      摘要:Lianpu Yin, recorded in the Treatise on Huoluan by Wang Mengying - a doctor of the school of warm diseases, is a common formula for the treatment of qi movement disorder, disturbance between the clear and the turbid, and spleen and stomach injuries. In this article, we introduce National Chinese Medical Science Master Prof. Li Diangui's treatment principle developed based on the theory of turbid toxicity, that is, “it is hard to move turbidity without qi transformation, and it is hard to clear heat without removing turbidity”. We also share his experience of simultaneous treatment in clinical diagnosis and treatment of disorders caused by combination of “damp-heat turbid toxicity” and “turbid toxicity of qi”. Meanwhile, we explain the formulation of Lianpu Yin from the perspective of turbid toxicity theory, and appreciate Prof. Li's original intention of using this formula, hoping to provide reference for determination of etiologic factors based on differentiation and herbal prescription and medication in the clinic. Two proven cases are also included.

      摘要:Insomnia classification by five kinds of spirits in traditional Chinese medicine is a diagnosis method for the classification of insomnia according to the patient's single symptom, in which the insomnia of kidney failing in storing will is its common syndrome. Its single main symptom is wake up early at night, which can be divided into kidney yin deficiency type, kidney yang deficiency type, kidney deficiency water flooding type, kidney essence deficiency type, non-consolidation of kidney qi type and deficiency of kidney qi failing to control respiring qi type according to clinical symptoms. By locating the prescriptions and medicines of ancient doctors in treating insomnia from kidney, the medicine characteristics of treating kidney failing in storing will type insomnia are obtained, such as nourishing yin and tranquilizing the mind with Rehmanniae Radix, Ziziphi Spinosae Semen, and Testudinis Carapacis Et Plastri Colla, warming yang and tranquilizing the mind with Aconiti Lateralis Radix Praeparata, Zingiberis Rhizoma, and Cinnamomi Cortex, dampness and tranquilizing the mind with Poria, Atractylodis Macrocephalae Rhizoma, and Plantaginis Semen, filling essence and tranquilizing the mind with Polygalae Radix, Rehmanniae Radix, and Ostreae Concha, benefiting qi and tranquilizing the mind with Corni Fructus, Poria and Lycii Fructus, and absorption qi and tranquilizing the mind with Ginseng Radix Et Rhizoma, Schisandrae Chinensis Fructus, Gecko.

      摘要:ObjectiveTo identify the source of immunosuppressant myriocin from Cordyceps and its parasitic fungi.MethodsDried raw materials of traditional Chinese medicine, Dachancao and Chanhua, and two parasitic fungi of cicada, Cordyceps cicadae (or Isaria cicadae Miq.BNCC114807) and Ophiocordyceps sp.FU30231, were selected as potential myriocin producing sources. Polymerase chain reaction (PCR) was applied to obtain internal transcribed space (ITS) sequences of strains of all materials. The fermentation broth of entomogenous fungi was prepared by ethanol-reflux extraction and 80% methanol extraction, respectively, and analyzed by LC-MS. The entomogenous fungi producing myriocin were identified by using myriocin as a control standard. After extensive fermentation and isolation of myriocin, the source of myriocin was finally confirmed.ResultsITS sequences analysis showed that the parasitic fungi of Dachancao and Chanhua were Tolypocladium paradoxum and Cordyceps cicadae, respectively. The results of LC-MS analysis of the extracts indicated that the production of myriocin was detected only in the extract of Ophiocordyceps sp.FU30231. After massive fermentation, isolation and purification, the ability of Ophiocordyceps sp.FU30231 to produce myriocin was confirmed by NMR spectrum.ConclusionFor the first time, Ophiocordyceps sp.FU30231 was identified as the immunosuppressant myriocin-producing fungus which parasitized on cicada.

      摘要:ObjectiveTo explore enzymes that catalyze specific hydroxylation modification of bufadienolides so as to promote the development and application of the efficacy components of the rare traditional Chinese medicine Chansu (Bufonis Venenum).MethodsThe steroid hydroxylase from Fusarium fujikuroi was discovered by sequence alignment, and its function was determined by heterologous expression in Pichia pastoris and isolation and identification of the metabolites transformed by the engineered strains.ResultsThe activity of the strain KM71H-FF294 was investigated by using bufalin and bufotaline as substrates. No product was produced when KM71H-FF294 transformed bufotaline, but the hydroxylation product 7β-OH bufalin was produced when bufalin was transformed.ConclusionSteroid hydroxylase FF294 from Fusarium fujikuroi could catalyze the conversion of bufalin into 7β-hydroxylated products, and this is the first report of an enzyme that catalyzes the hydroxylation of bufadienolides.

