摘要:The nervous system and the gastrointestinal system are closely connected physiologically and pathologically. The proposed theory of brain-gut axis further confirms the strong correlation between the brain and the gastrointestinal system. An increasing number of studies has indicated that gut microbiota can influence the development and progression of Alzheimer’s disease by acting on the brain-gut axis. Based on the theory of traditional Chinese medicine (TCM) and the microbiota-brain-gut axis, we propose the approach of treating the brain from the intestine, and discuss the strategy of applying TCM (including traditional Chinese herbal medicine, acupuncture and moxibustion) to the treatment of Alzheimer’s disease in order to provide reference for clinical TCM treatment.

摘要:According to the theory of concurrent pathogens in Shi’s traumatology theoretical system, the pain commonly seen in orthopedics and traumatology is prolonged and difficult to heal as a result of the concurrent “phlegm-dampness” pathogens flowing into the vessels and collaterals. Therefore, the treatment method of resolving phlegm-dampness and dredging collaterals is proposed and Niubangzi Decoction is applied to the treatment with good effects. Professor Shi Yinyu, the fourth-generation successor of Shi’s traumatology, believes that the contemporary concurrent pathogens have gradually changed from “phlegm-dampness” to internal heat due to yin deficiency or damp-heat, and has developed the heat pathogen theory based on the theory of concurrent pathogens, proposing that the methods of clearing heat, removing toxicity and cooling blood should be added to the treatment principles accordingly. One proven case is provided.

摘要:Psychocardiology is an interdisciplinary disease concept developed from the “biopsychosocial model” in recent years. One of the important theories proposed in the Inner Canon of Huangdi is that “the heart governs mental activities”, which can be used to guide the treatment of psychocardiological diseases. We summarized the connotation of “the heart governing mental activities”, discussed the connection and differences between the specific theory and psychocardiological diseases, and provided a comprehensive analysis of the current status of traditional Chinese medicine treatment and research on psychocardiological diseases based on “the heart governing mental activities” in order to provide reference for clinical practice.

摘要:ObjectiveTo systematically evaluate the clinical efficacy of Xuefu Zhuyu Decoction (XFZYD) for post-stroke depression.MethodsThe literature related to the XFZYD treatment of post-stroke depression (randomized controlled trials) was retrieved from databases including Chinese National Knowledge Infrastructure (CNKI), VIP Chinese Science Journals Database (VIP), WanFang Data, Chinese Biomedical Literature Service System (SinoMed), Embase, Pubmed, Cochrane Library and Web of Science. The collected literature was published from the establishment date of each database up to July 10, 2022. The quality of the included literature was evaluated with the Manual Cochrane 5.4.1, and RevMan 5.4.1 was used to perform meta-analysis.ResultsThe included 19 articles in the study involved 1,352 patients, with 679 patients in the observation group and 673 patients in the control group. Meta-analysis results showed that the clinical efficiency of the observation group was higher than that of the control group [OR=4.41,95%CI(3.02,6.44),P<0.000 01]; After treatment, observation group had lower scores than control group in Hamilton depression scale (HAMD) [MD=-4.98,95%CI(-6.12,-3.83)], NIH stroke scale (NIHSS) [MD=-4.09,95%CI(-4.74,-3.45)], and activity of daily living scale (ADL) [MD=-4.19,95%CI(-8.16,-0.22)], P<0.000 01; And the incidence of adverse reactions in the observation group was lower than that in the control group [OR=0.07,95%CI(0.02,0.26),P<0.000 01].ConclusionThe combined use of XFZYD with conventional western medicine has more satisfactory efficacy than the single use of conventional therapy for post-stroke depression, and can better improve clinical symptoms and patients’ activity of daily living with good safety; However, the relevant findings need further validation due to the quantity and quality of included literature.

