摘要

目的：观察盆炎方联合腹腔镜手术治疗输卵管性不孕症的临床疗效。方法：将60例输卵管性不孕症患者随机分为对照组和治疗组,每组30例。对照组行腹腔镜手术加输卵管通液术,治疗组在手术基础上加服盆炎方。两组总疗程均为3个月,1年后随访,观察宫内妊娠率、输卵管通畅率,比较血清白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、人细胞间黏附分子-1(ICAM-1)水平的变化情况。结果：①疗程结束1年后,治疗组、对照组宫内妊娠例数分别为19例、10例,组间宫内妊娠率比较,差异有统计学意义,治疗组明显高于对照组(P<0.05)。②组间治疗后比较,输卵管通畅率差异有统计学意义,治疗组明显高于对照组(P<0.05)。③治疗前后组内比较,两组血清IL-6、TNF-α、ICAM-1水平差异均有统计学意义(P<0.05)；组间治疗后比较,血清IL-6、TNF-α、ICAM-1水平差异有统计学意义(P<0.05)。结论：盆炎方联合腹腔镜手术治疗输卵管性不孕症的疗效满意,其机制可能与调节血清IL-6、TNF-α、ICAM-1等炎症因子的表达有关。

Abstract

Objective:To observe the clinical efficacy of laparoscopy combined with Penyan Prescription on tubal infertility. MethodsSixty patients with tubal infertility were randomly divided into the control group and treatment group,30 cases in each group. The control group was treated with laparoscopy combined with hydrotubation,and the treatment group was received Penyan Prescription treatment on the basis of operation. The total course of treatment in both groups was 3 months and 1 years follow-up. The changes of pregnancy rate and patency rate of fallopian tube were observed and the levels of serum interleukin-6 (IL-6),tumor necrosis factor-α(TNF-α),intercellular adhesion molecule-1 (ICAM-1) were compared. Results:①1 year after treatment, the numbers of intrauterine pregnancy in the treatment group and control group were 19 and 10 cases respectively. There was statistically significant difference on the rate of intrauterine pregnancy between the two groups,and the rate of intrauterine pregnancy in the treatment group was significantly higher than that in the control group(P<0.05).②After treatment, there was statistically significant difference on the patency rate of fallopian tube between the two groups,and the patency rate in the treatment group was significantly higher than that in the control group(P<0.05).③Comparison of treatment before and after,there were statistically significant differences on the serum level of IL-6,TNF-α and ICAM-1 in both groups(P<0.05).After treatment, there were statistically significant differences on the serum level of IL-6,TNF-α and ICAM-1 between the two groups(P<0.05). Conclusion:Laparoscopic combined with Penyan Prescription shows good effect in the treatment of tubal infertility. The mechanism may be related to the regulation of the expression of serum IL-6,TNF-α and ICAM-1.
