      摘要:This article introduced professor Chen Xiangjun’s experience in the treatment of systemic lupus erythematosus and its complications. It was concluded that the pathogenesis of systemic lupus erythematosus was based on deficiency of liver and kidney yin and marked by hyperactivity of evil heat. After a long illness, yang qi was exhausted and both yin and yang were weakened, which caused the changes in the disease. In the treatment of systemic lupus erythematosus and its complications, professor Chen made good use of traditional Chinese medicines such as Astragali Radix, Rehmanniae Radix, Polygonati Rhizoma, etc.. The treatment was based on the therapy of strengthening body resistance, then balancing yin and yang and giving consideration to the varying syndromes. A proven case was presented.

      摘要:ObjectiveThe GRADE system was revised to retain its scientific nature, reflect the characteristics of traditional Chinese medicine (TCM) and improve its practicality.MethodsThe initial revised entries of the modified Delphi expert questionnaire were formulated on the basis of literature research, and two rounds of expert surveys were conducted. An expert consensus meeting was held to form an evaluation tool.ResultsA total of 27 experts participated in the two rounds of questionnaire surveys on the initial revised entries and provided expert opinions, and 5 experts participated in the expert consensus meeting, who facilitated the formation of Evidence⁃Evaluation Tool of Clinical Evidence of Traditional Chinese Medicine Clinical Randomized Controlled Trials Based on GRADE System. The evaluation tool simplifies the GRADE system and adapts to the TCM clinical application, and it contains 9 revised items involving high-quality evidence sources, evidence downgrading factors and upgrading factors.ConclusionAfter revision, the GRADE system can better embody the theoretical and clinical characteristics of TCM and becomes more adaptable so that it can improve the application quality of GRADE system in the evaluation of TCM clinical evidence.

      摘要:ObjectiveTo investigate and compare the surface electromyography (sEMG) characteristics of the main force-generating muscles of the upper limbs of the operator conducting two different types of Shi’s cervical rotatory manipulation (cervical rotation manipulation and cervical rotation-traction manipulation).MethodsAn operator proficient in Shi’s cervical rotatory manipulation was selected to perform 2 different types of cervical rotatory manipulation, twice for each type, on 20 subjects in the same anatomical position. The sEMG signal data of the brachioradialis muscle, biceps brachii muscle, triceps brachii muscle, deltoid muscle, supraspinatus muscle and pectoralis major muscle on the right side of the operator were collected, and the sEMG characteristics of the upper limb force-generating muscles of the operator conducting cervical rotation manipulation and cervical rotation-traction manipulation were analyzed and summarized.Results①During the operation of cervical rotation manipulation, biceps brachii was the first to be activated, followed by deltoid, supraspinatus, triceps brachii, brachioradialis and pectoralis major, and the sequence of muscle discharge duration was consistent with the activation sequence. The muscle with the largest contribution rate was biceps brachii, and the muscle with the smallest contribution rate was pectoralis major. ②During the operation of cervical rotation-traction manipulation, brachioradialis muscle was the first to be activated, followed by triceps brachii, biceps brachii, deltoid, supraspinatus and pectoralis major. The sequence of muscle discharge duration was brachioradialis, biceps, triceps, deltoid, supraspinatus, and pectoralis major. Brachioradialis muscle had the longest duration of muscle discharge, biceps brachii and triceps brachii had almost equal duration, and pectoralis major muscle had the shortest duration. The brachioradialis muscle had the largest contribution rate, and the pectoralis major muscle had the smallest contribution rate. ③Compared with the values under the operation of cervical rotation-traction manipulation, the values of integrated EMG (iEMG) and (root mean square) RMS of brachioradialis muscle were lower (P<0.01), the values of iEMG of biceps brachii, deltoid, supraspinatus, pectoralis major and triceps brachii were higher (P<0.05, P<0.01), and the values of RMS of biceps brachii, deltoid, pectoralis major and triceps brachii were higher (P<0.05, P<0.01) under the operation of cervical rotation manipulation.ConclusionThe biceps brachii is the main muscle for the practice of cervical rotation manipulation, and the brachioradialis is the main muscle for the practice of cervical rotation-traction manipulation. Therefore, the muscle training for brachioradialis and biceps brachii can provide certain favorable conditions for manipulation.