摘要:ObjectiveTo investigate the factors influencing sleep disorders and the distribution characteristics of traditional Chinese medicine (TCM) syndromes in patients with early- or mid-stage Parkinson’s disease (PD).MethodsBased on the Parkinson’s Disease Sleep Scale (PDSS) score, 87 patients with early- or mid-stage PD were divided into the PD sleep disorder group (n=46) and the PD without sleep disorder group (n=41). Baseline data such as gender, age, age of onset, duration of illness, Hoeln-Yahr Stage, and scores of the Unified Parkinson Disease Rating Scale (UPDRS), Scales for Outcomes in Parkinson’s disease - Autonomic (SCOPA-AUT), Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA), Minimum Mental State Examination (MMSE) and the Parkinson’s Disease Questionnaire (PDQ-39) were analyzed and compared between the two groups. All patients with early- or mid-stage PD had TCM syndrome differentiation, the distribution pattern of TCM syndromes were analyzed, and the sleep condition in patients with different TCM syndromes were compared. Linear regression analysis or Logistic regression analysis was used to test for linear trends between the total PDSS score and each variable, and to explore the influencing factors of sleep disorders in early- or mid-stage PD patients.Results①No statistically significant baseline difference was found between the two groups (P>0.05). The scores of SCOPA-AUT, HAMA, HAMD, PDQ-39 and subscales of UPDRS (I mentation, behavior and mood, Ⅱ activities of daily living, Ⅲ motor exam) were higher in the PD with sleep disorder group than those in the PD without sleep disorder group (P<0.05). ②Among the 87 patients with early- or mid-stage PD collected in this study, there were 22 cases with qi and blood deficiency syndrome, 21 cases with qi stagnation and blood stasis syndrome, 15 cases with stirring wind due to phlegm-heat syndrome, and 29 cases with liver and kidney deficiency syndrome. There was a statistically significant difference in the distribution of TCM syndromes between the PD with sleep disorder group and the PD without sleep disorder group (P<0.05). The most common TCM syndrome in the PD with sleep disorder group was liver and kidney deficiency syndrome (43.50%) while that in the PD without sleep disorder group was qi stagnation and blood stasis syndrome (41.50%). ③The differences in the total PDSS scores of early- or mid-stage PD patients with different TCM syndrome were statistically significant (P<0.05), and the total PDSS score in patients with the liver and kidney deficiency syndrome was the lowest. ④The total PDSS score was negatively correlated with age, UPDRS Ⅰ, UPDRS Ⅱ, UPDRS Ⅲ, SCOPA-AUT, HAMA, HAMD and PDQ-39 scores (r<0, P<0.05), and UPDRS Ⅰ, UPDRS Ⅱ, SCOPA-AUT, HAMA, HAMD, and PDQ-39 were risk factors (P<0.05).ConclusionsOlder patients with early- or mid-stage PD are more likely to have sleep disorders, more severe motor and autonomic symptoms, as well as anxiety and depression, affecting their quality of life. The most common TCM syndrome of early- or mid-stage PD patients with sleep disorders is liver and kidney deficiency syndrome, and patients with this syndrome have the most serious sleep disorder.

摘要:Professor Guo Shikui’s clinical experience in treating Takayasu arteritis from the perspective of stasis is introduced. As stasis and obstruction of vessels and collaterals is considered as the key pathogenesis of Takayasu arteritis, activating blood for removing obstruction in vessels is used as the basic treatment method. The formula “Guanxin Ⅱ (GXⅡ)” is used as the basic formula to fully display the important effects of blood-activating and stasis-resolving medicinals, i.e., “to restore a smooth, balanced and harmonious state by regulating the flow of qi and blood”. Treatment is based on syndrome differentiation, and varies in different patients with different symptoms. Four proven cases are provided.

摘要:The clinical experience of Professor Chu Lixi in treating knee osteoarthritis (KOA) is introduced in this article. It is considered that “arthralgia comes earlier than flaccidity” in the development of KOA, with “arthralgia as the manifestation and flaccidity as the root”. The pathogenesis is characterized by “arthralgia developing into flaccidity, coexistence of arthralgia and flaccidity, and mutual impairment between arthralgia and flaccidity”. Professor Chu emphasizes the importance of combining syndrome differentiation-based treatment and stage differentiation-based treatment, giving considerations to the combined treatment of the whole and the local, the manifestations and the root, as well as arthralgia and flaccidity. Meanwhile, the combined use of oral and external treatment of traditional Chinese medicine is highly valued. Therefore, the principles of “combined internal and external treatment” and “one prescription for three uses” are fully practiced. In view of the internal pathogenesis of “imbalance of sinews and bones”, Professor Chu puts “equal emphasis on the treatment of sinews and bones”, advocating combined use of staged orthopaedic manipulative therapies and functional training to achieve “integrated medical treatment and rehabilitation”. One proven case is presented.

