      摘要:The increasing number of patients suffering from “intractable and miscellaneous diseases” has led to the study and discussion on the correlation between endogenous toxins and chronic intractable diseases. It is believed that “accumulation and damage of endogenous toxins” has become an important etiological and pathological factor of modern “intractable diseases” due to the influence of external environmental hazards and changes in lifestyle. As a relatively independent and objective being of pathogenic qi, endogenous toxins must have specific clinical manifestations, presenting the syndrome features of “accumulation of toxins” and “gradual manifestation of symptoms”. The connotation of pathogenic toxins changes with the development of the times. Since diseases caused by endogenous toxins are mostly chronic intractable diseases, the first-line and necessary treatment is to resolve toxins. Therefore, the connotation of toxin-resolving therapy must keep pace with the times to let its clinical value keep on shining today.

      摘要:Based on the “catch-up growth” theory, the treatment of polycystic ovary syndrome (PCOS) from the spleen and stomach was investigated. The study confirmed that the insulin resistance caused by “catch-up growth” could be significantly improved by traditional Chinese herbal intervention. In order to clarify the importance of treating PCOS from the perspective of spleen and stomach, the theory of spleen and stomach was adopted for the exploration of “catch-up growth”, and the pathogenesis of PCOS was explained from the perspective of “catch-up growth”.

      摘要:In order to improve the effectiveness and safety of cervical spine manipulation, and provide theoretical and technical support for the prevention and treatment of cervical spondylosis with traditional Chinese medicine characteristics, the best clinical indications for cervical spine manipulation, as well as the absolute and relative contraindications to manipulation are discussed and elaborated in this article based on over 100 years of clinical experience and scientific research of “Shi’s Traumatology”, together with the research and understanding on manipulation in other schools.

      摘要:Sarcopenia is defined as a progressive decline in skeletal muscle mass, strength and function in the elderly, accompanied by an increased risk of physical frailty, functional disability, falls, hospitalization and death. With the global rise in life expectancy and the growth of the geriatric population, sarcopenia is becoming a major geriatric disorder. Based on the theory of Traditional Chinese Medicine (TCM) and original literature and reviews in the field, TCM has unique advantages for the treatment of sarcopenia which has made some progress from the aspects of traditional Chinese herbal medicine, traditional Chinese exercises, acupuncture, and Tuina. However, the specific mechanism needs further study. Overall, as the TCM research on sarcopenia is still at an early stage, high-quality clinical evidence-based research and fundamental research are needed to enhance the role of TCM in the prevention and treatment of sarcopenia.

