摘要:Omicron variant of SARS-CoV-2 has the characteristics of rapid transmission, insidiousness, and relatively decreased virulence. Most infected patients have relatively mild clinical symptoms, but a certain proportion of elderly people with underlying diseases still turn into severe or critical cases. Since the spring of 2022, community and cluster transmission of COVID-19 have coexisted in Shanghai, and a high proportion of severe cases are those elderly with underlying diseases. Therefore, the key to reducing the mortality rate of COVID-19 lies in the effective treatment of severe cases. To treat and cure severe cases, Shanghai has established a “multidisciplinary joint diagnosis and treatment” model, insisting on the integrated application of traditional Chinese and western medicine, and emphasizing early intervention, syndrome differentiation-based treatment, one-person-one-policy, one-person-one-prescription, and preemptive treatment for disease interception, for the purpose of effective disease progression control and decrease in mortality rate. The article summarizes Shanghai’s diagnosis and treatment model and experience in treating severe COVID-19 patients and tries to improve the technical system of traditional Chinese medicine in treating severe COVID-19 cases.

摘要:ObjectiveTo investigate Shanghai residents’ acceptance of traditional Chinese medicine (TCM) for combating novel coronavirus disease (“COVID-19”) pandemic.MethodsConvenience and volunteer sampling methods were used to investigate the acceptance of TCM to combat the COVID-19 among medical personnel, community volunteers and residents in Shanghai.Results①A total of 662 valid questionnaires were collected. Among the 662 respondents, 41.1% were male, the age ranged from 20 to 60 years, and 29.2% were engaged in the medical profession. The results of univariate analysis showed that gender, age, occupation, education level, and personal monthly income had no significant effect on the acceptance of TCM. ②In this survey, 639 respondents believed in TCM treatment, accounting for 96.5% of the total. Among them, 584 respondents had used TCM before, accounting for 91.4%; 600 respondents showed their willingness to recommend others to use TCM for the pandemic prevention and control, accounting for 93.9%; and 500 respondents recommended TCM prescriptions alone to fight against the COVID-19, accounting for 78.2%. ③Compared with respondents who had never used TCM, those who had used TCM before were more likely to recommend others to use TCM for the pandemic prevention and control (P=0.004); those who had used TCM before were more concerned about the flavor and other issues of TCM (P=0.054, P=0.003), and those who had not used TCM before were more concerned about effectiveness and safety of TCM (P=0.005,P=0.045). ④Residents in Shanghai showed a wide range of demands for TCM, and the strongest ones were ranked in descending order: TCM treatment, TCM prevention and TCM science popularization, with a frequency of 452, 443, and 424, respectively.ConclusionsShanghai residents have a high level of acceptance of TCM and its use in the prevention and treatment of COVID-19 pandemic. Therefore, it is recommended to further the application of TCM in the pandemic prevention and control in response to the realistic demands.

摘要:The pathogenesis of collateral disease is characterized by stasis and stagnation. Insect drugs are good at dispelling the wind and dredging collaterals, resolving phlegm and removing stasis.Post-infectious cough and collateral disease share the similar pathogenesis. Therefore, in this article, we discussed the application of insect drugs in the clinical treatment of post-infectious cough based on the theory of collateral disease for the purpose of providing new insights for the treatment of post-infectious cough.

摘要:Prolapsed/sequestrated lumbar disc herniation has a tendency to resorb, and traditional Chinese medicine (TCM) conservative treatment can facilitate the process of resorption. The disease can often be relieved or even cured by non-surgical treatment with traditional Chinese medicine. This paper discusses the incidence of disc resorption in these two types of lumbar disc herniation, methods for predicting the occurrence of resorption, selection of non-surgical TCM treatment, and assessment of treatment risks.

