      摘要:Scapulohumeral periarthritis is a common disease in the department of orthopedics and traumatology. Traditional Chinese medicine has played an important role in its clinical diagnosis and treatment. Based on preliminary research experience and evidence-based medical material support, we conducted questionnaire surveys, went through multiple rounds of reasonings, argumentations and peer-review of national experts, and finally made the Clinical Guidelines for Diagnosis and Treatment of Scapulohumeral Periarthritis in Traditional Chinese Medicine Orthopedics and Traumatology: T/CACM 1179—2019 revised and published to further standardize the clinical diagnosis and treatment of scapulohumeral periarthritis and improve the clinical efficacy. The newly revised guidelines have comprehensively introduced the definition, main points of diagnosis, TCM syndrome differentiation, treatment principles, treatment methods, prevention and regulation of scapulohumeral periarthritis, which can provide reference for clinicians.

      摘要:The COVID-19 pandemic has caused serious social impact. However, under the guidance of the Chinese government and with the joint efforts of traditional Chinese and Western medicine, we have well controlled the domestic outbreak. In the process of pandemic prevention and control, the concept of “harmony” is embodied at macro level (the relationship between man and nature), meso level (the relationship between individual and society) and micro level (the relationship between body and mind). As an important philosophical foundation for the development of traditional Chinese medicine (TCM), the concept of “harmony” has played an important role in the TCM prevention and treatment of COVID-19. Therefore, we must pay special attention to the harmonious coexistence between man and nature in the process of social development. When major emergencies occur, we should not only pay attention to the systematicness and integrity of prevention and control measures, but also emphasize the “harmony in diversity” and organic coordination of prevention and control methods. Everyone and every country should have a big picture and a holistic view, and contribute to the construction of the community of human healthcare.

      摘要:ObjectiveTo evaluate the clinical efficacy of integrated traditional Chinese and western medicine in treating acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with phlegm-heat stagnating in the lung syndrome based on a Bayesian network meta-analysis.MethodsRandomized controlled trials (RCTs) of integrated traditional Chinese and western medicine in the treatment of AECOPD with phlegm-dampness stagnating in the lung syndrome published in PubMed, Web of Science, EMBASE and Cochrane Library, China national knowledge infrastructure (CNKI), WanFang, VIP, SinoMed (CBM) databases were searched from the establishment of each database up to August 2019. Data were extracted after the collected literatures were screened according to the inclusion criteria, and quality assessment and data analysis were conducted by Review Manager 5.3, Stata14.0 and R 3.5.1.ResultsA total of 19 RCTs were included in our study with a cumulative total of 1,486 cases, involving three Chinese herbal formulas (Qingjin Huatan Decoction, Weijing Decoction, and Sangbaipi Decoction). According to the results of network meta-analysis, statistically significant clinical effectiveness was observed in cases treated with Qingjin Huatan Decoction and conventional western medicine [RR=3.54, 95%CI(1.45, 8.22)], cases treated with Weijing Decoction and conventional western medicine [RR=4.34, 95%CI(2.16, 8.72)], and cases treated with Sangbaipi Decoction and conventional western medicine [RR=3.39, 95%CI(2.11, 5.67)] compared with that in cases treated with conventional western medicine alone. The clinical effectiveness and pulmonary function were used as evaluation indexes. The results showed that among the three traditional Chinese medicine formulas, Weijing Decoction had the best therapeutic effect on AECOPD.ConclusionCompared with conventional western medicine treatment alone, the integrated traditional Chinese and western medicine treatment has shown more satisfactory therapeutic results in treating AECOPD with phlegm-damp stagnating in the lung syndrome, and the combined use of Weijing Decoction and convention western medicine treatment may be the best treatment protocol. However, due to the low quality of the included studies, the exact conclusions still need to be verified by more high-quality clinical studies.