      摘要:ObjectiveTo explore the influencing factors of recurrence and metastasis after radical resection of colorectal cancer.MethodsData were collected on patients with recurrence and metastasis after radical resection of colorectal cancer, with the presence of recurrence and metastasis as the endpoint event. The Kaplan-Meier method was used to analyze the survival of patients after radical resection of colorectal cancer. The log-rank test was used perform univariate analysis on clinical factors (gender, age, preoperative serum carcinoembryonic antigen level, surgical method, primary tumor location, traditional Chinese medicine syndrome, completion of chemotherapy, and oral administration of traditional Chinese medicine) and pathological factors (overall shape type, tumor diameter, histological type, degree of differentiation, vascular and neural invasion, cancer nodules, anastomotic fistula, lymph node metastasis, lymphatic metastasis rate, infiltration depth, TNM staging, and number of lymph nodes detected). Cox regression model was used for multivariate analysis.Results①A total of 417 cases were collected, and the peak time for recurrence and metastasis after radical resection of colorectal cancer was 10~12 months after surgery, with a median relapse-free survival time (RFS) of 15.3 months. ②The results of univariate analysis showed that histological type, degree of differentiation, depth of infiltration, lymph node metastasis, TNM staging, lymph node metastasis rate, vascular and neural invasion, cancer nodules, anastomotic fistula, traditional Chinese medicine syndromes, completion of chemotherapy, and oral administration of traditional Chinese medicine were correlated with postoperative RFS of colorectal cancer (P<0.05). ③The results of multivariate Cox regression analysis showed that lymph node metastasis [OR=1.724, 95% CI (1.353, 2.206)], vascular and neural invasion [OR=1.661, 95% CI (1.116, 2.465)], cancer nodules [OR=1.801, 95% CI (1.083, 2.983)], different traditional Chinese medicine syndromes [OR=1.624, 95% CI (1.126, 2.473)], completion of full cycle chemotherapy [OR=1.738, 95% CI (1.163, 2.341)] and oral administration of traditional Chinese medicine [OR=1.791, 95% CI (1.328, 2.264)] were independent risk factors affecting RFS after colorectal cancer surgery. Patients with internal damp-heat accumulation syndrome had a higher risk of recurrence and metastasis. Oral administration of traditional Chinese medicine and completion of full cycle chemotherapy could prolong the RFS of patients.ConclusionsThe recurrence and metastasis of colorectal cancer after radical surgery are influenced by factors such as lymph node metastasis, vascular and neural invasion, cancer nodules, completion of chemotherapy, traditional Chinese medicine syndromes, and oral administration of traditional Chinese medicine. Early traditional Chinese medicine syndrome differentiation-based treatment is beneficial to prolong the survival of patients after radical resection of colorectal cancer.

      摘要:ObjectiveTo investigate the molecular mechanism of action of Puerarin (Pue) in intervening Alzheimer's disease (AD) based on network pharmacology and molecular docking methods, and conduct animal experimental verification.MethodsUse Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), Integrated Pharmacophore Matching Platform (PharmaMapper), etc, to obtain the target of Pue. Use The Human Gene Database(GeneCards) to obtain AD related targets and intersection targets after mapping. Apply Cytoscape v3.9.1 to construct protein interaction network diagrams, and screen out core targets for GO enrichment analysis, KEGG signaling pathway analysis and molecular docking. The 5-month-old progerin-1/2 conditional double knockout (PS cDKO) mice were treated with intraperitoneal injection of Pue (100 mg/kg) based on gene identification, followed by behavioral testing, and after that the sample material was taken for molecular biological testing.ResultsA total of 118 core targets for Pue intervention in AD were obtained through screening. GO functional enrichment analysis revealed 148 items (P<0.05), including 100 items for biological process, 23 items for cell composition, and 25 items for molecular function. KEGG pathway analysis enriched 11 signaling pathways (P<0.05), mainly including Alzheimer's disease signaling pathway, phosphatidylino-sitol 3-kinase/protein kinase B (PI3K/AKT) signaling pathway and mitogen-activated protein kinase (MAPK) signaling pathway, etc. Molecular docking results showed that Pue had better docking activity with AKT Serine/Threonine Kinase 1 (AKT1), cysteine proteinase 3 (CASP3), albumin (ALB), epidermal growth factor receptor (EGFR), tumor necrosis factor (TNF), and vascular endothelial growth factor A (VEGFA). Behavioral results showed that Pue could improve the impaired spatial memory in PS cDKO mice (P<0.05). Western blot results showed that Pue down-regulated the expression of Erk1/2 phosphorylation in the prefrontal cortex of PS cDKO mice, and inhibited the expression of Cleaved Caspase-3.ConclusionBased on network pharmacology and experimental studies, it has been shown that Pue can intervene in the course of AD through multiple targets and pathways, which can provide a reference for clinical practice and new drug development.