      摘要:ObjectiveTo explore and analyze the medication rules for the treatment of stroke in Puji Fang based on the Traditional Chinese Medicine Inheritance Support System (tcmiss) (V2.5) software.MethodsThe prescriptions for the treatment of stroke in Puji Fang were compiled and fed into the tcmiss (V2.5) to build a database, and then relevant statistical analysis tools of the software were used for data mining to analyze the medication rules in prescriptions.Results①There were 47 prescriptions that met the inclusion criteria, involving 137 traditional Chinese herbal medicines (TCHMS). ②There were 27 TCHMs with frequencies greater than or equal to 5, and Fangfeng (Saposhnikoviae Radix) was the most frequently used herbal medicine. ③TCHMs used were mostly warm in nature, pungent, sweet and bitter in flavor, and most belonged to the spleen, heart, liver and lung meridians. ④According to analysis results of the medication rules, there were 13 combination patterns out of 11 TCHMs, which were listed as below: Chuanxiong (Chuanxiong Rhizoma) – Fangfeng (Saposhnikoviae Radix), Gancao (Glycyrrhizae Radix et Rhizoma) – Fangfeng (Saposhnikoviae Radix), Fangfeng (Saposhnikoviae Radix) – Duhuo (Angelicae Pubescentis Radix), Fangfeng (Saposhnikoviae Radix) – Xixin (Asari Radix et Rhizoma), Fangfeng (Saposhnikoviae Radix) – Fuzi (Aconiti Lateralis Radix Praeparata), Chuanxiong (Chuanxiong Rhizoma) – Gancao (Glycyrrhizae Radix et Rhizoma), Renshen (Ginseng Radix et Rhizoma) – Fushen (Poria cum Radix Pini), Renshen (Ginseng Radix et Rhizoma) – Fangfeng (Saposhnikoviae Radix), Renshen (Ginseng Radix et Rhizoma) – Fuling (Poria), Shengjiang (Zingiberis Rhizoma Recens) – Fangfeng (Saposhnikoviae Radix), Gancao (Glycyrrhizae Radix et Rhizoma) – Fuling (Poria), Baishao (Paeoniae Radix Alba) – Fangfeng (Saposhnikoviae Radix), and Chuanxiong (Chuanxiong Rhizoma) – Gancao (Glycyrrhizae Radix et Rhizoma) – Fangfeng (Saposhnikoviae Radix). ⑤Based on the association rules analysis, 4 commonly used TCHMs combination patterns were obtained, including 4 TCHMS, i.e., Fangfeng (Saposhnikoviae Radix), Chuanxiong (Chuanxiong Rhizoma), Duhuo (Angelicae Pubescentis Radix) and Gancao (Glycyrrhizae Radix et Rhizoma). ⑥The cluster analysis showed that there were 4 core clusters which could potentially develop into 2 new formulas, i.e., Chuanxiong (Chuanxiong Rhizoma) – Gancao (Glycyrrhizae Radix et Rhizoma) – Bohe (Menthae Haplocalycis Herba) – Duhuo (Angelicae Pubescentis Radix), and Zhusha (Cinnabaris) – Gancao (Glycyrrhizae Radix et Rhizoma) – Bohe (Menthae Haplocalycis Herba) – Yuanzhi (Polygalae Radix).ConclusionsThe prescriptions recorded in Puji Fang for treating stroke mainly contain TCHMs for dispelling wind and dredging collaterals, invigorating qi and nourishing blood, and the representative TCHMs are Fangfeng (Saposhnikoviae Radix), Duhuo (Angelicae Pubescentis Radix), Xixin (Asari Radix et Rhizoma), Shengjiang (Zingiberis Rhizoma Recens), Fuzi (Aconiti Lateralis Radix Praeparata), Chuanxiong (Chuanxiong Rhizoma), Baishao (Paeoniae Radix Alba), Gancao (Glycyrrhizae Radix et Rhizoma), Renshen (Ginseng Radix et Rhizoma), Fushen (Poria cum Radix Pini) and Fuling (Poria). The new prescriptions developed based on the aforementioned basic prescription will provide reference to the follow-up study of stroke medication.

      摘要:ObjectiveTo investigate the regulatory effect of miR-155 on the progress of the subacute thyroditis (SAT) inflammatory response.MethodsWe collected 39 cases of subacute thyroiditis and 36 cases of normal control, detected circulating miR-155 expression level and the protein levels of inflammatory cytokines (IL-2, IL-6, IL-10, TNF-α) of subjects. Correlation analysis was carried out on the circulating miR-155 expression level of SAT patients with related inflammation indicators (WBC, ESR, CRP) and thyroid function indicators (TG, FT3, FT4, TSH). Nthy-ori 3-1 cells were induced by TNF-α with different concentrations (0, 1, 5, 25 μg·L-1) to establish a thyroid cell inflammation model, and the expression level of miR-155 was regulated by oligoRNA, which was transfected by lipofectamine 2000. Fluorescence microscope was used to identify transfection efficiency, qRT-PCR was performed to detect the expression of IL-6, IL-8, IL-1β mRNA and miR-155, and ELISA was performed to detect protein levels of IL-6, IL-8, and IL-1β.Results①The expression levels of miR-155, IL-6 and TNF-α in SAT patients were significantly higher than those of healthy control participants (P<0.05). ②The expression levels of miR-155 in serum were positively correlated with IL-6 (P<0.01, r=0.859), TNF-α (P<0.01, r=0.958), FT3 (P<0.05, r=0.641) and FT4 (P<0.01, r=0.775) in SAT patients. ③In Nthy-ori 3-1 cells, TNF-α could upregulate the expression level of miR-155, and induce the expression and release of IL-6, IL-8 and IL-1β mRNA, which had a concentration-dependent trend. ④The transfection efficiency reached about 90% after 6 hours of oligoRNA transfection. Compared with the condition in the negative control, miR-155 mimics (30 nmol·L-1) could significantly upregulate the expression level of miR-155 (P<0.05), while miR-155 inhibitor (100 nmol·L-1) could significantly downregulate the expression level of miR-155 (P<0.05). ⑤mRNA expression and release of IL-6, IL-8 and IL-1β showed a marked increase after overexpression of miR-155 (P<0.05); On the other hand, TNF-α induced mRNA expression and release of IL-6, IL-8 and IL-1β were suppressed after suppression of miR-155 (P<0.05).ConclusionsmiR-155 plays a role in promoting inflammation in the SAT, which provides a new direction for the target research of traditional Chinese medicine in treating SAT.