摘要:ObjectiveTo systematically evaluate the therapeutic effects of balance acupuncture on scapulohumeral periarthritis.MethodsThe literature of clinical randomized controlled trials on balanced acupuncture for the treatment of scapulohumeral periarthritis was searched in the databases of Chinese National Knowledge Infrastructure (CNKI), Wan Fang Data, VIP (Chongqing VIP Chinese Scientific Journals Database), China Biology Medicine disc (CBM disc), PubMed and Web of Science from the date of database establishment up to June 28, 2021, and the included literature was assessed for risk of bias using RevMan 5.3, and meta-analysis was performed with Stata 16.0.ResultsOverall, 28 pieces of eligible literature and 13,354 patients (experimental group: 11,447; control group: 1,907) were included in this study. According to the results of meta-analysis, we observed that the treatment group with balanced acupuncture had better clinical outcomes [RR=0.95, 95% CI: 0.91~0.98, P=0.004] and better improvement in visual analogue scale (VAS) score of pain [WMD=-1.90, 95% CI: -2.71~-1.09, P<0.001], shoulder activity scale scores [elevation: WMD=11.20, 95% CI: 7.10~15.29, P<0.001; abduction: WMD=7.40, 95% CI: 4.29 - 10.52, P<0.001 ; posterior extension: WMD=4.42, 95% CI: 2.36~6.49, P<0.001; external rotation: WMD=8.62, 95% CI: 7.65~9.59, P<0.001, and CMS score [WMD=7.50, 95% CI: 5.26~9.74, P<0.001] than the control group.ConclusionsBalance acupuncture is effective in treating scapulohumeral periarthritis, which can effectively alleviate pain and improve limited shoulder joint movement. However, the conclusions obtained have to be validated by future studies due to the limited quantity and quality of the literature.

摘要:Professor Liu Ximing’s experience in treating tinnitus from fire and heat was introduced. According to “tinnitus was caused by the rise of yang qi, just like the growth of all things” in Interpretation of Pulse, Su Wen, the theory that fire burning was the core pathogenesis of tinnitus was proposed. Guided by the theory of deficiency and excess, the pathogenesis of fire and heat could be divided into yang excess, qi stagnation, pathogen obstruction and yin deficiency. Correspondingly, treatments could be adopted in four ways of clearing heat and fire, promoting qi and relieving depression, dispelling pathogen, and replenishing yin and yang. Messenger drugs such as Scallion, Acori Tatarinowii Rhizoma and Magnetitum were essential for efficacy. By regulating the balance of zang-fu, adjusting deficiency and excess, and clearing the origins of fire and heat, hearing would be recovered. Four cases were appended to illustrate the theory.

摘要:The clinical experience of Professor Ding Yuanqing in the treatment of medication-overuse headache is introduced by analyzing a proven medical case. It is considered that the syndrome is caused by long-term headache, improper use of medication. In this article, the pathogenesis blood deficiency and collaterals obstruction, which is closely related to Jueyin liver and pericardium, is proposed firstly. The main pathogenesis is deficiency of yin and blood and the secondary one is endogenous wind disturbing collaterals and turbid phlegm causing blood stasis. Long-term disease will enter the collaterals, resulting in disharmony of body and spirit. In clinic, based on the theory of syndrome differentiation of the six meridians for headache, the syndrome can be treated by the methods of supplementing deficiency and purging excess-evil, simultaneously treating body and mind. Therefore, a good result can be received in treating the syndrome by using the modified prescription according to the change of syndrome as the Sishen Powder has the functions of expelling wind and dredging collaterals, regulating qi and blood, and dispersing obstruction to stop pain. One case was appended to illustrated the therapy.

摘要:A model of six meridians system containing three yin meridians and three yang meridians is constructed based on Huangdi Neijing (The Yellow Emperor’s Inner Canon). The model contains the traditional Chinese medicine (TCM) concepts of yin and yang, exterior and interior, internal organs, meridians and collaterals, three yin and three yang, twelve divisions and sequence, and these concepts can be mutually corroborated and interconnected in this system. The model’s explanation has defused controversies over the nature of the six meridians in history, reflects the holistic view of the six meridians system with three yin and three yang, and corresponds with “theory of synthesized concepts”.