      摘要:This paper introduced professor Shi Weiqun's experience in treating hepatic encephalopathy with Shengjiang Powder based on the theory of “release of fire depression”. Professor Shi believes that hepatic encephalopathy is prone to “stagnant heat”, and clinical syndrome differentiation should grasp the general pathogenesis of “stagnant heat”, all of which can be used in syndrome differentiation and application of “fazhi” with Shengjiang powder, which can improve clear yang, reduce turbid yin, and relieve stagnant heat. Regulating qi activity, helping evil release run through the treatment, the qi movement will be smooth, and the disease will be recovered.

      摘要:Taking the method of “acrid diffusing congestion” as the theoretical basis, the application of acrid herbs in the treatment of depression was investigated by observing the pathogenesis. Traditional Chinese medicine believes that “congestion” is the key pathogenesis of depression, including depression, phlegm, blood stasis, fire and deficiency, mixing with deficiency and excess; “Acrid” is yang in taste and the most dynamic in nature, and has a strong power of propagation, which can disperse congestion; “Diffusing formula treating congestion” is to use the diffusing formula to treat congestion diseases, which is in line with the pathogenesis of depression.

      摘要:This paper expounds the importance of the theory of “collateral disease-host-guest interaction” in the pathogenesis of connective tissue disease-interstitial lung disease (CTD-ILD) in traditional Chinese medicine. On this basis, it is proposed that obstruction of the lung and collaterals is the core pathogenesis of CTD-ILD, deficiency of the lung and collaterals is the basis of the pathogenesis of CTD-ILD, and damage of the lung and collaterals is the initial factor of CTD-ILD, and the block of phlegm and blood stasis and toxin is throughout. In addition, the basic treatment principles of “cleaning the guests” through the lung and collaterals and “protecting the master” reinforcing the lung and collaterals of CTD-ILD after a long illness are used in combination, which provides theoretical basis and diagnosis and treatment ideas for the clinical differentiation and treatment of CTD-ILD.

      摘要:ObjectiveTo analyze the effect of Xuanhushangtong Tablets combined with celecoxib on the rapid recovery of distal radius fractures.MethodsOne hundred patients with distal radius fractures were divided into control group and treatment group, with 50 cases in each group. All patients underwent the internal fixation of distal radius fractures under static suction combined anesthesia. All patients were treated with internal fixation of distal radius fractures under combined intravenous and inhalation anesthesia. The pain levels (VAS score) of the two groups were compared before surgery, 1 week and 2 weeks after surgery. At 2 weeks, 2 months, and 3 months after operation, wrist function (Cooney wrist score), the dorsal extension activity, flexion activity and quality of life (SF-36 score) were compared between the two groups.ResultsThe VAS scores at 1 weeks and 2 weeks after surgery were lower in the two groups than that before surgery, and compared with the control group, the VAS scores at 1 week and 2 weeks after surgery were significantly decreased in the treatment group (P<0.05). At 2 months, and 3 months after operation, the Cooney score, the dorsal extension activity, flexion activity and SF-36 score of the two groups were higher than those of 2 weeks after operation, and compared with the control group, the Cooney score, the dorsal extension activity, flexion activity and SF-36 score of the treatment group were significantly increased (P<0.05).ConclusionXuanhushangtong Tablet combined with celecoxib can alleviate the pain after surgery for distal radius fractures, improve the wrist mobility and improve the quality of life in the early stage, which is beneficial to the rapid recovery of patients.