      摘要:ObjectiveTo investigate the related prognostic impact factors of children’s mycoplasmal pneumoniae pneumonia (MPP).MethodsThe clinical data of 250 children with MPP who were hospitalized and treated in the pediatric ward of traditional Chinese medicine in Longhua Hospital Affiliated to Shanghai university of traditional Chinese medicine from January 2017 to December 2017 were retrospectively analyzed. The clinical efficacy of all the children was evaluated, and the prognostic impact factors of MPP were discussed by Ordinal Logistic regression analysis.ResultsThere were 195 cured cases (78.0%), 43 markedly effective cases (17.2%), 12 effective cases (4.8%) and 0 ineffective case (0%) in 250 children with MPP after treatment. The results of Ordinal Logistic regression analysis showed that, the integrated traditional Chinese and western medicine combined with external treatment and the use of traditional Chinese medicine before admission were helpful to the prognosis of MPP, and the abnormity of C reactive protein (CRP), the increase of platelet (PLT) level and the combination of other pathogens were the risk factors affecting the prognosis of MPP.ConclusionThe integrated traditional Chinese and western medicine combined with external treatment and early traditional Chinese medicine intervention is conducive to the prognosis of children with MPP, while the combination of other pathogens, the abnormity of CRP and the increase of PLT level suggest that the prognosis of the children with MPP is poor.

      摘要:Pulmonary nodules (PN) are a common reason for patients to visit the respiratory outpatient department. The key cause of pulmonary nodules lies in the internal deficiency of vital qi and qi depression due to the abnormality of seven emotions, as well as the external invasion of smoke poisoning, smog and the six pathogenic factors. The internal organs are thereby affected and the lung collaterals are damaged, resulting in such pathological changes as pathogenic qi lingering due to vital qi deficiency, qi depression, phlegm obstruction and blood stasis, which may even transform into heat and/or toxicity. The treatment should be guided by the idea of combining disease identification and syndrome differentiation, with supporting the vital qi and dispelling the pathogenic qi as the treatment principle, tonifying qi and strengthening the spleen, regulating qi and unblocking collaterals, resolving phlegm and removing blood stasis, clearing heat and detoxifying as the treatment methods. Early intervention should be encouraged according to the disease-syndrome progressive pattern of lung nodules to avoid the progress or cancerization.

      摘要:This article introduces Prof. Li Ji’s experience in treating pharyngeal itching triggered cough of laryngeal origin based on the “Neimocou” theory. The “Neimocou” theory is a theoretical model proposed by Prof. Li Ji and his research team for the syndrome differentiation and treatment of mucosa-related diseases. Based on this theory, pharyngeal itching triggered cough of laryngeal origin can be divided into three common syndromes: pathogenic wind attack, throat depression and stagnation, and flaring-up of deficiency-fire. The corresponding treatment methods for each syndrome include: dispersing wind to check itching, relieving spasm to check coughing; removing stagnation to relieve depression, penetrating the membrane and interstices; nourishing yin to clear heat, enriching yin to subdue fire. Satisfactory treating effects have been obtained with 2 proven cases provided for verification.

      摘要:The purpose of this paper is to explore Professor Wang Yusan’s experience in the treatment of laryngeal diseases. It is considered that the etiology of the diseases is classified into two categories of wind-cold and kidney deficiency, treated with modified Guizhi Decoction or Fugui Bawei Pills, by the method of warming and dissipating or warming and tonifying. Professor Wang determines the prescription for laryngeal diseases often relying on pulse diagnosis, and advocates treating the symptoms of excess-fire by meridian differentiation, and has discussed the theory of “diphtheria taboo table”.

      摘要:This paper introduces Professor Li Fazhi’s clinical experience in using Chailing Decoction. It is considered that Chailing Decoction is suitable for syndromes of the imbalance between liver and spleen, liver depression and spleen deficiency and water⁃dampness retention. Based on the theory of treating different diseases with the same therapy, Chailing Decoction is often flexibly used in the treatment of internal diseases such as hypertension, acne, insomnia, vertigo, pleural effusion and so on.

The study was hoped to provide literature basis for the formation of suitable technologies which could be promoted.

