      摘要:ObjectiveTo explain the “disease-syndrome-pathogenesis-method coherence” rule system and its logical relationships in the revised editions of Classification and Codes of Diseases and Patterns of Traditional Chinese Medicine and Clinic Terminology of Traditional Chinese Medical Diagnosis and Treatment (hereinafter referred to as “the new edition of TCM National Standards”) for the purpose of promoting the soul recasting of contemporary TCM clinics and its return to the academic noumenon and clinical path.MethodsBased on the retrospective literature review of traditional Chinese medicine, terminology and standardology, we adopted computer ontology technology and artificial intelligence to trace the origin of standards, standard knowledge system, standard rule system and TCM disease classification system, organize the concepts of “disease”, “syndrome”, “pathogenesis” and “method”, analyze the academic concepts of “seeking the root of the disease” and “identifying the root of the disease and observing the pathogenesis of the disease”, and present the internal logical relationships of the “disease-syndrome-pathogenesis-method coherence” in the new edition of TCM National Standards with the help of the “TCM National Standard Application Service Platform” (hereinafter referred to as “Standard Platform”).ResultsThe new edition of TCM National Standards has defined the connotation and extension of TCM terms such as “disease”, “syndrome”, “pathogenesis” and “method”, etc., determined the basic rule that must be followed in the TCM clinical diagnosis and treatment to be “seeking the root of the disease, connecting disease differentiation with syndrome differentiation, matching pathogenesis with the syndrome, and prescribing in accordance with the method”. Being the “root” and “soul” of TCM clinical thinking and diagnosis and treatment standard, it is this basic rule that runs through the whole revising process of TCM National Standards. The concept and rule of “disease-syndrome-pathogenesis-method coherence” have also been incorporated into the knowledge system and rule system of the new edition of TCM National Standards.ConclusionThe diagnosis and treatment rule of “disease-syndrome-pathogenesis-method coherence” contained in the new edition of TCM National Standards can be fully demonstrated and widely applied through the Standard Platform. Meanwhile, it can guide TCM clinic practice to follow its own academic and clinical path and standardize its medical activities.

      摘要:Professor Wang Qi’s clinical experience in treating extra-pulmonary diseases through medicines for regulating the lung meridian is introduced. As the lungs regulate the water passages, govern the skin, store the corporeal soul, and have an exterior-interior relationship with the large intestine, extra-pulmonary diseases such as skin diseases, hydrological diseases, emotional diseases and colorectal related diseases are closely related to the lungs. The treatment is therefore based on the physiological characteristics of the lungs. The treatment methods of clearing lung heat, ventilating lung qi, and moistening the lungs to calm the corporeal soul are introduced, which possess considerable clinical value and are worthy of promotion.

      摘要:The origins and processed products of Rehmanniae Radix were analyzed, by consulting the ancient Chinese herbal books of origin changes , cultivation conditions, the research on Rehmanniae Radix processed products in the tomb of Haihunhou, as well as combining with the laws of ancient Chinese climate change. The results found that the change of the origin of Rehmanniae Radix and the use of processed products in different periods were closely related to climate change. The natural environment affected the authenticity and processing methods, and also provided the literature basis for the selection of the original producing area and the redevelopment of Rehmanniae Radix processed products in the future.

      摘要:ObjectiveTo evaluate the efficacy and safety of Ginkgo Folium extract in the treatment of diabetic kidney disease(DKD), and provide evidence for the clinical application.MethodsThe literatures of PubMed Database, Cochrane Library, Embase Database, Wiley Database, Ovid Database, CNKI, VIP, Wan Fang Database and CBM were searched by computer, and the quality of literatures was evaluated by the Cochrane collaboration’s tool for assessing risk of bias. The included efficacy indicators were extracted and analyzed with RevMan 5.3 software.Results①A total of 1 375 patients in twenty randomized controlled trails diagnosed with DKD were included. ②Meta-analysis results showed as follows: compared with western medicine treatment alone, Ginkgo Folium extract combined with western medicine treatment in DKD patients could improve total efficacy rate[OR=5.52, 95%CI(3.46,8.81), P<0.000 01]. ③UAER, TC, TG, SCr, and BUN all showed great heterogeneity, so Ginkgo Folium extract was divided into intravenous drip group and oral group according to different medication methods, and subgroup analysis was conducted. The results showed that Ginkgo Folium extract could significantly reduce UAER, TC and TG in patients with DKD both in the intravenous drip group and the oral group(P<0.000 1). In terms of the efficacy of Ginkgo Folium extract on SCr, there had no statistically significant difference in intravenous drip(P>0.05), while there had statistically significant difference in the oral group(P<0.05). In terms of the efficacy of Ginkgo Folium extract on BUN, there had no statistically significant difference both in intravenous drip group and oral group(P>0.05). ④No significant adverse reactions were found in the included studies.ConclusionCompared with western medicine alone,Ginkgo Folium extract combined with western medicine could improve the clinical efficacy, reduce proteinuria, improve lipid metabolism in DKD patients, and with good safety.