摘要:ObjectiveTo observe the clinical efficacy of modified Xuanbai Chengqi Decoction (XBCQ) in the treatment of mycoplasma pneumoniae pneumonia (MPP) of phlegm-heat blocking lung syndrome, investigate the effect of modified XBCQ on the intestinal microflora and the content of secretory immunoglobulin A (sIgA) in pediatric patients, and explore the mechanism of traditional Chinese medicine (TCM) in the treatment of MPP based on gut-lung axis.MethodsSixty children with MPP of phlegm-heat blocking lung syndrome were randomly divided into a control group (n=30) and an observation group (n=30); and 30 healthy children were recruited at the same time as a normal group. The control group was administered with azithromycin, and the observation group was administered with modified XBCQ together with the treatment of the control group. The treatment of both groups lasted 5d. We observed the clinical efficacy, collected feces samples from children in each group, and used real-time quantitative polymerase chain reaction to detect the number of Bifidobacterium, Lactobacillus, Ruminococcus flavefaciens and Clostridium butyricum, and enzyme-linked immunosorbent assay to detect the sIgA content. We used the Person correlation coefficient to describe the correlation between the number of intestinal microflora and the sIgA content in feces, and we also performed safety evaluation.Results①The total effective rate of the observation group was 93.33%, and that of the control group was 70.00 %. The clinical efficacy of the observation group was better than that of the control group (P<0.01). ②The number of Bifidobacterium, Lactobacillus, Ruminococcus flavefaciens and Clostridium butyricum in feces was lower in children with MPP of phlegm-heat blocking lung syndrome than that in healthy children of the normal group (P<0.001); after treatment, the number of Bifidobacterium, Lactobacillus, Ruminococcus flavefaciens and Clostridium butyricum increased in the observation group (P<0.001) and was higher than that in the control group (P<0.001). ③Children with MPP of phlegm-heat blocking lung syndrome had lower fecal sIgA content than healthy children of the normal group (P<0.05); after treatment, the fecal sIgA content in the observation group increased (P<0.001) and was higher than that of the control group (P<0.001). ④The number of Bifidobacterium, Lactobacillus, Ruminococcus flavefaciens and Clostridium butyricum in feces of children with MPP of phlegm-heat blocking lung syndrome was positively correlated with the sIgA content (P<0.001). ⑤During the course of treatment, no significant adverse reaction was observed in patients under treatment.ConclusionsSatisfactory results have been obtained among patients with MPP of the phlegm-heat blocking lung syndrome treated by modified XBCQ with conventional therapy. The mechanism is related to the increase of beneficial intestinal microflora and intestinal mucosal immunity regulation.