      摘要:ObjectiveTo investigate the mechanism of Xiaoxuming Decoction on acute lung injury (ALI) induced by lipopolysaccharide (LPS) by down-regulating USP9X, and to explore possible action mechanism.MethodsThe medicated serum was prepared, and the type Ⅱ alveolar epithelial cells were pretreated with 10% Xiaoxuming Decoction medicated serum. ① Effect of Xiaoxuming Decoction on the inflammation and function of alveolar type Ⅱ epithelial cell induced by LPS: The experiment was divided into the blank group, the LPS induction group and the LPS+Xiaoxuming Decoction group. Inflammatory factors including TNF-α, IL-1β and IL-8 were detected by ELISA. QRT-PCR and Western blot were used to detect the expression of tight junction proteins including Occludin, ZO-1 and Claudin. ② Xiaoxuming Decoction improved the inflammation and function of ALI cell model through USP9X: The experiment was divided into the blank group, the LPS induction group and the LPS+Xiaoxuming Decoction group. The expression of USP9X was detected by PCR and Western blot. ③ The interaction between USP9X and NLRP3 protein: After USP9X overexpressed in human alveolar epithelial cells, Co-IP combined with mass spectrometry was used to detect the interaction between USP9X and NLRP family, and Western blot was used to confirm the relationship between NLRP3 and USP9X. ④Xiaoxuming Decoction alleviating the pyrolysis of human alveolar epithelial cells caused by LPS: The experiment was divided into the blank group, the LPS induction group and the LPS+Xiaoxuming Decoction group. NLRP3, IL-1 β, Pro-IL-1 β, Caspase-1 and Pro-Caspase-1 were detected by Western blot. NLRP3, IL-1β Caspase-1 and CCK-8 were detected by PCR.Results① Compared with the model group, Xiaoxuming Decoction significantly decreased the protein expression levels of inflammatory cytokines including TNF-α, IL-1β and IL-8 in type Ⅱ alveolar epithelial cells induced by LPS(P<0.05), and significantly increased the mRNA and protein expression levels of tight junction proteins including Occludin, ZO-1 and Claudin. ② Compared with the model group, the serum pretreatment of Xiaoxuming Decoction significantly reduced the transcription and protein expression of USP9X (P<0.05). ③ There was an interaction between NLRP3 and USP9X family detected by Co-IP combined with mass spectrometry and confirmed by Western blot. ④ Compared with the model group, the Xiaoxuming Decoction significantly reduced the protein and transcription levels of NLRP3, IL-1β, Pro-IL-1β, Caspase-1 and Pro-Caspase-1 in type Ⅱ alveolar epithelial cells induced by LPS, and alleviated the occurrence of pyrolysis (P<0.05).ConclusionXiaoxuming Decoction can effectively improve acute lung injury, which may be related to the down-regulation of USP9X to regulate the expression of NLRP3 ubiquitination.

      摘要:ObjectiveTo investigate the effects of Qinfei Oral Liquid (QFL) on airway inflammation and gut microbiota in asthmatic mice.MethodsFemale BALB/c mice were sensitized and challenged with ovalbumin (OVA) to establish a murine model of asthma. After successful modeling, QFL group was given QFL by gavage, the budesonide (BUD) group given budesonide suspension by nebulization, fecal microbiota transplantation (FMT) group received fecal bacteria transplantation from normal mice, OVA group and normal group given equal volume saline by gavage, and all the groups were treated for 5 consecutive days. Within 4 h after the last treatment, mice were sacrifice by excessive anesthesia, and lung tissues were harvested for HE staining to assess airway inflammation; Intestinal contents were taken for sequencing of the V3⁃V4 region of the 16S rDNA gene. Software such as QIIME2, LEfSe and PICRUSt 2 were used to analyze the effects of QFL intervention on the structure, abundance, diversity, and characteristics of intestinal flora in asthmatic mice, and the KEGG database was used to analyze and predict the functions of intestinal microbes.ResultsThe administration of QFL, budesonide and fecal microbiota transplantation all ameliorated airway inflammation in asthmatic mice to varying degrees. At the phylum level, the p_Firmicutes was decreased, while the p_Actinobacteria and TM7 were increased in the OVA group, and the abundance was restored to normal after treatment with QFL. At the genus level, the abundances of Lactobacillus, Prevotella, Odoribacter decreased and Desulfovibrio increased in the ova group, which could be improved after treatment with QFL. OVA group heterogeneity bacteria are mainly affiliated with f_Prevotellaceae of the p_Bacteroidetes and f_Clostridiaceae of the p_Firmicutes; Key bacteria in the QFL group were mainly affiliated with f_Burkholderiaceae and f_Pasteurellaceae of the p_Proteobacteria, and f_Micrococcaceae of the p_Actinobacteria. The gut flora metagenomic functional prediction of asthmatic mice showed that: biosynthesis of fatty acids, branched chain amino acid degradation, arginine and pyruvate metabolism as well as pyruvate metabolism and thiamine metabolism pathway related gene functions were down⁃regulated, while selenium compound metabolism pathway functions were up⁃regulated; Pathways such as galactose metabolism, steroid hormone biosynthesis and branched chain amino acid degradation were up⁃regulated after treatment with QFL, while the function of phenylalanine metabolic pathway was down⁃regulated.ConclusionThe modulation of intestinal flora composition and its related metabolic pathways in asthmatic mice may be one of the important mechanisms by which QFL ameliorates airway inflammation in asthmatic mice.

