摘要:Lower limb chronic ulcer (ecthyma) is a common and frequently-occurring disease in the Department of Peripheral Vascular Diseases. It often occurs in the lower limbs, and is usually accompanied by a history of varicose veins in the lower limbs. The ulcer is difficult to heal, or it recurs due to injury after recovery, causing intractability in the clinical practice. The various internal and external treatments of traditional Chinese medicine (TCM) have unique advantages and remarkable efficacy in treating the lower limb chronic ulcer. Professional Committee on Peripheral Vascular Disease of Shanghai Association of Integrative Medicine organized domestic experts in the field of peripheral vascular diseases to form an expert consensus on TCM diagnosis, treatment and efficacy evaluation of the lower limb chronic ulcer so as to provide reference for the clinical diagnosis, treatment and scientific research work.

摘要:According to traditional Chinese medicine (TCM) theory, the pathogenic characteristics of chemotherapy-induced thrombocytopenia (CIT) are summarized under the guidance of holistic concept and syndrome differentiation-based treatment, taking the deficiency of liver, spleen and kidney as the root, fire-exuberance and heat-toxin as the manifestation, and internal stagnation of blood stasis as the critical change. The therapeutic countermeasures are provided correspondingly. The method of invigorating spleen and nourishing liver is adopted to produce blood, the method of purging fire and removing toxicity is used to treat the symptoms and the method of activating blood and dispelling stasis is for the treatment of manifestation. Treating methods should be adjusted as the condition changes to achieve therapeutic effects. A proved case is attached for verification.

摘要:ObjectiveTo explore the medication rules of traditional Chinese medicine (TCM) prescriptions for hyperprolactinemia (HPRL), extract the core prescriptions, and preliminarily seek the relevant mechanisms based on text mining and bioinformatics methods, in order to guide the clinical treatment.MethodsThe literatures of TCM cases and clinical research on the treatment of HPRL were searched from China National Knowledge Infrastructure (CNKI), WanFang Database (WangFang) and VIP Database (VIP). The TCM syndrome types of HPRL, the nature, flavor and channel tropism of medicines, the rules of prescription and dosage and the prescription characteristics were analyzed, and the core herbs and prescriptions were mined through TCM Inheritance Auxiliary Platform. The effective targets and mechanisms of core prescriptions were analyzed by bioinformatics methods.ResultsA total of 75 cases with complete prescriptions were screened and input involving 75 relevant prescriptions and 147 Chinese medicinal herbs. The main TCM syndrome of HPRL was liver stagnation and kidney deficiency syndrome, the nature and flavor of the Chinese medicinal herbs were mostly warm, flat, sweet, bitter and spicy, and the Chinese medicinal herbs mainly belonged to the liver, spleen and kidney channels. The dosage of raw barley germinating fruit in treating HPRL was mostly 60 g. The high-frequency Chinese medicinal herbs included root of common peony, root of Chinese angelica, root of Chinese thorowax, raw barley germinating fruit, Indian bread, root of ural licorice and so on, and 40 relevant herb groups were obtained. The extracted core prescription was modified Xiaoyaosan Decoction, and could be used to treat HPRL through 28 target proteins involving 928 biological processes and 36 signaling pathways. The key therapeutic effects were closely related to the PI3K-Akt signaling pathway, JAK-STAT signaling pathway, VEGF signaling pathway, Th17 cell differentiation signaling pathway, and dopaminergic neurosynaptic conduction pathway.ConclusionThe therapeutic method of TCM on HPRL is mainly soothing liver, strengthening spleen and nourishing kidney, which could be summarized as the regulation of three internal organs. The nature and flavour of herbs is sweet, warm and mild. The core prescription is based on Xiaoyaosan Decoction, and the analysis of bioinformatics method shows that each herb in the core prescription can effectively act on HPRL, which has potential guidance value for TCM in treating HPRL.