      摘要:This paper introduced the experience of professor Shen Pi’an in treating primary Sjogren’s syndrome. Professor Shen considered that it was debatable to attribute primary Sjogren’s syndrome to the category of “dryness arthralgia” in traditional Chinese medicine, he considered that the disease was caused by wind, cold, dampness, heat, phlegm, blood stasis, toxin and kidney deficiency, which were “7+1” factors. The main treatment was clearing heat and detoxification, nourishing yin and promoting fluid, and the treatments of syndromes were considered simultaneously. One proved case was presented.

      摘要:Professor Qu Huanru’s experience in the treatment of inflammatory arthropathies based on “Shaoyang managing bone” was introduced. Professor Qu clinically found that the etiology and pathogenesis of some inflammatory arthropathies were caused by Shaoyang cardinal disadvantage, so he started research from the theoretical origin of “Shaoyang managing bone” and extended the connotation reasonably. During the clinical practice, he might promote the regulation of qi, or reduce the fire to relieve depression, or dredge the triple energizer, and then the purpose of dredging the whole body qi, reconciling qi, blood and fluid, relaxing the meridian, dredging the collaterals and relieving pain could be achieved. A proven case was presented.

      摘要:Clinical observation has shown that the cause of back pain in many patients with lumbar disc herniation does not lie in pathological changes such as herniated nucleus pulposus or disc degeneration, but in abnormalities of the transverse abdominal muscles. Under the guidance of modern anatomy and auxiliary examinations, we have adopted the relaxing needling technique (Huici) to treat lumbar disc herniation based on the theory of abdominal meridian tendons, and we have achieved good results. A proven case is provided.

      摘要:The purpose of this article is to explore the treatment of scapulohumeral periarthritis based on the theory of Shaoyang. The circulation path of Shaoyang meridians is closely related to the sites of scapulohumeral periarthritis. The proper physiological function of Shaoyang can harmonize the circulation of qi and blood and movement of tendons and bones. However, the abnormal function of Shaoyang may cause qi stagnation and blood stasis, interior generation of phlegm and turbidity, resulting in the coagulation and obstruction of tendons and bones. In terms of treatment, first, apply acupuncture, small needle knife and tuina manipulations to the selected acupoints related to Shaoyang meridians based on the circulation path of Shaoyang meridians to relieve symptoms; Second, regulate the qi movement of Shaoyang to smoothen the circulation of qi and blood and nourish the tendons and bones as Shaoyang governs the bones and functions as the pivot; Third, treat injury of tendons and muscles from Shaoyang visceral organs and use associated prescriptions of Chaihu classic formulas to harmonize visceral organs and achieve a holistic treatment as the liver and the gallbladder are exteriorly and interiorly related.

      摘要:Based on syndrome differentiation of six channels theory,the paper discusses the syndrome differentiation and treatment of hematogenous sepsis. It is considered that the deficiency of stomach qi is the root cause of the disease with the first attack of the pathogenic factors on Taiyang meridian and then spreading into the interior rapidly. In the acute stage,Sanyang disease is the main condition. The main pathogenesis of the disease is Shaoyang Yangming syndrome complex and interior yangming disease. In order to prevent the disease from spreading into yin, timely clearing away pathogenic heat is focused on to cut off the development of the disease treated with the prescriptions such as Danggui Chaihu Decoction, Da Chaihu Decoction, Qianjin Baitong Zhuangre fang, Baihu Decoction, which are prescribed according to syndrome differentiation. And two proven cases are presented.