      摘要:ObjectiveTo evaluate the clinical effect of acupuncture and moxibustion assisted improvement of intrauterine blood perfusion in patients undergoing frozen-thawed embryo transfer (FET) based on network meta-analysis.MethodsRelevant literature on acupuncture and moxibustion assisted intervention in FET was retrieved from PubMed, Embase, Cochrane Library, Chinese National Knowledge Infrastructure (CNKI), Wanfang Data (WanFang), VIP Chinese Scientific Journals Database (VIP), Chinese Biomedical Literature Service System (SinoMed) and other databases from the date of database establishment up to March 31, 2022. Cochrane risk of bias assessment tool was used to screen the included literature and evaluate their quality, and RevMan 5.3, GeMTC0.14.3 and Stata 16.0 were used for data analysis.ResultsThe included 17 articles involved 1,679 patients, 15 interventions and 3 evaluation criteria. As shown in the results of network meta-analysis, the top 3 SUCRA-based treatment ranks were as follows: ①Type A endometrial blood flow rate: acupoint injection + Western medicine treatment (97.8%), auricular point + Western medicine treatment (84.4%), and acupuncture + moxibustion (71.0%); ②Uterine artery blood flow pulsation index (PI): transcutaneous electrical acupoint stimulation (TEAS) + Western medicine treatment (15.3%), TEAS (17.5%), and acupuncture + moxibustion (30.7%); ③Uterine artery blood flow resistance index (RI): electroacupuncture (16.7%), TEAS (28.7%), and acupuncture + moxibustion (36.1%).ConclusionsAmong the acupuncture and moxibustion assisted protocols to improve FET intrauterine blood perfusion, acupoint injection + Western medicine treatment was the most effective in improving endometrial blood flow classification; TEAS + Western medicine treatment, and electroacupuncture were the most effective in reducing PI and RI; and acupuncture + moxibustion had the optimal combined mean efficacy. However, the above conclusion still needs to be verified by a larger sample of high-quality studies.

      摘要:ObjectiveTo investigate the distribution of related symptoms and traditional Chinese medicine (TCM) patterns in women with perimenopausal syndrome.MethodsThe data of patients with perimenopausal syndrome were collected with the self-developed menopausal application, and modified Kupperman Index (KI), menopausal status classifications and TCM syndromes were determined to analyze the distribution of main symptoms, menopausal status and TCM syndromes. The differences in modified KI scores of patients with different TCM syndromes, as well as the differences in age of patients with different menopausal status and TCM syndromes were compared.Results①A total of 846 valid cases were collected, of which more than half presented with fatigue (95.74%), mood fluctuation (89.13%), insomnia (86.05%), painful intercourse (70.33%), palpitations (68.91%), headache (68.91%), bone and joint pain (65.37%), depression and suspicion (64.07%), hot flashes and sweating (63.36%), and dizziness (62.53%), and other symptoms. ②The majority of cases collected in our study were mild and moderate, with 296 (34.99%) mild ones and 399 (47.16%) moderate ones. According to the TCM syndrome differentiation, the predominant ones were kidney yin-yang deficiency syndrome (42.67%) and kidney deficiency and liver depression syndrome (32.51%), and the rest were liver-kidney yin deficiency syndrome (13.59%) and disharmony between heart and kidney syndrome (11.23%). ③The modified KI scores of patients with different TCM syndromes differed, with the highest scores in patients with kidney deficiency and liver depression syndrome and the lowest scores in patients with liver-kidney yin deficiency syndrome, and the differences were statistically significant (P<0.05). Among the subscales of the modified KI, patients with kidney deficiency and liver depression syndrome had the highest score in the subscale of hot flashes and sweating (P<0.05); patients with disharmony between heart and kidney syndrome had the highest score in the subscale of insomnia (P<0.05); and patients with kidney yin-yang deficiency syndrome had the highest score in the subscale of depression and suspicion (P<0.05). ④Patients with severe menopausal status were the oldest, and the menopausal status grading increased with age (F=28.31, P<0.001). ⑤Patients with kidney yin-yang deficiency syndrome were the youngest, followed by those with kidney deficiency and liver depression syndrome, liver-kidney yin deficiency syndrome, and disharmony between heart and kidney syndrome (F=13.606, P<0.001).ConclusionsFatigue, mood fluctuation, and insomnia are common symptoms in patients with perimenopausal syndrome, and the dominant TCM syndromes are kidney yin-yang deficiency syndrome and kidney deficiency and liver depression syndrome. Patients with kidney deficiency and liver depression syndrome have a relatively more severe menopausal status, and the menopausal status worsens with age.