      摘要:Based on the theory of " all diseases are caused by depression", this paper discusses the diagnosis and treatment of systemic lupus erythematosus in depression from five aspects: the causes of disease, the meaning of depression, the signs of depression, the treatment of depression and examples of medical records. It emphasizes that clinical treatment pays attention to dredging qi and regulating emotions.

      摘要:This article introduced Bian Songjing’s experience in treating stroke and da jue from qi and blood. Mr.Bian believed that stroke and da jue were two categories and couldn’t be divided. Stroke was in qi and approximates neuropathy in modern medicine; da jue was in blood and approximates vascular disease in modern medicine. In the treatment, stroke should be identified as qi disease and da jue as blood disease, stroke empirical should be based on opening the orifices and expelling wind, deficiency syndrome should be based on filling the orifices and extinguishing wind; Da jue empirical should be based on removing blood stasis throughout, and da jue deficiency syndrome should be based on tonifying deficiency and solidifying. However, stroke and da jue could be combined with disease, in other words, when qi and blood were weak together, treatment should distinguish the priorities, qi and blood were treated together.

      摘要:Professor Wang Qingqi’s experience in treating intractable headache by applying the four methods of dredging collaterals is introduced. According to traditional Chinese medicine theory, prolonged illness enters the collaterals, so does the persistent pain. Professor Wang believes that it is pathogens stagnating in the collaterals and deficient collaterals failing to nourish that causes intractable headache because loss of smooth flow and nourishment leads to pain. Intractable headache is a disease with the coexistence of multiple syndromes and intermingled deficiency and excess. Therefore, when it comes to the treatment, Professor Wang adheres to the principle of “dredging for normal function” and uses the four methods of dredging collaterals flexibly. The compatibility of pungent medicine, insect medicine, rattan medicine and medicine for nourishing blood and dredging collaterals in the prescription can often have significant effect with just one dose. One proven case is included for clarification.

      摘要:Through the description of the theory of dredging intestines, combined with the evidence of relevant literature, taking the clinical application of the theory of dredging intestines in stroke, coma, vertigo, insomnia, headache, dementia, depression, tremor and other aspects of encephalopathy as examples, this paper discussed that the theory of dredging intestines should play an important role to guide the treatment of encephalopathy based on syndrome differentiation, so as to improve the clinical efficacy of traditional Chinese medicine in treating encephalopathy.

      摘要:From the perspective of the pathogenesis of flaccidity due to Bi and the method of treating flaccidity by dredging Bi, this paper analyzed three Daoyin methods for the treatment of Bi and flaccidity syndrome contained in the theory of General Treatise on Causes and Symptoms of All Diseases,explained the Daoyin methods of wind withering syndrome, wind Bi hand and foot non-following syndrome, rheumatism Bi syndrome and its connotation of treating flaccidity by dredging Bi, to deepen Daoyin theory and enhance clinical practice value.

      摘要:ObjectiveTo observe the clinical efficacy of Fuzheng Hejie Recipe in the treatment of asymptomatic children infected with SARS-CoV-2 Omicron variant.MethodsSixty-six asymptomatic children infected with SARS-CoV-2 Omicron variant were randomly divided into a treatment group (n=33) and a control group (n=33). The treatment group was administered with granules of Fuzheng Hejie Recipe and the control group was treated with the recombinant human interferon alpha-2b spray. The treatment course was 7 d for both groups. The safety of medication was observed, and the negative conversion time of nucleic acid test (NAT) for SARS-CoV-2 and the negative conversion rate of NAT within 7 d were compared.Results①No patient dropouts or patient discontinuers were found during the trial. ②The negative conversion time of NAT was (9.33±3.07) d in the treatment group and (11.85±2.39) d in the control group; There was statistically significant difference in the negative conversion time of NAT between the two groups (P<0.05). ③On day 2 through 7, the negative conversion rate in the treatment group was significantly higher than that in the control group (P<0.05). ④During the trial, none of the infected children had allergic reactions, nausea or vomiting, etc., and no abnormality was found in the liver and kidney function test in both groups.ConclusionFor asymptomatic children infected with SARS-CoV-2 Omicron variant, Fuzheng Hejie Recipe can significantly shorten the negative conversion time and increase the negative conversion rate of NAT with good safety.