      摘要:Based on the current status of Bupleuri Radix in modern clinical prescription application,through textual research on the records of Bupleuri Radix in the materia medica of the past dynasties,this paper analyzed the ancient and modern Bupleuri Radix use, source, place of origin, efficacy, meridian, etc.. By further combining modern pharmacology and high performance liquid chromatography analysis, it was found Bupleurum chinense was more suitable for soothing liver and relieving depression,and Bupleurum scorzonerifolium was more suitable for relieving heat.It could be preliminarily inferred that the Bupleuri Radix used in Treatise on Febrile Diseases was Bupleurum chinense.

      摘要:Comparing the processing methods of Chinese medicinal materials between Menghe medical school and Guide on Preparation of Drugs, this paper discussed the similarities and differences between the two in processing auxiliary materials, processing technology, etc. There was a certain overlap between the two in processing auxiliary materials and technology. The processing methods included in Guide on Preparation of Drugs were relatively simple. The book paid more attention to the quality control of the whole process from material selection to clinical use. While Menghe medical school was better at using medicines processing and co-replication, and focused on expanding the types of processed medicine to cope with complex clinical needs.

      摘要:ObjectiveTo observe the clinical efficacy of Shi-style spine balance manipulation (SBM) combined with muscle and bone guidance (MBG) in the treatment of lumbar disc herniation (LDH) of the middle-aged and elderly patients.MethodsTotally 144 middle-aged and elderly patients with lumbar disc herniation from four sub-centers were randomly divided into a treatment group (n=72) and a control group (n=72). The treatment group was given Shi-style spine balance manipulation combined with muscle and bone guidance, and the control group was given pelvic mechanical traction combined with rehabilitation training of lumbar and back muscles. The treatment of Shi-style spine balance manipulation and pelvic traction lasted 4 weeks, while the muscle and bone guidance as well as the rehabilitation training of lumbar and back muscles lasted 6 months. The visual analogue scale (VAS) scores, Japanese Orthopaedic Association (JOA) scores, Oswestry Disability Index (ODI), and the changes of spinal-pelvic sagittal parameters were compared before treatment and at 2, 4, 12, 24 weeks of treatment, respectively.Results①During the trial, one case was excluded from the test group and two cases were excluded from the control group. Finally, 71 cases completed the trial in the treatment group and 70 cases in the control group. ②The total effective rate was 91.55% in the treatment group and 88.57% in the control group. The clinical efficacy of the treatment group was better than that of the control group (P<0.05). ③With the progress of treatment, the VAS scores gradually decreased in both groups (F=307.785,P<0.001), and the VAS scores of patients in the treatment group were significantly lower than those in the control group at 2, 4, 12, 24 weeks of treatment (P<0.05). ④With the progress of treatment, the JOA scores gradually increased in both groups (F=316.774,P<0.001), and the JOA scores in the treatment group were higher than those in the control group at 3 and 6 months of treatment (P<0.05). ⑤With the progress of treatment, the ODI values of the two groups gradually decreased (F=1328.008,P<0.001), and the ODI values in the treatment group were lower than those in the control group at 2 and 4 weeks of treatment (P<0.05). ⑥After treatment, there was no significant difference in the spinal-pelvic sagittal parameters in both groups (P>0.05), and the difference in the spinal-pelvic sagittal parameters between the two groups was not statistically significant (P>0.05).ConclusionShi-style spine balance manipulation combined with muscle and bone guidance has achieved a satisfactory result in the treatment of lumbar disc herniation in the middle-aged and elderly people, which can significantly relieve pain, enhance somatic function and improve patients’ quality of life. Its clinical efficacy is not inferior to that of pelvic mechanical traction combined with rehabilitation training of lumbar and back muscles.

