiren (Coicis Semen), Chuanniuxi (Cyathulae Radix), Cangzhu (Atractylodis Rhizoma), Huangbo (Phellodendri Chinensis Cortex), Qinjiao (Gentianae Macrophyllae Radix), Duhuo (Angelicae Pubescentis Radix), and Weilingxian (Clematis Radix et Rhizoma), etc.]. ②TCHMs for the treatment of gout were mainly sweet, bitter and pungent in taste, and cold, neutral and warm in nature, manifesting their therapeutic effects on the liver, lung, stomach, spleen and kidney meridians in preference, with the effects of clearing heat, tonifying deficiency, promoting diuresis and defusing dampness, activating blood circulation and resolving stasis and dispelling wind-dampness. ③The association rules showed that the medicinal combinations commonly used in the clinical treatment of gout were Tufuling (Smilacis Glabrae Rhizoma)-Yiyiren (Coicis Semen) and Cangzhu (Atractylodis Rhizoma)-Huangbo (Phellodendri Chinensis Cortex), etc. According to systematic cluster analysis, 6 new medicinal combinations were formed and the core prescription for gout might include Dilong (Pheretima), Biejia (Trionycis Carapax), Qinjiao (Gentianae Macrophyllae Radix), Chuanniuxi (Cyathulae Radix), Yiyiren (Coicis Semen) and Tufuling (Smilacis Glabrae Rhizoma). ④The results of the network pharmacological analysis showed that baicalin, wogonin, quercetin, and kaempferol might be the key components for the treatment of gout, which could act on genes such as tumor necrosis factor (TNF), serine/threonine kinase 1 (AKT1), interleukin-6 (IL-6) and sarcoma (SRC), etc., and these components exerted their therapeutic effects through regulating apoptosis, transforming growth factor-β (TGF-β), and other multiple pathways.ConclusionsTCM treatment of gout follows the principle of treating both the manifestations and the root cause. The main method is clearing heat and tonifying deficiency, which is supported by promoting diuresis and defusing dampness, as well as activating blood circulation and resolving stasis. The treatment of gout with TCHMs is mainly achieved by acting on key genes such as TNF, AKT1, IL-6 and SRC and by regulating apoptosis, necroptosis, TGF-β and other signaling pathways.

      摘要:Through the systematic study of ancient literature in the past dynasties, the historical evolution and key information of Sijunzi Decoction were summarized and analyzed from six aspects: prescription origination and historical development, prescription name and composition, formula analysis and function, origin and processing of the contained herbs, prescription dosage, decoction method and usage. It provided a theoretical basis of the development and utilization of classic famous prescription Sijunzi Decoction.

      摘要:Through historical research and modern literature research on the effects of Foshou Powder (Angelicae Sinensis Radix and Chuanxiong Rhizoma) in treating fetal unsteadiness, the bidirectional effect of Foshou Powder(“Stillborn could be delivered, live could be safe”) was discussed. The study provided theoretical basis for the bidirectional effect of Foshou powder in the treatment of fetal unsteadiness.

      摘要:Based on the theory of “brain-heart-kidney-essence chamber” axis, the pathogenesis and medication of benign prostatic hyperplasia were discussed. Benign prostatic hyperplasia was a disorder of the essence chamber, the direct cause was the blockage of the essence chamber and the failure to open and close the essence chamber. Dysregulation of brain and heart, and absent-mindedness were important factors. The root of the problem was kidney deficiency and malfunction of qi. Under the medication guidance of the "brain-heart-kidney-essence chamber" axis, we advocated the treatment concept of “mind and body in tune” and “overall and local treatment together”. The treatment method could be adopted to dredge the essence chamber, regulate the heart and brain, and nourish the kidney qi. The main drug combinations were tri-herbal medicines of Astragali Radix-Typhae Pollen-Plantaginis Semen, Polygalae Radix-Salviae Miltiorrhizae Radix Et Rhizoma-Poria, Cinnamomi Cortex-Rubi Fructus-Rosae Laevigatae Fructus. The overall regulation of brain, heart, kidney and essence chamber could promote the recovery of benign prostatic hyperplasia. And one proven case was presented.
