阿尔茨海默病(AD)是老年人比较常见的一种慢性、进行性、退化性神经变性疾病,由于器质性病变导致出现以认知和记忆功能下降、生活能力减退及精神症状和行为障碍为主的临床综合征。其中医的核心病机是五脏精气亏虚,基于中医“五神藏”理论,从心神、肝魂、脾意、肺魄、肾志这五神与AD的密切关系,可验证其并非简单的脑病。针刺作为传统中医的重要手段,具有调和五脏、改善气血运行的作用。近年来的临床和实验研究发现,采用针对“五神藏”理论特定穴位的针刺能够有效改善患者的认知功能、记忆力和行为表现,其机制涉及调节β-淀粉样蛋白(Aβ)沉积及Tau蛋白磷酸化、抑制神经炎症及调节肠道菌群等多靶点作用。基于“五神藏”理论的针刺治疗方案体现了中医“形神合一”的整体观念,为AD的防治提供了新思路,具有良好的临床应用前景,其作用机制有待进一步深入研究。
Alzheimer Disease (AD) is a prevalent chronic, progressive, and degenerative neurological disorder in the elderly. It presents as a clinical syndrome characterized by cognitive and memory decline, impaired daily living activities, psychiatric symptoms, and behavioral disturbances resulting from organic brain lesions. From the perspective of traditional Chinese medicine (TCM), the core pathogenesis involves the depletion of essence and qi within the five zang-organs. Based on the "five zang-organs storing spirits" theory, the close relationship between AD and the five spirits—specifically, xinshen (heart-mind), ganhun (liver-ethereal soul), piyi (spleen-intellect), feipo (lung-corporeal soul), and shenzhi (kidney-will)— demonstrates that AD is not merely a localized brain disorder. As a cornerstone of TCM, acupuncture harmonizes the five zang-organs and improves the circulation of qi and blood. Recent clinical and experimental studies have shown that acupuncture targeting specific acupoints based on the "five zang-organs storing spirits" theory effectively enhances cognitive function, memory, and behavioral performance. Its underlying mechanisms involve multi-target actions, including the regulation of beta-amyloid (Aβ) deposition and Tau protein phosphorylation, inhibition of neuroinflammation, and modulation of the gut microbiota. Acupuncture protocols derived from this theory embody the holistic TCM concept of the "unity of form and spirit", offering novel insights into AD prevention and treatment. While showing significant clinical potential, the underlying mechanisms require further in-depth investigation.