摘要:The purpose of this paper is to summarize Professor Liu Ping’s clinical experience in the treatment of insomnia with liver cirrhosis based on the theory of “liver storing soul”. It is considered that the basic pathogenesis of insomnia with liver cirrhosis is deficiency of yin and blood of the live and failure of the liver to storing soul. Hence, the basic therapy is enriching yin and nourishing blood as well as clearing heat, and the prescription used is the decoction of modified Yiguanjian. And one proven case is presented.

摘要:ObjectiveTo explore the distribution of traditional Chinese medicine (TCM) syndromes in patients with colorectal precancerous lesions, and follow up their recurrence rate and canceration rate.MethodsThe patients with colorectal precancerous lesions admitted to Changzheng Hospital affiliated to Naval Military Medical University from January 2010 to December 2015 were enrolled as the research objects, and their baseline conditions (pathological type, gender, age, adenoma location and size, etc.), TCM syndrome differentiation, recurrence rate and canceration rate were analyzed retrospectively.Results①In the course of study, a total of 316 effective cases were collected. Among them, there were 217 cases of low-grade intraepithelial neoplasia (LGIN) and 99 cases of high-grade intraepithelial neoplasia (HGIN); there were slightly more males (58.54%) than females (41.46%); the age range of most patients was from the age of 40 to 60 (65.82%); precancerous lesions mostly occurred in the left colon (73.42%); most tumors were less than 2 cm in size (86.08%). ②There were 73 cases of spleen deficiency and qi stagnation syndrome, 86 cases of damp-heat accumulation and binding syndrome, 85 cases of excessive phlegm-turbidity stagnation syndrome, 37 cases of yang deficiency and cold coagulation syndrome, and 35 cases of blood stasis syndrome. ③The results of TCM syndrome differentiation showed that the cases with damp-heat accumulation and binding syndrome and excessive phlegm-turbidity stagnation syndrome accounted for 27.22% and 26.90% respectively, followed by cases with spleen deficiency and qi stagnation syndrome (23.10%). In the LGIN group, the cases with spleen deficiency and qi stagnation syndrome (27.65%) and damp-heat accumulation and binding syndrome (27.65%) were in the majority; In the HGIN group, the cases with excessive phlegm-turbidity stagnation syndrome accounted for 32.32%, which was the highest proportion, followed by the cases with damp-heat accumulation and binding syndrome, which accounted for 26.26%. ④In the LGIN group, cases with the damp-heat accumulation and binding syndrome had the highest 5-year recurrence rate (61.67%), followed by the cases with yang deficiency and cold coagulation syndrome (58.33%), excessive phlegm-turbidity stagnation syndrome (56.6%) and spleen deficiency and qi stagnation syndrome (40.00%), and cases with blood stasis syndrome had the lowest 5-year recurrence rate (35.00%). In the HGIN group, cases with the blood stasis syndrome had the highest 5-year recurrence rate (80.00%), followed by cases with damp-heat accumulation and binding syndrome (69.23%), excessive phlegm-turbidity stagnation syndrome (62.50%), spleen deficiency and qi stagnation syndrome (53.85%) and yang deficiency and cold coagulation syndrome (53.85%). ⑤In terms of canceration rate, patients with canceration usually had excessive phlegm-turbidity stagnation syndrome, damp-heat accumulation and binding syndrome or blood stasis syndrome, and the canceration rates of each syndrome were 9.38%, 6.67% and 3.85% respectively.ConclusionsPatients with colorectal precancerous lesions mostly have damp-heat accumulation and binding syndrome, excessive phlegm-turbidity stagnation syndrome or spleen deficiency and qi stagnation syndrome. Phlegm-turbidity, damp-heat and blood stasis are not only pathological products, but also important pathogenic factors leading to the recurrence and canceration of colorectal precancerous lesions, which should be paid special attention to in clinical practice.