摘要:Professor Yan Juntao’s academic experience of treating sinews for shoulder periarthritis (hereinafter referred to as “frozen shoulder”) is introduced. According to Professor Yan, the main pathogenesis of frozen shoulder in traditional Chinese medicine is “sinew impediment”, and its onset is caused by a combination of factors. In the clinic, all causative factors should be identified and considered. Treatment of sinews and staged treatment are recommended. The treatment concept of “mastering all methods of treatment for treatment” is highlighted. It is advocated that Tuina massage should be a combination of dynamic and static manipulations, that Tuina exercise should be applied with syndrome differentiation, and that modern rehabilitation should be integrated into the theoretical framework and clinical diagnosis and treatment of frozen shoulder. His innovation is unique and advantageous.

摘要:This paper explored the theory of “Ying Wei Sheng Hui” from the perspective of blood circulation in modern medicine. Ying qi and Wei qi keep running all the time in the body and play the role of defending, warming, regulating and nourishing, etc. It is quite similar to modern medicine’s blood circulation that runs round and forth in the human body, exerting functions such as material transport, body fluid regulation, maintaining homeostasis, achieving blood defense and endocrine secretion. There is a certain internal relationship between “Ying Wei Sheng Hui” and human blood circulation, but the meaning of “Ying Wei Sheng Hui” is much more than blood circulation.

摘要:From the aspects of traditional Chinese medicine theory spleen-brain correlation and apoplexy, preliminary clinical practice and research, this article expounded the thinking of treating ischemic stroke from the spleen. The etiology and pathogenesis of ischemic stroke were complex, involving multiple zang-fu organs. “Although five zang-fu organs are related, the spleen is pivotal”. The mechanism of ischemic stroke caused by spleen dysfunction was mainly in pathogen excessiveness and spleen deficiency causing cerebral occlusion; spleen failing to transform properly, phlegm and blood stasis blocking collaterals; spleen failing to manage blood and vessel, brain collaterals stagnation; incoordination between spleen and stomach, qi movement disorder. Therefore, invigorating spleen and strengthening healthy energy should be applied the whole process of ischemic stroke prevention and treatment.

摘要:Professor Shi Xinde’s new understanding of the ancient famous prescription “Xiaoyao Powder” is introduced systematically from three aspects: pathogenesis and symptoms recorded in the original text, principles of formula composition, and characteristics of the formula and syndromel. It is concluded that Xiaoyao Powder is a basic formula for regulating qi, blood and water, and the basic pathogenesis is liver blood deficiency, involving nutrient qi and defense qi disharmony syndrome, internal retention of water-dampness (phlegm-dampness) syndrome and liver qi stagnation syndrome, with left thin pulse and right wiry pulse. Two proven cases are presented.

摘要:The application scope of classical famous prescription Xiayuxue Decoction was discussed and expanded based on the analysis of the formula indications. It is thought that blood stasis is the cause of indications of Xiayuxue Decoction, and blood stasis stagnating in the lower energizer is its pathogenesis, which should be distinguished from general blood stasis. The connotation of the Decoction can be explained from four points: firstly herbs with drastic downward-expelling effect can attack blood stasis and herbs with stasis-breaking effect can promote the generation of new blood; secondly the orderly ascending and descending movement of herbal action can dredge collaterals; thirdly the combination of herbs with drastic and mild properties, reducing and nourishing functions can neutralize the strong attacking-expelling side effect; and fourthly the worm medicine can penetrate deeply into the place where the pathogen ambushes. One proven case is provided.