      摘要:ObjectiveTo observe the efficacy of acupuncture combined with trimetazidine in the treatment of radial artery injury related to coronary angiography based on color doppler ultrasound.MethodsA total of 86 patients with radial artery injury related to coronary angiography were included and randomly divided into the control group and the treatment group, 43 cases in each group. All the included patients were treated with transradial artery angiography, and were orally treated with trimetazidine after operation. Based on above, the patients in the treatment group were treated with acupuncture after operation. The treatment course of the two groups was 15 days. The clinical effect was evaluated after 15 days of treatment. Before (on the day after operation) and after treatment (after 15 days of treatment), the radial artery internal diameter, end diastolic velocity (EDV), peak systolic velocity (PSV), resistance index (RI) and pulsatility index (PI) were detected by doppler ultrasound instrument. Before and after treatment, the prothrombin time (PT) and activated partial thromboplastin time (APTT) were detected. In the course of treatment, the complications were observed and recorded, and the incidence of the complications was calculated.Results①After treatment, the total clinical effective rates of the treatment group and the control group were 90.70% and 74.42% respectively, and the effect of the treatment group was better than that of the control group (P<0.05). ②After treatment, the radial artery internal diameter in the two groups was significantly increased compared with that before treatment (P<0.05), and the radial artery internal diameter of the treatment group was obviously greater than that of the control group (P<0.05); the EDV and PSV of radial artery in the two groups were significantly accelerated compared with those before treatment (P<0.05), the RI and PI of radial artery blood flow were significantly decreased compared with those before treatment (P<0.05), and the EDV and PSV of radial artery in the treatment group were obviously faster than those in the control group (P<0.05), the RI and PI of radial artery blood flow were obviously less than those in the control group (P<0.05). ③After treatment, the PT and APTT in the two groups were significantly prolonged compared with those before treatment (P<0.05), and the PT and APTT of the treatment group were obviously longer than those of the control group (P<0.05). ④The incidence of the complications was 11.63% in the treatment group and 37.21% in the control group, and the incidence of the complications in the treatment group was lower than that in the control group (P<0.05).ConclusionAcupuncture combined with trimetazidine can effectively improve the vascular elasticity of radial artery after coronary angiography, accelerate the blood flow velocity of radial artery, inhibit the thrombogenesis, and relieve vasospasm, thus reduce the incidence of complications including radial artery stenosis and occlusion and enhance the clinical efficacy.