摘要:ObjectiveTo observe the changes of urodynamics and contractility of isolated detrusor strips at different time points after electroacupuncture (EA) in rats with detrusor overactivity (DO), and provide a theoretical guidance and basis for the temporal dynamics in the effectiveness of clinical acupuncture treatment.MethodsOne hundred and sixty Wistar female rats were randomly divided into control, model, EA and drug groups, each with 40 rats, and each group was further randomly divided into 4 subgroups (corresponding to different observation time points: 2 h, 24 h, 48 h and 72 h after the intervention), 10 rats in each subgroup. Except for the control group, all rats were prepared into DO model by intraperitoneal injection of cyclophosphamide. On the basis of the model, the EA group was given EA at Ciliao (BL32) and Huiyang (BL35) points, and the drug group was administered with imatinib mesylate by gavage. Rats in the control, model and drug groups were bound and fixed for 20 min as those in the EA group, and rats in the control, model and EA groups were administered with drinking water of the same body mass by gavage as those in the drug group, once a day for 3 consecutive days. The effective bladder capacity, urinary storage time and voiding pressure of the rats were analyzed at each observation time point by urodynamic detection technique. Changes in contractility of the isolated detrusor strips of the bladder were observed in each group using the isolated detrusor traction test.ResultsA total of 124 rats were finally included in the statistical analysis. After modeling, the effective bladder capacity of rats was significantly smaller and urinary storage time was significantly shorter in the model group than those of rats in the control group (P<0.05). At the time points of 2 h, 24 h, 48 h and 72 h after intervention, the effective bladder capacity of rats was significantly larger and urinary storage time of rats was significantly longer in the EA group and the drug group than those of rats in the model group (P<0.05), but the differences between the EA group and the drug group were not statistically significant (P>0.05). At 24 h after intervention, the effective bladder capacity of rats was larger and urinary storage time of rats was longer in the EA group than those of rats observed at other three time points (P>0.05). The contraction of isolated detrusor strips of rats in the model group had lower amplitude than that in the control group, while the contraction frequency was higher than that in the control group (P<0.05). Compared with the contraction amplitude of detrusor strips of rats in the model group, the contraction amplitude in the EA group increased significantly at 24 h after intervention (P<0.05), and the increase continued until 72 h after intervention, but there was no increase in the contraction amplitude in the drug group. Both EA and drug groups could reduce the contraction frequency of bladder detrusor strips in DO model rats, and the effect was optimal at 24 h after intervention (P<0.05).ConclusionElectroacupuncture can effectively increase the effective bladder capacity and urinary storage time of DO rats, increase the contraction amplitude of isolated detrusor strips and stabilize the contraction frequency of detrusor strips. The effect of electroacupuncture lasts at least for 72 h after acupuncture, and the effect is optimal at 24 h after intervention.

摘要:ObjectiveTo explore the effect and mechanism of Bushen Huanjing Recipe on sarcopenia mice.MethodsThe aged senescence accelerated mouse prone 8 (SAMP8) was used as a sarcopenia model, and 36 SAMP8 mice were randomly divided into 0.9%NaCl solution group (control group), Bushen Huanjing Recipe group (Bushen group) and ferroptosis inhibitor Ferrostatin-1 (Ferr-1) intraperitoneal injection group (Ferr-1 group). Gavage intervention was given in the control and the Bushen groups for 12 weeks, and inhibitor injection intervention was given for 3 weeks. After the intervention, the muscle mass of the mice was weighed. The muscle strength of the mice was detected by a muscle functional test system, and the maximum grip strength of the mice was detected by a grip strength meter. The microstructure of the mice muscle fibers was detected by the laminin immunofluorescence staining. The contents of reduced nicotinamide adenine dinucleotide phosphate (NAPDH), glutathione (GSH), malondialdehyde (MDA) and 4-hydroxynonenal (4-HNE) by immunohistochemistry staining were used to detect lipid oxidation in muscles of the mice. The expression of ferroptosis related proteins such as anti-glutathione peroxidase 4 (Gpx4), SCL7A11, P53 and P21 in mice muscles was detected by Western blot, and the morphology of mitochondria of muscle fiber in the mice were detected by a transmission electron microscope.ResultsCompared with the control group, Bushen Huanjing Recipe and Ferr-1 intervention increased the muscle mass, strength and grip strength as well as the area of muscle fibers in the mice, improved the morphology of mitochondria in muscle fibers(P<0.01), and reduced lipid peroxidation. The expression of antioxidant glutathione peroxidase (Gpx4) and SLC7A11, the key element of cystine / glutamate antiporter, was significantly increased (P< 0.001), while the expression of cell senescence-related protein p53 and its downstream protein P21, which negatively regulated SLC7A11, was significantly decreased (P<0.001).ConclusionThe Bushen Huanjing Recipe can effectively treat sarcopenia in mice by regulating ferroptosis.