      摘要:ObjectiveTo explore the mechanism of Qingfei Granule regulating airway injury in COPD rats by interfering with TLR4/NF- κB signal pathway from the point of view of airway inflammation and airway barrier injury.MethodsIn 60 SD rats, 10 rats were randomly selected as the normal group, and the other 50 rats were exposed to cigarette smoke combined with endotracheal injection of LPS to establish COPD model. After successful establishment of the model, 50 rats were randomly divided into the model group, dexamethasone group (dexamethasone 0.2 mg/kg), Qingfei Granule low, medium and high dose groups (Qingfei Granule 5.4 g/kg, 10.8 g/kg, 21.4 g/kg, respectively), with 10 rats in each group and were respectively given the corresponding drugs by intragastric administration for 8 weeks. The changes of pulmonary function and ultrastructure of lung were observed under transmission electron microscope. The contents of cytokines (including TGF- β1, TNF- α, IL-1β, IL-6, IL-10, VEGF, CTGF and ET-1) in serum and bronchoalveolar lavage fluid (BALF) were measured by ELISA. The expressions of TLR4, NF⁃κB, Bcl⁃2 and Bax mRNA in lung tissue were measured by qRT-PCR. The protein expressions of TLR4, NF-κB, Bcl-2 and Bax in lung tissue were measured by Western blot.Results①Compared with the model group, the pulmonary ventilation function of different Qingfei Granule groups was improved, showing that the level of FEV0.2/FVC was increased (P<0.05), while the levels of RL and Re decreased in different degrees. The improvement degree of FEV0.2 and FEV0.2/FVC in high dose group of Qingfei Granule was similar to that in dexamethasone group(P>0.05).②Through the observation under transmission electron microscope, compared with the model group, the cytoplasmic texture of type Ⅱ alveolar epithelial cells in the high dose group of Qingfei Granule was uniform. There was no obvious destruction of nuclear membrane. More mitochondria and lamellar bodies could be seen, and most of the cells were normal. ③Compared with the model group, Qingfei Granule could reduce the levels of TNF-α and IL-8 in serum and BALF fluid of rats(P<0.05), and the medium and the high dose groups had a certain dose-dependent relationship. ④Compared with the model group, Qingfei Granule could increased the expression of miRNA⁃146a mRNA, reduce the expression of TLR4, NF⁃κB, Bax mRNA and its protein, increased the expression of Bcl⁃2 mRNA and its protein in lung tissue of rats(P<0.05).ConclusionQingfei Granule can repair the airway barrier injury by inhibiting the expression of mRNA and protein related to TLR4/NF-κB signal pathway and inhibiting the apoptosis of alveolar epithelial cells.