      摘要:ObjectiveTo observe the clinical effect of acupuncture and medicine combined (Xiaochaihu Decoction combined with Tiaosheng Jieyu acupuncture) in treating patients with chronic kidney disease (CKD) stages 2-3b and depression.MethodsAltogether 120 patients with CKD stages 2-3b, depression and TCM diagnosis of spleen and kidney qi deficiency syndrome and Shaoyang syndrome were randomized into control group, Xiaochaihu Decoction (XCHD) group and acupuncture-medicine combined treatment (AMCT) group, with 40 patients in each group. All three groups were given routine basic treatment, while the XCHD group was additionally administered with Xiaochaihu Decoction, and the AMCT group was also treated with Tiaosheng Jieyu acupuncture plus Xiaochaihu Decoction. The treatment course of each group lasted 12 weeks. The clinical improvement on CKD, Self-Rating Depression Scale (SDS) and Hamilton Depression Scale (HAMD) was observed. The changes in the TCM syndrome scores and levels of blood creatinine (SCr), blood urea nitrogen (BUN), serum 5-hydroxytryptamine (5-HT) and serum brain-derived neurotrophic factor (BDNF) were compared before and after treatment. The changes in the glomerular filtration rate (eGFR) and 24-h urine protein quantification value were estimated. Safety evaluation was also performed.Results①During the trial, there were 8 drop-out cases in the control group, 4 drop-out cases in the XCHD group, and 6 drop-out cases in the AMCT group. Finally, 102 cases completed the trial, including 32 cases in the control group, 36 cases in the XCHD group, and 34 cases in the AMCT group. ②The improvement on CKD, SDS and HAMD in the XCHD group and the AMCT group was better than that in the control group (P<0.05), and the AMCT group had even better improvement on HAMD than the XCHD group (P<0.05). ③After treatment, the TCM syndrome scores decreased in each group (P<0.05); the XCHD group and the AMCT group both had lower TCM syndrome scores compared with the control group (P<0.05); and the AMCT group had even lower TCM syndrome scores than the XCHD group (P<0.05). ④The levels of SCr, BUN, eGFR and quantification value of 24-h urine protein decreased in the AMCT group after treatment (P<0.05), while there was no statistical difference in the above-mentioned indexes before and after treatment in the control group and XCHD group (P>0.05); After treatment, the AMCT group had lower quantification value of 24-h urine protein than the control group and the XCHD group (P<0.05); There was no statistical difference in the levels of SCr, BUN and eGFR among the groups (P>0.05). ⑤After treatment the serum levels of 5-HT and BDNF increased in each group (P<0.05); The AMCT group had a higher serum level of 5-HT than the control group (P<0.05); There was no statistical difference in the serum BDNF level among the groups (P>0.05). ⑥During the trail, no significant adverse reactions were observed in each group.ConclusionXiaochaihu Decoction and Tiaosheng Jieyu acupuncture combined treatment has achieved a satisfactory result in dealing with patients with CKD and depression, which can effectively improve patients’ TCM symptoms and depression, reduce renal function damage. The mechanism may be related to the elevation of serum levels of 5-HT and other depression related factors.

      摘要:ObjectiveTo observe the clinical effect of application of Huoxue Huayu decoction combined with methotrexate in the treatment of rheumatoid arthritis.Methods80 patients with rheumatoid arthritis were randomly divided into treatment group and control group, 40 patients in each group. Both groups were treated with methotrexate. The treatment group was added with Huoxue Huayu Decoction, while the control group was added with Huoxue Huayu Simulated Decoction. The course of treatment in both groups was 6 months. After treatment, the TCM syndrome curative effect, TCM syndromes score, disease activity(DAS28) score, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), rheumatoid factor (RF), anti-cyclic citrullinated peptide (CCP)antibody, fibrinogen (Fg), vascular endothelial growth factor (VEGF) and proliferation of synovial pannus were observed.Results①Finally, 73 patients completed the clinical trial, 37 patients in the treatment group and 36 patients in the control group. ② After treatment, the total effective rate of the treatment group was 89.19%, and that of the control group was 77.78%. Comparing the curative effect of TCM syndromes between the two groups, the treatment group was better than the control group (P<0.05). ③After treatment., the scores of DAS28 and TCM syndromes of the two groups decreased (P<0.05). After treatment, there was no significant difference in DAS28 score between the two groups (P>0.05), and the TCM syndrome score of the treatment group was significantly lower than that of the control group (P<0.05). ④After treatment, the levels of RF, ESR, CRP and anti-CCP in both groups were lower than those before treatment (P<0.05). After treatment, the levels of RF and CRP in treatment group were lower than those in control group (P<0.05). ⑤After treatment, the levels of Fg and VEGF in the treatment group were lower than those before treatment (P<0.05), and were lower than those in the control group (P<0.05). ⑥Comparison between before and after treatment, the synovial pannus grades of both groups were better than those before treatment (P < 0.05); After treatment, the grade of synovial pannus in the treatment group was better than that in the control group (P<0.05).ConclusionThe application of Huoxue Huayu decoction combined with methotrexate in the treatment of rheumatoid arthritis has a good clinical effect, it can effectively down regulate the levels of CRP, Fg and VEGF, and inhibit the proliferation of synovial pannus.