      摘要:ObjectiveTo observe the effect of Compound Sophorae Decoction (CSD) on inflammatory factors and autophagy in colon tissue of mice with ulcerative colitis (UC), and to explore its mechanism of action.MethodsTwenty SPF C57BL/6 male mice were randomly divided into normal group, model group, CSD group and mesalazine group, with 5 mice in each group. After one week of adaptive feeding, UC mouse models were prepared by giving 3% dextran sulfate sodium (DSS) ad libitum for 1 week to mice in all three groups except the normal group. From the first day of modeling, the normal and model groups were intragastrically administered with 0.9% sodium chloride solution by mass fraction daily, and the CSD group and the mesalazine group were intragastrically administered once daily for 1 week with CSD (7.28 g/kg) and mesalazine (0.52 g/kg) respectively. Body weight, stool characteristics and hematochezia were recorded every day, and disease activity index (DAI) scores were measured. After the final gavage, the colonic tissues of mice in each group were taken to measure the length of the colon, and the degree of intestinal mucosal inflammatory injury was macroscopically observed for CMDI scoring. HE staining was used to observe the histopathological changes, ELISA was used to detect the contents of IL-1β and IL-18, and Western blot and RT-qPCR were used to detect autophagy-related molecule protein and mRNA expression.ResultsAccording to the inter-group comparison between normal group and model group, the DAI score, CMDI score, the expression of pro-inflammatory cytokines IL-1β, IL-18 and P62 molecules increased (P<0.05), the colon length was shortened (P<0.05), the expression of anti-inflammatory cytokines IL-4 and IL-10 decreased (P<0.05), and the expression levels of Beclin1 and LC3 protein and mRNA decreased significantly (P<0.05) in the model group. According to the inter-group comparison between the model group and the CSD and mesalazine groups, the DAI score, CMDI score, pro-inflammatory cytokines IL-1β, IL-18 and P62 molecules decreased (P<0.05), the colon length significantly increased (P<0.05), and the expression levels of anti-inflammatory factors IL-4, IL-10, Beclin1 and LC3 significantly increased (P<0.05) in the CSD group and mesalazine group. According to the inter-group comparison between the CSD group and the mesalazine group, the CMDI score increased (P<0.05), the expression of IL-1β, IL-18 and P62 increased (P<0.05), the expression of IL-4, IL-10, Beclin1 and LC3 decreased (P<0.05), and the differences in colon length as well as DAI score were not statistically significant (P>0.05) in the CSD group.ConclusionCSD can reduce the colonic inflammation and pathological injury in UC model mice by promoting the expression of autophagy in colonic tissue.

      摘要:ObjectiveTo analyze and identify the chemical composition and the main absorbed components in mice after oral administration of Huangqi Decoction granules by UHPLC-Q-Exactive Orbitrap HRMS technique.MethodsChromatographic separation was performed on an Acquity UPLC BEH C18 column (100 mm×2.1 mm, 1.7 µm), and 0.1% formic acid aqueous solution-methanol as the mobile phase with gradient elution. The mass spectra were collected using electrospray ionization source ESI+ and ESI- modes with alternating scans and automatic triggering of the secondary mass spectrometry scan function (Full MS/dd-MS2). The chemical composition in Huangqi Decoction granules, absorbed components and metabolites in the serum of mice after oral administration of Huangqi Decoction granules were identified by Xcalibur 3.0 software and compared with reference standards, literatures, and database.ResultsA total of 167 chemical compositions were identified from Huangqi Decoction granules, and 60 prototypical components and 43 metabolites were detected in the serum, including liquiritigenin, calycosin, formononetin and astragaloside Ⅳ, etc.ConclusionThis study successfully completed the rapid identification of chemical composition from Huangqi Decoction granules, and clarified main blood components after treated with Huangqi Decoction granules, which can provide partial reference for the analysis of effective substances, study of quality control and explore of action mechanism of Huangqi Decoction granules.

      摘要:ObjectiveTo investigate the effects of notoginsenoside R1, ginsenoside Rg1 and Rb1 on intestinal mucus permeation by using three in vitro intestinal mucus permeation models[purified mucin infiltration model (PIM), artificial intestinal mucus infiltration model (AIM) and rat intestinal mucus infiltration model (RIM)] in order to provide experimental basis for the oral absorption and application of notoginseng Radix Et Rhizoma.MethodsQuantitative analysis methods of notoginsenoside R1, ginsenoside Rg1 and Rb1 were established by high performance liquid chromatography (HPLC), and their linear range, precision, stability, reproducibility and sample recovery rate were investigated. The apparent permeability coefficients (Papp) of different concentrations of notoginsenoside R1, ginsenoside Rg1 and ginsenoside Rb1 solutions on three intestinal mucus permeation models were determined and analyzed to compare their intestinal mucus permeation effects.ResultsThe HPLC methods for the quantitative analysis of notoginsenoside R1, ginsenoside Rg1 and ginsenoside Rb1 were successfully established. The precision, stability, reproducibility and recovery all met the requirements. The permeability results of three kinds of saponins in PIM, AIM and RIM models showed that the permeability of notoginsenoside R1 and ginsenoside Rg1 and ginsenoside Rb1 in the three models increased in different degrees with the increase of drug concentration. In PIM, notoginsenoside R1 showed a significant increase in the permeation rate compared with ginsenoside Rb1 and ginsenoside Rg1 at different concentrations (P<0.05). In AIM, the permeation rate at a concentration of 20 g·L-1 was PappR1 > PappRb1 > PappRg1 (P<0.05). In RIM, the permeation rate at a concentration of 20 g·L-1 was PappRg1 > PappR1 > PappRb1 (P<0.05).ConclusionsNotoginsenoside R1, ginsenoside Rg1 and ginsenoside Rb1 with different concentrations have good stability and reproducibility in three intestinal mucus permeation models, and the permeability increases with the increase of drug concentration. Notoginsenoside R1 with strong water solubility and relatively low molecular weight is more permeable in PIM and AIM with less lipids, while ginsenoside Rg1 with strong fat solubility is stronger in RIM.

