系统梳理针刺通过调节脑-肠轴治疗单纯性肥胖的作用机制。针刺对脑-肠轴的调节是多靶点、多层次的。在肠道端,可重塑肠道菌群、调节短链脂肪酸这类有益代谢物的生成、调节肠道激素分泌;在中枢,可借助迷走神经和血液循环这两条通路,调控下丘脑的关键核团和中枢神经递质,从而抑制食欲;同时,针刺还能通过减轻全身范围的炎症反应,改善胰岛素抵抗,对体内代谢环境进行整体调节。建议未来临床研究关注腹部穴位(如天枢、中脘)与四肢穴位(如足三里)的配伍方案,并把肠道菌群谱和炎症因子作为疗效评价的参考指标。同时,结合多组学技术与功能性磁共振成像(fMRI)等检查技术,推动针刺治疗单纯性肥胖的标准化进程。
This review provides a systematic review of the underlying mechanisms through which acupuncture regulates the brain-gut axis to treat simple obesity. Acupuncture exerts multi-target, multi-level regulatory effects on the brain-gut axis. At the enteric level, acupuncture remodels the gut microbiota, promotes the production of beneficial metabolites such as short-chain fatty acids (SCFAs), and regulates gastrointestinal hormone secretion. Within the central nervous system, acupuncture signals travel via vagal and circulatory pathways to modulate key hypothalamic nuclei and central neurotransmitters, thereby suppressing appetite. Concurrently, acupuncture ameliorates systemic inflammation and improves insulin resistance, leading to a comprehensive optimization of the host metabolic environment. For future clinical research, we recommend evaluating acupoint compatibility regimens that combine abdominal points [e.g., Tianshu (ST25), Zhongwan (CV12)] with limb points [e.g., Zusanli (ST36)]. Furthermore, gut microbiota profiling and circulating inflammatory cytokines should be integrated as critical biomarkers for clinical efficacy assessment. Meanwhile, incorporating multi-omics approaches alongside advanced imaging technologies,such as functional magnetic resonance imaging (fMRI) and other examination techniques, will be vital to standardizing acupuncture protocols for simple obesity.