      摘要:ObjectiveTo systematically evaluate the efficacy and safety of Dachengqi Decoction (DCQD) combined with octreotide and conventional therapy in the treatment of early postoperative inflammatory small bowel obstruction (EPISBO).MethodsClinical randomized controlled trials (RCTs) that observed the effectiveness and safety of DCQD plus octreotide and conventional therapy in treating EPISBO with octreotide plus conventional therapy as the control were retrieved by searching the following databases as China National Knowledge Infrastructure (CNKI), Wanfang Data Knowledge Service Platform (WanFang Data) and VIP Chinese Journal Service Platform (VIP), etc. The collected RCTs were published between the dates of database establishment and August 5, 2022. The Cochrane systematic review manual was used to evaluate the methodological quality of the included RCTs and extract relevant data, and RevMan 5.4 was used to perform a meta-analysis of the extracted data.ResultsThe included 6 articles involved 624 cases. The meta-analysis showed that the total effective rate of DCQD plus octreotide and conventional therapy for EPISBO was higher than that of octreotide plus conventional therapy [OR=7.16,95%CI=(3.32,15.44)], and DCQD plus octreotide and conventional therapy could better relieve abdominal pain and bloating [MD=-1.81,95%CI=(-2.71,-0.92)], facilitate anal gas evacuation [MD=-0.95,95%CI=(-1.21, -0.69)] and defecation [MD=-0.62,95%CI=(-0.95,-0.29)], and shorten the time course of symptom disappearance [MD=-1.55,95%CI=(-2.70, -0.40)], with a statistically significant difference (P<0.01). DCQD plus octreotide and conventional therapy had no significant advantage in lowering levels of tumor necrosis factor-α (TNF-α) [MD=-3.87,95% CI=(-8.32,0.58)], hypersensitive C-reactive protein (hs-CRP) [MD=-4.11,95% CI=(-8.90,0.69)], interleukin-6 (IL-6) [MD=-41.86,95%CI=(-107.38,23.65)], and the incidence of adverse reactions [MD=0.91, 95%CI=(0.40,2.07)] (P>0.05).ConclusionDCQD plus octreotide and conventional therapy has quite good efficacy in the treatment of EPISBO, but the conclusion needs to be validated by studies of higher quality due to the low quantity and quality of included literature.

      摘要:This paper introduced Xu Zhenye’s clinical experience in the treatment of advanced non-small cell lung cancer without gene mutation from four aspects: syndrome differentiation according to the classical books, emphasizing tongue diagnosis in differential diagnosis, reinforcing tonifying essence and qi in prescription, paying attention to the combination of disease differentiation and syndrome differentiation. And one proven case was presented.

      摘要:This paper introduced professor Yu Jian’s experience in treating children with SARS-CoV-2 infection based on the principle of strengthening vital qi and detoxification, and treating both symptoms and root causes. Professor Yu proposed that children infection should be mainly manifested in Wei and Qi defence; the symptoms of mild type were mainly wind-heat syndrome and wind-heat with dampness syndrome; the common type had the symptoms of the virus entering the lungs; in the severe/critical cases, SARS-CoV-2 infection was more in-depth. For the severe/critical type, the combination of traditional Chinese and western medicine was needed to treating both symptoms and root causes. Professor Yu modified the Xuanfei Baidu Recipe to make it more suitable for severe/critical children infection to clearing heat and eliminating pathogen, purging lung and detoxification. One proven critical case was presented.