      摘要:ObjectiveTo optimize water extraction technology of Erchen Sangbei Zhike Granules.MethodsTaking extraction times, extraction time, soaking time and water addition ratio as factors, contents of ferulic acid and hesperidin in water extract and yield of extract as indexes, optimum range of every factors based on single factor test,using information entropy weighting method to determine index weight and calculate comprehensive score, optimum extraction process of Erchen Sangbei Zhike Granules was optimized by response surface methodology with comprehensive score as evaluation index.ResultsThe weight coefficients of ferulic acid content, hesperidin content and extract yield obtained by entropy weight method were 0.391 5, 0.337 3 and 0.271 2, respectively;the regression model P<0.000 1 obtained by response surface design had good predictability.ConclusionConsidering the convenience of actual production, the extraction process of Erchen Sangbei Zhike Granules is determined as adding 13 times water, soaking for 60 min, extracting for 120 min and extracting twice.

      摘要:ObjectiveTo evaluate the clinical efficacy of Dr. Yu’s integrated Chinese and Western medicine classification and treatment protocol for polycystic ovary syndrome (PCOS).MethodsA total of 130 patients with PCOS were randomly divided into a treatment group (n=65) and a control group (n=65). The treatment group was treated with the integrated Chinese and Western medicine classification and treatment protocol first proposed by Dr. Yu Jin (i.e., patients were first classified into 4 types: PCOS Ia, PCOS Ib, PCOS Ⅱa and PCOS Ⅱb, and then given Kuntai Prescription together with ethinyl estradiol (EE) or dexamethasone or metformin according to patients’ condition), while the control group was treated with the recommended western medicine based on the conventional treatment scheme. The duration of treatment was 6 months in both groups. The clinical efficacy, ovulation rate, pregnancy rate and medication safety were observed, and the reproductive endocrine-related indexes [follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), testosterone (T), dehydroepiandrosterone sulfate (DHEA-S), sex hormone-binding globulin (SHBG), cortisol (C), 17α-hydroxyprogesterone (17α-OHP)], glucose metabolism indexes [fasting plasma glucose (FPG), fasting insulin (FINS), homeostatic model assessment for insulin resistance (HOMA-IR)], body mass index (BMI), waist-to-hip ratio (WHR) before and after treatment were compared. The ovulation rate and pregnancy rate were observed during 6 months follow-up after treatment.Results①There were 122 cases who finally completed the trial, 60 cases in the treatment group and 62 cases in the control group. ②Among them, 2 cases (3.3%) had PCOS Ia, 19 cases (31.7%) had PCOS Ib, 34 cases (56.7%) had PCOS Ⅱa, and 5 cases (8.3%) had PCOS Ⅱb. ③The total effective rate was 91.7% in the treatment group and 82.3% in the control group. The difference in the clinical efficacy between the two groups was statistically significant, and the treatment group showed better clinical efficacy than that of the control group (P<0.05). ④The intra-group comparison showed that BMI, WHR, FINS and HOMA-IR were lowered after treatment in both groups, and the difference was statistically significant (P<0.05). The inter-group comparison showed that there was no significant difference in BMI, WHR, FINS, and HOMA-IR between the two groups after treatment (P>0.05). ⑤The intra-group comparison showed that LH and T levels were significantly lowered in both groups after treatment (P<0.05), and SHBG levels significantly increased in the treatment group after treatment (P<0.05) while the difference in SHBG levels in the control group was not significant before and after treatment (P>0.05). According to the inter-group comparison after treatment, the differences in levels of FSH, E2, DHEA-S, C and 17α-OHP between the two groups were not statistically significant (P>0.05), but the differences in SHBG and T levels were statistically significant, with the improvement in the treatment group being better than that in the control group (P<0.05), while the difference in LH levels was statistically significant, with the improvement in the control group being better than that in the treatment group (P<0.05). ⑥The inter-group comparison showed that the ovulation rate and pregnancy rate were significantly higher in the treatment group than those in the control group (P<0.05). ⑦There were no serious adverse events related to medication administration that required discontinuation during the treatment course in the two groups.ConclusionCompared with the single use of conventional Western medicine, Dr. Yu’s integrated Chinese and Western medicine classification and treatment protocol for PCOS has shown better clinical efficacy, which can improve the overall reproductive endocrine and glucose metabolism disorders of the human body to promote the ovulation rate and pregnancy rate.