The intervention lasted for 2 weeks from the 15th day. At the end of the experiment, pulmonary function, peripheral blood leukocyte count, eosinophil count, interleukin-4 (IL-4), interleukin-5 (IL-5), interleukin-13 (IL-13) in bronchoalveolar lavage fluid and lung HE staining were detected, and meanwhile, real-time fluorescence quantitative reverse transcription and polymerase chain reaction (RT-qPCR) was used to detect the expression of IL-4, IL-5 and IL-13 mRNA in lung tissue.ResultsCompared with the model group, the pulmonary function indices such as forced vital capacity (FVC), forced expiratory volume in 0.1 s (FEV0.1), FEV0.1/FVC and expiratory flow rate (FEF50%) in the traditional Chinese medicine group were significantly higher (P<0.05), while the peripheral blood leukocyte count and eosinophil count were significantly decreased (P<0.05). The contents of IL-4, IL-5 and IL-13 in bronchoalveolar lavage fluid and the expression of mRNA in lung tissue were significantly decreased (P<0.05). HE staining showed that the bronchial structure was clear, and inflammatory cell infiltration and glandular hyperplasia were significantly alleviated in traditional Chinese medicine group.ConclusionXCNG can improve airway inflammation in rats with bronchial asthma, and its mechanism may be related to the regulation of inflammatory factor gene expression.

摘要:ObjectiveThe manganese-containing silica carrier was prepared, which was loaded with quercetin and blocked with carboxymethyl chitosan. The structure and release characteristics of the nano-drug loading system were studied, and the thermodynamic and kinetic model of quercetin adsorption by manganese-containing silica nanoparticles was fitted.MethodsManganese-containing silica carriers were synthesized using surfactant as template. Langmuir, Freundlich and Temkin isothermal adsorption models were used to fit the thermodynamic data of quercetin adsorption on manganese-containing silica nanoparticles. Quasi-first-order kinetics, quasi-second-order kinetics and intra-particle diffusion model were used to fit the adsorption kinetic data. The in vitro release of nano-drug loading system was evaluated by dialysis bag method.ResultsThe nano-drug loading system of quercetin based on carboxymethyl chitosan was successfully prepared. The thermodynamic and kinetic adsorption of quercetin on manganese-containing silica carriers decreased in the order of 25 ℃>35 ℃>45 ℃. The best thermodynamic fitting model was Freundlich isothermal adsorption model, and the best kinetic fitting model was quasi-second-order kinetic model. In pH6.5, the cumulative release of nano-drug loading system reached 92.53% within 50 hours, which was higher than that at other pH values (5.5 and 7.4). This effect was beneficial to the treatment of tumors.ConclusionThe adsorption of quercetin on manganese-containing silica carrier corresponds with Freundlich isothermal model and quasi-second-order kinetic model. The temperature has an effect on the adsorption of quercetin on the carrier, and the environmental pH value affects the release of quercetin in the nano-drug loading system.

摘要:ObjectiveBy observing the NF-κB signaling pathway and inflammatory factor-related indexes, the effects of Yunnan Baiyao aerosol and Tuina rolling on the repair of skeletal muscle in rabbits in the acute stage of skeletal muscle injury were analyzed, and the mechanism of the action on skeletal muscle injury repair was explored.MethodsTotally 32 New Zealand rabbits were randomly divided into blank group, model group, Tuina rolling group and Yunnan Baiyao group, with 8 rabbits in each group. Each group were further randomly divided into 2 subgroups (corresponding to 24 h and 72 h after injury), with 4 rabbits in each subgroup. Except the blank group, the rabbits in the other groups were molded with a gravity hammer, and the homemade massager and Yunnan Baiyao aerosol were respectively used to treat the rabbits for three days at 24 h and 72 h after injury. Local damaged tissue was observed by hematoxylin-eosin (HE) staining 24 hours after treatment. The contents of TNF-α and IL-1β in the damaged tissues were detected by enzyme linked immunosorbent assay (ELISA), and the contents of NF- κB p65 protein and nuclear factor κB inhibitor protein α (IκBα) in the damaged tissues were detected by Western blot.Results①The results of HE showed that compared with the blank group, the model group had inflammatory infiltration and serious muscle injury, indicating that the model was successful. Compared with the model group, the muscle fibers were seriously broken and the neutrophils were widely released in the 24 h Tuina rolling group. Compared with the model group, the injury degree in 24 h and 72 h Yunnan Baiyao groups and in 72 h Tuina rolling group was lighter, the number of inflammatory cells was reduced, and the muscle cells were arranged in a cylindrical shape. ②The results of ELISA showed that compared with the blank group, the contents of TNF-α and IL-1β in the whole tissue of the model group increased (P<0.05). Compared with the model group, the contents of TNF-α and IL-1β increased in 24 h Tuina rolling group, but decreased in 72 h Tuina rolling group (P<0.05). Compared with the model group, the contents of TNF-α and IL-1β in Yunnan Baiyao group decreased at each time point (P<0.05). ③The results of Western blot showed that compared with the blank group, the expression of NF-κB p65 protein increased and the content of inhibitor protein IκBα decreased at each time point in the model group (P<0.05). Compared with the model group, the expression of NF-κB p65 protein decreased significantly, but the expression of IκBα increased in 72 h Tuina rolling group, and NF-κB p65 protein decreased significantly and IκBα protein increased significantly in 24 h and 72 h Yunnan Baiyao group (P<0.05).ConclusionsYunnan Baiyao aerosol can down-regulate TNF-α and IL-1β, inhibit NF-κB signal transduction, reduce apoptosis of skeletal muscle injury cells, inhibit inflammatory release, and promote skeletal muscle function repair in 24 h and 72 h after muscle tissue injury, especially in the acute stage of skeletal muscle injury. But the Tuina rolling method has those effects only in 72 hours after muscle tissue injury.