      摘要:ObjectiveTo explore the clinical efficacy of Bawei Shenqi Pill in the treatment of ankylosing spondylitis with deficiency of kidney yang.MethodsUsing a randomized, double-blind, placebo-controlled clinical trial, 80 patients of ankylosing spondylitis with deficiency of kidney yang were randomly divided into the treatment and the control group, 40 in each group. The treatment group was administered with the Bawei Shenqi Pill and meloxicam,while the control group was given the Bawei Shenqi Pill placebo and meloxicam, with a course of 12 weeks. TCM curative effect and TCM syndrome score, spinal pain score, spinal mobility index(occiput to wall distance, chest expansion, finger to floor distance and Schober test), bath ankylosing spondylitis disease activity index(BASDAI), bath ankylosing spondylitis function index(BASFI), C reactive protein(CRP), erythrocyte sedimentation rate (ESR) and scores of quality of life assessment scale were observed after the treatment.Results①There were 74 cases of this experiment completed, including 38 cases in the treatment group and 36 cases in the control group. ② The total effective rate of TCM in the treatment group and the control group were 92.1% and 36.1% respectively, and the two groups were statistically significant in clinical efficacy (P<0.05). ③ After treatment, the TCM syndrome scores of the two groups were improved (P<0.05); the difference scores of low back pain, low back stiffness, limited activity, mental fatigue, pale complexion and low back and knee soreness in the treatment group were better than those in the control group (P<0.05).④ After treatment, the spinal pain score was improved in both groups (P<0.05); the difference of the spinal pain score in the treatment group was significantly better than that in the control group (P<0.05).⑤ After treatment ,The spinal mobility index was improved in both groups (P<0.05); the difference scores of chest expansion and Schober test in the treatment group were better than those in the control group (P<0.05). ⑥ After treatment, BASDAI score and BASFI score in both groups improved (P<0.05); the difference of BASDAI score and BASFI score in the treatment group were better than those in the control group (P<0.05).⑦ After treatment, CRP in the treatment group was improved (P<0.05), but there was no significant change in the control group (P>0.05); there was no significant difference between the two groups (P>0.05). After treatment, there was no significant difference in ESR between the two groups (P>0.05). ⑧After treatment, the physiological function(PF), role-physical (RP), physical pain(BP), general health(GH), energy(VT), social function(SF) in both groups and mental health(MH) in the treatment group were improved (P<0.05); The difference scores of GH, VT and MH in the treatment group was significantly better than those in the control group (P<0.05).ConclusionBawei Shenqi Pill can significantly improve the clinical efficacy of TCM in patients with ankylosing spondylitis of kidney yang deficiency type, improve joint inflammation and joint function to a certain extent, and improve the quality of life.

      摘要:ObjectiveTo evaluate the efficacy and safety of Bushen Qiangdu Qingre Granules in the treatment of kidney deficiency and damp-heat syndrome ankylosing spondylitis (AS) patients.MethodsA randomized double-blind and placebo-controlled clinical trial was carried out in 70 AS patients with kidney deficiency and damp-heat syndrome, who were randomly divided into the treatment group and the control group, with 35 cases in each group. Both groups received NSAIDs. The treatment group was added with Bushen Qiangdu Qingre Granule, and the control group was added with simulated Chinese medicine granule. The course of treatment in both groups was 12 weeks. The spinal pain scores, patient overall assessment (PGA), Bath AS disease activity index (BASDAI), Bath AS function index(BASFI), AS disease activity score (ASDAS-CRP)and TCM syndrome scores between the two groups were compared at 4 and 12 weeks, respectively, to evaluate the clinical efficacy and safety.Results①A total of 65 patients completed the clinical trial, including 32 cases in the treatment group and 33 cases in the control group. ②After 4 weeks treatment, the proportion of patients who met the ASAS20 and BASDAI 50 standards in the treatment group was higher than that in the control group (P<0.05). After 12 weeks treatment, the proportion of patients who met the ASAS20,ASAS40, ASAS5/6 and BASDAI 50 standards in the treatment group was higher than that in the control group (P<0. 05). ③After 12 weeks treatment, the clinical improvement rate of ASDAS-CRP in the treatment group was higher than that in the control group (P<0.05). ④After 4 and 12 weeks of treatment, the efficacy for TCM syndromes of the treatment group was better than that of the control group (P<0.05). ⑤Compared with before treatment, the spinal pain score, PGA, BASDAI in two groups both decreased after 4 weeks of treatment(P<0.05) , and BASFI, ASDAS-CRP, TCM syndrome score in the treatment group decreased (P<0.05) ;The comparison between groups showed that BASDAI and TCM syndrome score in the treatment group were lower than those in the control group (P<0.05). After 12 weeks of treatment, compared with before treatment, the spinal pain score, PGA, BASDAI, ASDAS-CRP, TCM syndrome score in two groups decreased (P<0.05), and BASFI in the treatment group decreased (P<0.05); The comparison between groups showed that the spinal pain score, PGA, BASDAI, ASDAS-CRP and TCM syndrome score in the treatment group were lower than those in the control group (P<0.05). ⑥No serious adverse reaction occurred in the two groups during the treatment.ConclusionBushen Qiangdu Qingre Granule has good clinical efficacy in the treatment of AS with kidney deficiency and damp-heat syndrome, it can effectively reduce disease activities, improve kidney deficiency and damp-heat syndrome and clinical indicators, with safety.

