摘要

基于“白气微泄”理论辨治轻型系统性红斑狼疮（SLE）。梳理该理论的古籍内涵，“白气”即肺气，“微泄”指肺气轻微耗泄致卫气宣发失职、腠理不密，与轻型SLE“病位在表、邪浅证轻、脏腑未损”的病理特征高度契合；其核心病因病机在于“白气生成不足、白气耗泄加剧、卫郁气泄”；据此确立“微病微调”为核心治则，围绕“复白气循行、补白气耗损、防白气耗伤”构建诊疗框架，配套形成“玉屏风散合桑菊饮化裁方+宣肺护表洗方”的方药体系。并附验案1则。

Abstract

This study adopts the theory of "mild leakage of white qi" for syndrome differentiation and therapeutic management of mild systemic lupus erythematosus （SLE）. The ancient classical connotation of the aforementioned theory is elucidated： the term "white qi" corresponds to lung qi， while "mild leakage" refers to mild consumption and dissipation of lung qi that impairs the dispersion function of defensive qi and loosens cutaneous interstitial striae. This pathological essence is highly consistent with the pathological hallmark of mild SLE， namely superficial lesion localization， superficial pathogenic invasion with mild clinical presentations， and absence of substantial visceral injury. Its core etiology and pathogenesis fall into three categories： deficient biosynthesis of lung qi， aggravated consumption and leakage of lung qi， and defensive qi stagnation complicated with pathological qi dissipation. On this basis， the core therapeutic principle of minimal regulation for mild disorders is formulated， and a standardized diagnostic and therapeutic framework is established centering on three therapeutic objectives： restoring the physiological circulation of lung qi， replenishing consumed lung qi， and preventing progressive lung qi damage. A complete medication regimen is subsequently developed， comprising an oral modified formula derived from Yupingfeng Powder and Sangju Decoction as well as an external topical lotion for diffusing lung qi and consolidating the superficial defensive barrier. One proven case is presented.