摘要:ObjectiveTo compare serum and brain Gamma-aminobutyric acid (GABA) as well as glutamic acid (Glu) in the sleep deprived rats due to Zhi unrest in the kidney or Po unrest in the lung, in order to provide reference for their classification and diagnosis of insomnia in traditional Chinese medicine based on five-spirit.MethodsThirty-six female Sprague Dawley (SD) rats were randomly divided into three groups: Zhi unrest in the kidney group (kidney group), Po unrest in the lung group (lung group) and normal group. The sleep deprived model of Zhi unrest in the kidney was prepared by long time sitting on the cold and wet sand in the rat cage. The model of Po unrest in the lung was prepared by the shallow water environment in narrow and long platforms. The sleep was evaluated through sleep monitor and the learning and memory function was evaluated by Water Morris Maze test. The levels of serum GABA and Glu as well as brain Glu were detected by enzyme-linked immunosorbent assay (ELISA), and the levels of GABA in different brain regions were detected by immunohistochemistry.ResultsCompared with the normal group, the sleep time was shortened in the 2 model groups. The arousal times increased in the lung group and decreased in the kidney group. The kidney group showed a decrease in sleep time due to inability to fall asleep again in the late stage of sleep, while the lung group showed a decrease in sleep time caused by frequent awakening in the morning and afternoon (P<0.05). The escape latency in the kidney group was longer and the number of times crossing the platform in the kidney group was less than those in the lung group (P<0.05). The levels of serum GABA and Glu decreased significantly in the lung group. The levels of serum and brain Glu decreased significantly in the kidney group and lower than that in the lung group. The levels of GABA in hypothalamus, thalamus, retrosplenial granular b cortex, motor cortex, etc., decreased significantly in the two model groups with lower levels in the lung group(P<0.05). The level of GABA in auditory area and hippocampal amygdaloid transitional area in the lung group was lower than that in the kidney group, while the level of GABA in hippocampal amygdala transitional area in the kidney group was higher than that in the normal group(P<0.05).ConclusionsThere exists learning and memory dysfunction in the 2 types of insomnia model rats, which is worse in the rats with Zhi unrest in the kidney. Insomnia rats with Zhi unrest in the kidney have lower Glu level, while insomnia rats with Po unrest in the lung have lower GABA level, which indicates that there are differences in Glu, GABA and other molecular levels between Zhi unrest in the kidney and Po unrest in the lung in the diagnosis of TCM insomnia based on five-spirit classification.

摘要:The research progress on traditional Chinese medicine extracts in treating Parkinson’s disease(PD) based on C. elegans PD models was reviewed. We summarized the common models of C. elegans in the research of PD, and found that some traditional Chinese medicine extracts, such as oleuropein and cannabidivarin, etc.,had therapeutic effects on various types of C. elegans PD models. Traditional Chinese medicine extracts protected neurons through different ways, and retarded the pathological changes related to PD.
