摘要

老年共病是指老年人同时患有多种慢性疾病。老年人营卫态势失序、循行分布紊乱、动静异常、顺逆失衡，导致各种慢性疾病共存。“营卫倾移”贯穿老年共病发生发展始终，依据老年人的生理特点和共病的发生发展规律，从营卫来源、性质、循行分布和生理、病理状态等角度出发，围绕营卫态势、位置、动静、顺逆的异常变化，提炼出营卫亏虚、阴阳虚衰，营卫涩滞、郁而停遏，营卫不偕、虚气留滞的“营卫倾移”核心病机，分别施以调补营卫、益阴回阳，通达营卫、去宛陈莝，调和营卫、补虚通滞的辨治方法。并初步探讨“营卫倾移”与现代医学中的营养代谢、免疫炎症反应、脂质斑块沉积之间的关联性。并附验案1则。

Abstract

Geriatric multimorbidity refers to the coexistence of multiple chronic diseases in the elderly patients. The ying-wei disharmony， disturbance of circulatory distribution， abnormality of dynamic-static balance， and imbalance of ascending-descending and interior-exterior flow contribute to the simultaneous presence of various chronic conditions. The pathogenesis of "imbalance between ying and wei" runs throughout the onset and progression of geriatric multimorbidity. Based on the physiological characteristics of the elderly and the developmental patterns of multimorbidity， this paper analyzes the sources， nature， circulation， and physiological and pathological status of ying-wei. Focusing on abnormalities in the state， location， movement， and directional flow of ying-wei， it refines the core pathogenesis of "imbalance between ying and wei" into three primary syndromes： deficiency of ying-wei with decline of yin and yang； stagnation and obstruction of ying-wei with depressive stasis； and disharmony of ying-wei with qi deficiency and retention. Accordingly， differentiated treatment strategies are proposed， including： tonifying and regulating ying-wei， nourishing yin and restoring yang； unblocking ying-wei to resolve stasis and disperse lingering accumulation； and harmonizing ying-wei while supplementing deficiency and resolving stagnation. Furthermore， this paper preliminarily explores the correlation between "imbalance between ying and wei" and modern medical mechanisms such as nutritional metabolism， immune-inflammatory responses， and lipid plaque deposition. One clinical case is presented for reference.