      摘要:Notoginseng Radix et Rhizoma is a commonly used traditional Chinese herbal medicine in membrane protection method, and its application in gastrointestinal diseases often relates to the protection of mucosal barrier. Through literature review and analysis, we discussed the application and development of Notoginseng Radix et Rhizoma in membrane protection method in terms of its use in protecting the gastric membrane and intestinal membrane, administration method, and the related modern pharmacological research, with the expectation of providing more ideas and references for the contemporary clinical application of Notoginseng Radix et Rhizoma in the treatment of gastrointestinal diseases.

      摘要:This paper introduces Professor Cui Yun’s experience in treating male diseases from spleen and stomach. It is believed that spleen and stomach transport essence, qi, blood and body fluid, nourish other four visceral organs, and then play a regulatory role in male reproductive organs. Abnormal of spleen and stomach is an important cause of male diseases. To treat male diseases from spleen and stomach, it is necessary to balance the deficiency and excess of spleen and stomach, and rationally choose the method of replenishing deficiency and reducing excess. Two medical cases are also presented.

      摘要:This paper discussed the theoretical basis of treating numbness from the five zang organs, summarized the clinical experience of ancient and modern doctors, and concluded five methods of treating numbness: from the lung treatment, supplementing the lung and strengthening the health , improving qi and dredging collaterals should be applied; from the spleen treatment, invigorating the spleen and replenishing qi, eliminating phlegm and dampness should be applied; from the liver treatment, soothing the liver and relieving depression, nourishing the blood and softening the liver should be applied; from the kidneys treatment, nourishing yin and yang, protecting fundamental and filling the kidney essence should be applied; from the heart treatment, tonifying the heart and nourishing the blood, dredging meridians and calming mood should be applied.

      摘要:The symptoms, syndromes, pathogenesis and medicinal herbs of Zeqi Decoction were analyzed by reviewing ancient classics and modern literature. Zeqi Decoction syndrome mainly manifests such symptoms as fever or no fever, coughing with qi counter-flow, dull pain in the chest and hypochondrium, fluid retention in the chest, and deep pulse, etc. The pathogenesis can be summarized into three aspects from the superficial layer to the deep layer: pathogenic qi invading the lung, and the lung failing to disperse and descend; the depressed pathogenic qi transforming into heat, and phlegm intermingling with blood stasis; qi deficiency of the lung and spleen, and internal fluid retention. The three aspects can appear simultaneously and intermingle through the whole disease course. Zeqi Decoction uses medicinal herbs in both cold and warm nature, combines reinforcement with reduction, and can treat both manifestations and root cause of disease. It is applicable to the disorder of phlegm and fluid retention with intermingled deficiency and excess, cold and heat as it has the effects of expelling water retention and activating yang, clearing heat and removing stasis, stopping coughing and resolving phlegm, and strengthening the spleen and replenishing qi. In modern clinical application, Zishen might refer to different medicinal herbs for different diseases. For instance, it could be Quanshen (Bistortae Rhizoma) for treating acute respiratory tract infection, or Shijianchuan (Salviae Chinensis Herba) for treating lung cancer, or Ziwan (Asteris Radix et Rhizoma) for treating chronic bronchitis.