      摘要:ObjectiveTo observe the effect of electric acupuncture (EA) on promoting the repair of gastric mucosal injury in mice with stress ulcer, and explore the correlation between repair effect of EA and promoting repair factor release and up-regulating hypoxia-inducible factor-1α (HIF-1α) expression.MethodsMale C57BL/6 mice were randomly divided into control group (Sham group), model group (CRS group) and model+EA group (CRS+EA group), with 9 mice in each group. Except for the Sham group, the stress ulcer model (CRS model) was established in the CRS and CRS+EA groups after 3 days of cold-restraint stress, and EA intervention was performed 3 d after modeling. In the CRS+EA group, EA was applied to Zusanli (ST 36) and Liangqiu (ST 34) for 15 min, once in the morning and once in the afternoon, for 2 d. On day 6 of the experiment, the whole stomachs were separated from mice and were sampled to assess the gastric mucosal ulcer index (UI) score. The mRNA expression levels of repair factors [including trefoil factor 2 (TFF2), vascular endothelial growth factor (VEGF) and heat shock protein 70 (HSP70)] were determined by RT-qPCR, and the protein content and mRNA expression level of HIF-1α were determined by Western blot and RT-qPCR.Results①The CRS group and the CRS+EA group had significantly higher gastric mucosal UI score than the Sham group (P<0.05); and the CRS+EA group had significantly lower UI score than the CRS group (P<0.05). ②The CRS group had significantly higher levels of VEGF and HSP70 than the Sham group (P<0.05), and the CRS+EA group showed significantly higher levels of TFF2, VEGF and HSP70 than the Sham group (P<0.05). The CRS+EA group had significantly higher levels of TFF2, VEGF and HSP70 than the CRS group (P<0.05). ③There was no statistically significant difference in HIF-1α expression between the CRS group and the Sham group (P>0.05), while the CRS+EA group had significantly higher HIF-1α expression than the Sham group (P<0.05) and the CRS group (P<0.05).ConclusionElectric acupuncture promotes the expression of repair factors (TFF2, VEGF, HSP70) and facilitates the repair of injured gastric mucosa in mice with stress ulcer. The efficacy of EA may be associated with a significant upregulation of HIF-1α expression after treatment.

      摘要:Long-term residual negative symptoms and cognitive impairment are common in schizophrenia, which severely affect the quality of life and social functioning of patients. Therefore, intervention for residual symptoms has become an important component of schizophrenia rehabilitation. Traditional Chinese medicine (TCM) exercise therapy (Gongfa) is a moderately intense aerobic mind-body exercise that emphasizes the “form-spirit integration” and has been progressively applied in clinical practice as an adjunct to schizophrenia rehabilitation. In this paper, we review the status quo of the application of TCM exercise therapy in the rehabilitation of schizophrenia residual symptoms, as well as discuss and analyze TCM exercise therapy based on TCM theories, for the purpose of providing a theoretical basis for the application of TCM exercise therapy in the rehabilitation of schizophrenia residual symptoms.

      摘要:Ulcerative colitis (UC) is a chronic immune-mediated intestinal inflammatory disease caused by complex interactions among host genetic risk factors, environmental factors and gut microbiota imbalance, and can increase the risk of colorectal cancer. Specific bacterial metabolites, bile acids, can act as signal transduction molecules that affect the susceptibility of the host to intestinal inflammation. Dysregulated bile acid signaling may contribute to the dysregulation of the immune response in UC patients. In recent years, there has been considerable interest in the study of traditional Chinese herbal medicines (single herbs or extracts, herbal pairs and herbal formulas) interfering with bile acid metabolism and its receptors. We review the advances in the studies of bile acid metabolism and its receptors regulating intestinal immunity in UC patients and the intervention of traditional Chinese herbal medicines on bile acid metabolism and its receptors in order to provide ideas for the clinical treatment of ulcerative colitis.