      摘要:ObjectiveTo observe the clinical efficacy and safety of Wumen Qingre Huazhuo Formula combined with external application of golden plaster in the treatment of acute gouty arthritis.MethodsA total of 62 patients with acute gouty arthritis were randomly divided into treatment group and control group,31 cases in each group. The patients in the treatment group were treated with Wumen Qingre Huazhuo Decoction and external application of golden plaster, while the patients in the control group were treated with oral etoricoxib and external application of Sodium aescinate gel. After persistent treatment for 1 week, the pain VAS score, degree of swelling, temperature of skin, leukocyte, C-reactive protein and serum uric acid, as well as the incidence of adverse drug reactions were compared between the two groups.ResultsCompared with the control group, the VAS score of the treatment group had no significant change at 6 h after treatment(P>0.05), but the VAS scores decreased significantly at 1 day and 7 days after treatment (P<0.05). Compared with the control group, swelling score, skin temperature score and C-reactive protein levels of the affected limbs in the treatment group decreased significantly at 7 days after treatment (P<0.05), while the levels of leukocyte and serum uric acid had no significant change (P>0.05). The incidence of adverse drug reactions was 9.68% in the treatment group and 12.90% in the control group, the difference was not statistically significant (P>0.05).ConclusionWumen Qingre Huazhuo Formula combined with external application of golden plaster can better relieve the acute symptoms of gouty arthritis, improve joint function, reduce inflammation, and do not increase the incidence of adverse drug reactions in the treatment of acute gouty arthritis.

      摘要:ObjectiveTo investigate the clinical efficacy of Xiaotan Tongluo Gel combined with gabapentin capsules and methylcobalamin tablets in treating postherpetic neuralgia (PHN).MethodsTotally 125 patients with PHN were randomly divided into a treatment group (n=65) and a control group (n=60). Both groups were administered with gabapentin capsules and mecobalamin tablets orally. Additionally, the treatment group was provided with Xiaotan Tongluo Gel for external application while the control group was provided with placebo gel. Both groups were treated continuously for 2 courses with 14 days in each course. The clinical efficacy was observed, and the changes of Visual Analogue Scale (VAS) score, Pittsburgh Sleep Quality Index (PSQI) score and serum substance P (SP) level were compared.Results①The total effective rate was 70.77% in the treatment group and 53.33% in the control group after 14 days of treatment, and the clinical efficacy of the treatment group was better than that of the control group (P<0.05). After 28 days of treatment, the total effective rate was 89.23% in the treatment group and 70.00% in the control group, and the clinical efficacy of the treatment group was better than that of the control group (P<0.01). ②The VAS score, PSQI score, and serum SP level decreased in both groups after 14 days and 28 days of treatment compared with those before treatment (P<0.01). The VAS score, PSQI score and SP level in the treatment group were lower than those in the control group after 14 days and 28 days of treatment, respectively (P<0.05, P<0.01).ConclusionCompared with the clinical efficacy of gabapentin capsules and methylcobalamin tablets, the application of Xiaotan Tongluo gel together with gabapentin and methylcobalamin has achieved more satisfactory results in the treatment of PHN, which could better alleviate pain and improve patients’ quality of life.

      摘要:The application of mathematical models in the study of traditional Chinese medicine compounds has been illustrated from three aspects: the composition, the compatibility law and the efficacy evaluation. Meanwhile, an academic hypothesis has been put forward, introducing the mathematical model of tumor growth kinetics (TGK) to explain the therapeutic efficacy of traditional Chinese medicine compound intervention on malignant tumor, which may be more in line with the clinical practice of “syndrome differentiation-based treatment” of traditional Chinese medicine. Our hypothesis is a breakthrough from the traditional thinking of traditional Chinese medicine compound mechanism research and can comprehensively explain the immutable nature of “disease-efficacy” of traditional Chinese medicine intervention on malignant tumor.