      摘要:ObjectiveTo evaluate the effects of Artemisia annua tea extract (AT) on improving skin lesions in dinitrochlorobenzene (DNCB)-induced atopic dermatitis (AD) in BALB/c mice and to explore the possible mechanism.MethodsThirty-two male BALB/c mice were divided into normal control group, model group, AT group and dexamethasone group, with 8 mice in each group. In addition to the normal control group, AD was induced by DNCB stimulation in each mouse. At the 7th day, the model animal was given intragastric administration of AT (3 g·kg-1) and dexamethasone(1.5 mg·kg-1)for 13 days continuously. The effects of the drug on AD mice skin lesions were evaluated by clinical skin score, ear thickness and ear weight indexes. The infiltration of eosinophils and mast cells in mouse skin tissue were detected by hematoxylin and eosin (HE) and toluidine blue (TB) staining. IL-13 level was detected by enzyme-linked immunosorbent assay (ELISA). The mRNA expressions of inflammatory genes such as IL-13, thymic stromal lymphopoietin (TSLP), IL-17 and TNF-α in ear tissues were detected by qPCR.Results①Compared with the normal control group, after repeated application of DNCB on the skin of the back and ear of mice, the skin of the model group mice showed erythema, swelling, fester,dryness, flakiness, and other manifestations. The epidermis of the mice was thickened, and the infiltration of eosinophils and mast cells was increased (P<0.05). The serum IL-13 level was significantly increased (P<0.05). The mRNA expressions of IL-13, TSLP, IL-17 and TNF-α in ear tissue were significantly up-regulated (P<0.05). ②Compared with the model group, the AD-like symptoms of BALB/c mice induced by DNCB were significantly alleviated by AT treatment. The skin dryness and flakiness, erythema, swelling and fester were significantly improved (P<0.05), ear thickening was significantly inhibited (P<0.05), and mast cells and eosinophilic infiltration were significantly decreased (P<0.05). At the same time, the serum IL-13 level was significantly decreased (P<0.05), and the mRNA expressions of IL-13, TSLP, IL-17 and TNF-α in ear tissues were also significantly down-regulated (P<0.05).ConclusionAT may have a therapeutic effect on AD by inhibiting the expression of Th2-related inflammatory cytokines including IL-13, TSLP, IL-17 and TNF-α.

      摘要:The research progress of appetite suppression by targeted appetite regulation mechanism with traditional Chinese herbal medicines (TCHMs) is reviewed in this article. It is found that 15 components of TCHMs, such as evodiamine, saponin Rb1, celastrol and saikosaponin A, have significant inhibitory effects on appetite, and its mechanism involves 8 appetite regulation links, including neuropeptide Y, 5-hydroxytryptamine, leptin and gut microbiome, etc.

      摘要:Metabolomics is an emerging discipline that analyzes all low molecular weight metabolites (organic and inorganic metabolites with relative molecular mass below 1,000 as the research core) of a certain organism or cell, and is a scientific method that reveals the metabolic essence of the organism’s life activities through the study of metabolites. Science of acupuncture and moxibustion, as an important part of traditional Chinese medicine, follows the principle of holistic concept and syndrome differentiation based treatment. The multi-level and multi-target characteristics of acupuncture effect are highly consistent with the integrity and systematicness of metabolomics, which has a broad application prospect in the effect mechanism study of acupuncture and moxibustion. At present, there are both opportunities and challenges for the development of acupuncture and moxibustion in the process of international standardization. The application of metabolomics technology to elucidate the scientific connotation of effect mechanism of acupuncture and moxibustion in a comprehensive and systematic manner is of great significance for the modernization of acupuncture and moxibustion, and the use of metabolomics technology may also provide important technical support for the objective and scientific explanation of the effect mechanism.

      摘要:Blueberry (Vaccinium spp.) has been widely cultivated and exploited in China, and used for medicine and food dual-purpose. Except blueberry fruits, blueberry leaves as a by-product of blueberry plants also contain extensive and excellent pharmacological activities. Polyphenols, the main constituents of blueberry leaves, can be divided into phenolic acids, flavonols, anthocyanins, and proanthocyanidins. In this article, we reviewed the research progress on the chemical composition and pharmacological effects of blueberry leaves and the active ingredient extraction and determination methods, and analyzed their correlations with varieties, growth factors and compounds stability. The aforementioned work can provide a basis for further development and utilization of blueberry leaves.