      摘要:ObjectiveTo observe the clinical effect of acupuncture combined with tendon-regulating manipulation on residual symptoms and functional rehabilitation of elderly patients after posterior lumbar fusion (PLF).MethodsSixty-eight elderly patients after PLF were randomly divided into a treatment group (n=34) and a control group (n=34). In the control group, functional exercise was performed under the guidance of physicians, while in the treatment group acupuncture combined with tendon-regulating manipulation was conducted besides functional exercise under the guidance of physicians. The course of treatment in each group lasted 3 months. The fusion of bone graft was observed, and the changes of Visual Analogue Scale (VAS) score, Japanese Orthopaedic Association (JOA) low back pain score and American 12-Item Short Form Survey (SF-12) score were compared.Results①Finally 32 cases in the treatment group and 30 cases in the control group completed the trial. ②According to the intra-group comparisons before operation, 2 weeks after operation and 3 months after operation, the VAS score, JOA score and SF-12 score were significantly different in both groups (P<0.05). According to the inter-group comparisons, the improvement of VAS score, JOA score and SF-12 score in the treatment group was better than that in the control group at 2 weeks and 3 months after operation (P<0.05). ③There was no significant difference in the intervertebral fusion rate between the two groups at 3 months and 6 months after operation (P>0.05).ConclusionCompared with the effect of functional exercise, acupuncture combined with tendon-regulating manipulation takes a positive role in promoting the functional rehabilitation after PLF, eliminating postoperative residual symptoms such as pain of the lower back and legs, and improving the quality of life of elderly patients.

      摘要:ObjectiveTo observe the effects of different doses of Zhige Tongmai Granules (ZGTMG) on the expression of protein and mRNA of tumor necrosis factor-α (TNF-α), matrix metalloproteinase-9 (MMP-9), vascular cell adhesion molecule-1 (VCAM-1), intercellular adhesion molecule-1 (ICAM-1) and tissue inhibitor of matrix metalloproteinase (TIMP) in plaque tissues of diabetic atherosclerosis (AS) rats.MethodsTen of 70 SPF-grade Wistar male rats at 4 weeks of age were randomly selected as the normal group and administered with basic feeds only, and the remaining 60 were the diabetic modeling group. The diabetic atherosclerotic rat model was designed by streptozotocin (STZ) injection (30 mg/kg), intra-aortic balloon pump injuries and the high-fat diet. The modeled rats were randomly divided into a model group, ZGTMG low-dose group (low-dose group, gavage administration of ZGTMG at 1.05 g·kg-1·d-1), ZGTMG medium-dose group (medium-dose group, gavage administration of ZGTMG at 2.1 g·kg-1·d-1), ZGTMG high-dose group (high-dose group, gavage administration of ZGTMG at 4.2 g·kg-1·d-1) and rosuvastatin sodium group (modern medicine group, gavage administration of rosuvastatin sodium at 1.05 mg·kg-1·d-1) according to the blood glucose level, with 10 animals in each group. After 6 weeks of administration, atheromatous plaques were obtained from the abdominal aorta as the specimen and the protein and mRNA expression levels of TNF-α, MMP-9, VCAM-1, ICAM-1 and TIMP were detected.ResultsThe protein and mRNA expression levels of TNF-α, MMP-9, VCAM-1, ICAM-1 and TIMP were lowered significantly in the high-dose group and modern medicine group compared with those in the model group, and the differences were statistically significant (P<0.05). The TNF-α mRNA expression, and the protein and mRNA expression levels of MMP-9, VCAM-1, ICAM-1 and TIMP were lowered in the low-dose and medium-dose groups compared with those in the model group, and the differences were statistically significant (P<0.05). Significantly lowered protein expression levels of TNF-α, MMP-9, VCAM-1, ICAM-1 and TIMP as well as the lowered TIMP mRNA expression were detected in the high-dose group, and compared with the protein and mRNA expression levels in the low dose group the differences were statistically significant (P<0.05). The protein expression levels of VCAM-1 and TIMP were significantly lowered in the medium-dose group compared with those in the low-dose group, and the differences were statistically significant (P<0.05). The expression levels of MMP-9, ICAM-1 and TIMP were significantly lowered in the high-dose group compared with those in the medium-dose group, and the differences were statistically significant (P<0.05).ConclusionZGTMG can significantly lower the protein and mRNA expression levels of TNF-α, MMP-9, VCAM-1, ICAM-1 and TIMP in the plaque tissues of diabetic AS rats, decrease the expression of inflammatory factors and improve the inflammatory response of vascular endothelium, and the clinical effect of the high-dose group is better than that of the medium-dose and low-dose groups.