摘要:ObjectiveTo establish UPLC fingerprint and content determination methods of Codonopsis Radix dispensing granules (CRDG) and honey bran frying Codonopsis Radix dispensing granules (HBFCRDG),and to provide a comprehensive evaluation of the quality of CRDG and HBFCRDG.MethodsUPLC analysis was performed on a Waters ACQUITY UPLC HSS T3 column (2.1 mm×100 mm,1.8 μm),with a mobile phase consisting of acetonitrile (A) - 0.1% formic acid (B) at the flow rate of 0.3 mL·min-1. The column temperature was controlled at 30 ℃ and the detection wavelength was set at 268 nm. The injection volume was 3 μL.ResultsUPLC fingerprint and content determination methods of HBFCRDG were established. Lobetyolin displayed ideal linearity within the range of 2.4-100.0 mg·L-1 (R2=0.999 7). The recoveries of lobetyolin in three levels of 50%,100% and 150% were 97.72%, 96.23%, 98.60% respectively, and the total average recovery was 97.52% with RSD of 1.76%. The RSD values of precision, repeatability and stability were all less than 3%. Nine common peaks were designated in fingerprint and peaks of lobetyolin, tangshenoside Ⅰ,lobetyolinin and lobetyol were confirmed by standard references. The content of lobetyolin in 28 batches of CRDG and HBFCRDG was between 0.06-0.39 mg·g-1. According to the results of PCA and PLS-DA,HBFCRDG can be distinguished from CRDG obviously. What’s more,it showed that there were differences in samples of CRDG from different manufacturers.ConclusionThe method has the advantages of simple operation, high detection efficiency and strong specificity, and can be used for the quality evaluation of CRDG and HBFCRDG.

摘要:ObjectiveTo explore the effect of Xifeng Zhidong Decoction on inflammatory reaction of mouse microglia (BV-2 cells) induced by lipopolysaccharide (LPS).MethodsThe microglia of mice were divided into three groups: control group, LPS group and Xifeng Zhidong Decoction group. The DMEM medium was added to the control group. LPS was used to induce inflammation on microglia at a concentration of 0.1 mg·L-1 in the LPS group, and intervention was performed by Xifeng Zhidong Decoction with a concentration of 12.5, 25, 50, 100, 200 and 400 mg·L-1 respectively in the Xifeng Zhidong Decoction group. The effect of Xifeng Zhidong Decoction on the viability of microglia was observed by cell proliferative toxicity test (CCK-8). The level of nitric oxide (NO) in the supernatant was detected by enzyme linked immunosorbent assay (ELISA), and the expression of inflammatory factors in microglia was detected by fluorescence quantitative polymerase chain reaction (real-time PCR). Western blot and immunofluorescence staining were used to detect the expression of nitric oxide-induced synthase (iNOS) and arginine-1 (Arg-1) proteins.ResultsThere was no significant effect on the activity of microglia in different doses of Xifeng Zhidong group or LPS group (P>0.05). Compared with the control group, the NO content in microglia supernatant of the LPS group increased significantly(P<0.001); the mRNA expression of TNF-α, IL-6, IL-1β of M1 type microglia increased significantly(P<0.001), but the mRNA expression of TGF-β of M2 type microglia decreased (P<0.05) ; the protein expression of M1 marker iNOS increased (P<0.01), but the expression of M2 marker Arg-1 protein decreased (P<0.05). Compared with the LPS group, Xifeng Zhidong Decoction at 100, 200, and 400 mg·L-1 could significantly inhibit the release of NO(P<0.001), decrease the expression of TNF-α, IL-6 mRNA of M1 type microglia(P<0.001, P<0.05). Xifeng Zhidong Decoction increased the expression of IL-10 and TGF-β mRNA of M2 type microglia at 400 mg·L-1(P<0.05), reduced the expression of M1 type marker iNOS protein at 200 mg·L-1 (P<0.01), and increased the expression of M2 type marker Arg-1 protein at 100 mg·L-1 (P<0.05).ConclusionXifeng Zhidong Decoction can inhibit the transformation of microglia to M1 damaged type induced by LPS, and promote the transformation to M2 repair type.
