摘要

非糜烂性胃食管反流病（NERD）病位在食管与胃。食管为“胃之系”和胃之延续，故归于六腑，具有胃腑通利水谷、通降腑气、喜柔用空、体阳而用阴等生理特点与功能。NERD基本病机为胃失和降、胃气上逆，然不同主症存在病机之殊，以烧心为主症者，常呈胃阴不足之候。以“通化宣平”理论为指导，研究食管的生理功能及病理变化，阐释胃阴不足型NERD的发病机理，厘清复杂症候，可解决诊疗难点，明确治疗方案。

Abstract

Non-erosive gastroesophageal reflux disease （NERD） primarily affects the esophagus and stomach. The esophagus functions as a crucial conduit "linking to the stomach" and as an extension of the stomach， thus it belongs to the category of the six fu organs， with physiological characteristics and functions including promoting the smooth passage of food and water， descending qi of the fu organs， preferring softness and emptiness， and possessing yang in form but functioning with yin. The fundamental pathogenesis of NERD is the disharmony in the stomach's descending function， leading to stomach qi rebellion. However， the specific pathogenesis varies according to the primary symptoms. In cases where heartburn is the main symptom， it is often indicative of a deficiency of stomach yin. Guided by the "Tonghua Xuanping （meaning regulating the flow， supporting transformation and digestion， promoting qi circulation， and achieving balance）" theory， we investigate the physiological functions and pathological changes of the esophagus， explain the pathogenesis of NERD with stomach yin deficiency syndrome， clarify complex syndromes， address diagnostic and therapeutic challenges， and establish a clear treatment plan.