      摘要:ObjectiveTo observe the effect of Guanxinkang (GXK) on the expression of proteins related to MAPKs/NF-κB signal pathway in the liver tissue of LDLR-/- mice.MethodsTwenty-four male LDLR-/- mice were randomly divided into model group, GXK low, medium and high dose groups, and C57BL/6J mice were used as the control group. Except for the control group, each group was given high-fat feed for 12 weeks. The control group and the model group were given 200 μL of 0.9% NaCl solution daily. The GXK low, medium and high dose groups were given 7.24 g/kg, 14.48 g/kg, 28.96 g/kg of Chinese medicine decoction, 200 μL daily for 12 weeks. The serum levels of TC, TG, ALT and AST were detected by biochemical assay; the plaque area was observed by oil red O staining; the liver tissue structure was observed by hematoxylin-eosin (HE) staining; the expressions of Erk1/2, P38, JNK, IκB, NF-κB proteins were determined by Western blot; the mRNA and contents of IL-1, IL-6 and TNF-α were detected by RT-PCR and ELISA respectively; and the expression of TNF-α was observed by immunofluorescence.ResultsCompared with the model group, the levels of serum TC, TG, ALT and AST were decreased in each dose group of GXK (P<0.05); the area of aortic plaques was reduced and pathological changes in the liver tissue were improved. And the phosphorylation level of Erk1/2, P38, JNK, IκB, and NF-κB protein in liver tissue was decreased in GXK groups (P<0.05). The expressions of IL-1β, IL-6, TNF-α mRNA in the liver tissue were down-regulated in GXK groups (P<0.05). The contents of IL-1β, IL-6 and TNF-α in serum were decreased (P<0.05) and the expression of TNF- α in liver tissue was reduced.ConclusionGXK can reduce liver inflammation and improve atherosclerosis in LDLR-/- mice by inhibiting MAPKs/ NF-κB signal pathway.