      摘要:ObjectiveTo observe the clinical efficacy of press needle combined with opioid analgesics in treating moderate-to-severe cancer pain and its effect on serum levels of β-endorphin and substance P.MethodsSeventy-six patients with moderate-to-severe cancer pain were randomized into a treatment group (n=38) and a control group (n=38). Patients in the control group received conventional treatment (including active treatment of primary diseases, nutritional support, and opioid analgesics) while those in the treatment group were treated with press needle in addition to the conventional treatment. The treatment in both groups lasted 14 consecutive days. The incidence of adverse reactions was observed, and the pain numerical rating scale (NRS), Brief Pain Inventory (BPI), Karnofsky Performance Status (KPS), and serum levels of β-endorphin and substance P were assessed and compared before and after treatment.Results①There were 4 drop-outs in the treatment group and 3 drop-outs in the control group during the trial. Finally, 69 patients completed the trial with 34 patients in the treatment group and 35 patients in the control group. ②After treatment, the pain NRS score decreased and KPS score increased in the treatment group (P<0.05), while the differences before and after treatment in pain NRS and KPS scores in the control group were not statistically significant (P>0.05); After treatment, the pain NRS score in the treatment group was lower than that in the control group and the KPS score in the treatment group was higher than that in the control group (P<0.05). ③In the treatment group, the scores of worst pain in last 24 hours, pain on average over the last 24 hours, pain right now, impact on general activity, mood, normal work, sleep, and total score of impact of pain after treatment were lower than those before treatment (P<0.05), and the scores of impact on walking ability increased after treatment (P<0.05), but the differences before and after treatment in the scores of the above items in the control group were not statistically significant (P>0.05); After treatment, the differences between the two groups in the scores of the above items were not statistically significant (P>0.05). ④After treatment, the serum level of β-endorphin increased and that of substance P decreased in both groups (P<0.05); After treatment, the serum level of substance P in the treatment group was lower than that in the control group (P<0.05). ⑤The incidence of constipation, nausea and vomiting and the total incidence of adverse reactions in the treatment group were lower than those in the control group (P<0.05).ConclusionsAdditional use of press needle has a better clinical effect on patients with cancer pain as it can relieve pain, improve patient’s quality of life, and reduce adverse reactions due to opioid analgesics. The mechanism may be related to the regulation of serum β-endorphin and substance P expression levels.

      摘要:ObjectiveTo screen out the main differential components of mountain-cultivated ginseng, cultivated ginseng, red ginseng and American ginseng based on ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS) and multivariate statistical methods, and to establish a rapid nondestructive method for identifying different varieties of ginseng based on near-infrared spectroscopy (NIRS) technology and multivariate statistical methods.MethodsMass spectrometry data was collected in negative ion mode using the Waters Acquity UPLC BEH C18 chromatographic column and gradient elution with a mobile phase of 0.1% formic acid in water and acetonitrile, and ginsenosides were identified in mountain-cultivated ginseng, cultivated ginseng, red ginseng and American ginseng by comparing with literature data and reference substances. The main differential components of the four kinds of ginseng species were screened by orthogonal partial least squares discriminant analysis (OPLS-DA) with the variable importance projection (VIP) value greater than 1. The NIRS of the four kinds of ginseng species powders was collected, the instrument noise and background interference were deducted by spectral pre-processing method, and a rapid nondestructive discriminant model of the four kinds of ginseng species powders was constructed through OPLS-DA.ResultsA total of 54 ginsenoside components were identified in the four kinds of ginseng species, and 18 main differential components were screened out. The NIRS technology using the second-order derivative preprocessing method and OPLS-DA could help obtain the discriminant analysis of the four kinds of ginseng species.ConclusionsThe material basis and differential components of mountain-cultivated ginseng, cultivated ginseng, red ginseng and American ginseng have been elucidated, which provides a reference for the quality markers of different varieties of ginseng products. Meanwhile, a rapid nondestructive method for identifying different varieties of ginseng powders has been established, which provides a strong technical support for identifying adulteration of different ginsengs in the production and on the market.

      摘要:The research progress on traditional Chinese medicine(TCM) in preventing and treating hypertensive left ventricular hypertrophy (LVH) has been reviewed. Studies have found that Astragali Radix, Salviae Miltiorrhizae Radix Et Rhizoma, Gastrodiae Rhizoma etc. have cardiac protective effects and can improve LVH related indicators. Antihypertensive drugs combined with TCM syndrome differentiation can further improve the reversal rate of LVH and its prognosis, and the mechanism is mostly related to the regulation of cytokines, inhibition of Renin-Angiotensin-Aldosterone System, improvement of oxidative stress and inhibition of cell signal transduction pathway.

      摘要:This paper reviewed the relationship between skeletal muscle atrophy and apoptosis in chronic obstructive pulmonary disease (COPD) and the research progress on traditional Chinese medicine intervention. Based on the theory of “spleen dominating muscles”, it explored that the apoptosis of skeletal muscle cells in COPD could cause skeletal muscle atrophy. And it reviewed the mechanism of traditional Chinese medicine with the function of strengthening the spleen and tonifying the lung to intervene skeletal muscle atrophy in COPD by regulating cell apoptosis, from the aspects of traditional Chinese medicine extract, traditional Chinese medicine compound and traditional Chinese medicine preparation.