      摘要:ObjectiveTo explore the effect and mechanism of depression on atherosclerosis(AS), and explore the effect and possible mechanism of saikosaponin a (SSa) on AS in mice with AS complicated with depression.Methods①Thirty male Apoe-/- mice were randomly divided into AS group (n=6) and AS complicated with depression group (n=24), and the AS group was administered with high-fat diet for 8 weeks and chronic unpredictable mild stress (CUMS) to establish the AS with depression mouse model. After the model was established successfully, the body mass, blood lipid (TC, LDL-C) level, serum CRP level were compared and aortic atherosclerosis were observed by HE staining and compared between AS group and AS complicated with depression group. ②AS complicated with depression group (n=18) was randomly divided into a normal saline (SLYS) group (n=6), a fluoxetine (FXT) group (n=6) and a SSa group (n=6). After 4 weeks of intragastric administration of SSa, fluoxetine and 0.9% NaCl solution 5 mg·kg-1·d-1 in the corresponding groups, the body mass, serum levels of blood lipid (TC, LDL-C) and CRP were compared, aortic atherosclerosis was observed by HE staining, NLRP3 content in aortic plaque was observed by immunohistochemical staining, and animal behavior was evaluated by tail suspension test and forced swimming test.Results①The body mass and serum TC, LDL-C, CRP levels in AS complicated with depression group were higher than those in AS group (P<0.05); The degree of aortic atherosclerosis in AS complicated with depression group was significantly higher than that in AS group. ②After 4 weeks of intervention with corresponding drugs, the body mass and LDL-C level of AS and depression mice in SSa group were lower than those in FXT group (P<0.05); serum CRP level of AS and depression mice in SSa group was lower than that in SLYS group (P<0.05); the degree of aortic atherosclerosis of AS and depression mice in SSa group was milder than that in FXT Group and SLYS Group, and the expression of NLRP3 in AS plaque of AS and depression mice in SSa group was less than that of FXT group and SLYS group (P<0.05); the rest time of forced swimming and tail suspension in SSa group was shorter than that in SLYS group and FXT group (P<0.05).ConclusionsDepression can increase body mass and blood lipid level, enhance inflammatory reaction and aggravate AS. SSa can significantly improve depression, and may alleviate AS by reducing the inflammatory reaction of mice with AS and depression through lowering the expression of NLRP3.

      摘要:ObjectiveTo explore the preliminary mechanism of intervention with gold capsule on insulin resistance (IR) in rats with type 2 diabetes mellitus (T2DM) based on the PI3K/AKT pathway.MethodsForty SPF-grade Wistar male rats were selected, 10 of which were randomly chosen into a blank control group, and a T2DM model was established in the remaining 30 rats. Those successfully modeled rats were assigned to model, metformin and gold capsule groups randomly, with 10 rats in each group. The metformin group was administered with metformin hydrochloride (0.2 g/kg), the gold capsule group was administered with gold capsule (1.265 g/kg) and the model group and control group were administered with equal volume of 0.9% NaCl solution by gavage once a day for 10 consecutive weeks. The blood glucose levels [fasting plasma glucose (FPG), glycated hemoglobin (HbAlc)], PI3K/AKT signaling pathway protein expression levels [PI3K, AKT, glucose transporter protein 4 (GLUT-4)] and homeostatic model assessment for insulin resistance (HOMA-IR) level were tested in each group.ResultsCompared with the rats in the blank control group, the rats in the model group had higher levels of FPG, HbAlc, FINS and HOMA-IR, but lower expression levels of PI3K, AKT and GLUT-4, and the differences were statistically significant (P<0.05). Compared with the rats in the model group, the rats in the metformin and gold capsule groups showed lower levels of FPG, HbAlc, FINS and HOMA-IR, but higher expression levels of PI3K, AKT and GLUT-4, and the differences were statistically significant (P<0.05).ConclusionGold capsule can increase the expression of PI3K, AKT and GLUT-4 in T2DM rats, which may work by activating PI3K/AKT signaling pathway to reduce blood glucose levels and improve IR in rats.

      摘要:Lumbar intervertebral disc herniation is a common disease which occurs frequently in the orthopedic clinic. At present, the treatment of lumbar intervertebral disc herniation is mainly divided into surgical treatment and (non-surgical) conservative treatment. Long-term clinical practice and research have shown that the non-pharmacological conservative treatment has a better therapeutic effect and higher clinical application value for lumbar intervertebral disc herniation. The article reviewed the current research status and development trend of non-pharmacological conservative treatment options such as bed rest, acupuncture, Tuina, Daoyin, traction and extracorporeal shock wave, and provided references and ideas for improving the therapeutic effect of lumbar intervertebral disc herniation in the clinic.