摘要:ObjectiveTo explore the mechanism of anti-colorectal cancer activity of Canmei Formula at the transcriptome level by second generation sequencing.MethodsMume Fructus and Bombyx Batryticatus, the core drugs for the treatment of intestine wind from the Compendium of Materia Medica, were selected to compose of Canmei Formula and to interfere with RKO cells (intestinal cancer cells). The control group(RKO-Control)and the experimental group (RKO-treatment)were sequenced by the second generation high-throughput sequencing platform (IlluminaHi-Seq), and the biological function enrichment analysis was carried out by using gene ontology (GO) and Kyoto encyclopedia of genes and genomes (KEGG). Molecular docking methods was used to verify the binding effect of active ingredients with key targets. Results The number of differential genes was 2 516, including 1 279 up-regulated genes and 1 237 down-regulated genes. Three core differential genes related to colorectal cancer, FOXD1, FBLN2 and LDLRAD2, were screened. GO analysis showed that the differential genes were mainly involved in single-organism metabolic process, cell cycle and response to organic substance, while KEGG analysis showed that differentially expressed genes were mainly involved in metabolic pathway, cell cycle and RNA transport. Molecular docking results preliminarily verified that the main active components of Canmei Formula had high affinity with core targets.ConclusionCanmei Formula may play a role in anti-colorectal cancer from the aspects of gene transcriptional expression, cell metabolism and apoptosis through multi-target and multi-pathway.

摘要:ObjectiveStudy the effect of different doses of Wuzhuyu Decoction on cisplatin induced chemotherapy-related vomiting and intestinal mucositis in rats, and further explore its mechanism.MethodsThe active components of Wuzhuyu Decoction were identified by time-of-flight high-resolution mass spectrometry. Totally 48 SD rats were randomly divided into the control group, the model group, the different concentrations of Wuzhuyu Decoction groups (5,10,20 mL/kg) and the positive control group(tolansetron group),with 8 rats in each group. The heterophilic kaolin rats was observed on the 1st, 3rd, 5th and 7th day after modeling. The concentration of serum 5-hydroxytryptamine (5-HT) rats in each group was quantified by liquid chromatography-mass spectrometry (LC/MS), and the changes of colonic tissue and the expression of γ-H2AX and CD45 were observed by HE staining and immunofluorescence.ResultsEvodiamine and rutaecarpine are the main components of Wuzhuyu Decoction. After treatment, compared with the model group, the amount of heterophilic kaolin and the concentration of serum 5-HT in the high and medium dose groups of Wuzhuyu Decoction decreased significantly (P<0.01), and the pathomorphological observation showed that the high and medium dose groups of Wuzhuyu Decoction had protective effect on rat colonic mucosa and decreased the expression of γ-H2AX and CD45 (P<0.01).ConclusionWuzhuyu Decoction can treat cisplatin-induced vomiting and reduce intestinal inflammation in rats, and its mechanism may be related to reducing the concentration of 5-HT in serum and inhibiting the expression of γ-H2AX and CD45 in intestinal tract.

摘要:ObjectiveTo elucidate the effect and mechanism of Yiqi Fuzheng Prescription in regulating the phenotypic transformation of tumor-associated macrophages (TAMs).Methods①The model of M2 TAMs was established in vitro. The concentration of Yiqi Fuzheng Prescription was determined by CCK-8 experiment. The expression of M1 and M2 TAMs was identified by flow cytometry. The mRNA expression levels of CCL2, AMPK and mTOR were detected by RT-PCR, and Western blot was used to detect the expression of CCL2, AMPK and mTOR in TAMs. ②The subcutaneous transplanted tumor model of Lewis lung cancer in mice was established, and the proportion of M1 and M2 TAMs in each group was detected by flow cytometry.Results① Yiqi Fuzheng Prescription could down regulate the expression of M2 TAMs and up regulate the expression of M1 type TAMs in vitro; At the same time, Yiqi Fuzheng Decoction down regulated the mRNA expression of CCL2 and mTOR in M2 TAMs (P<0.001), and up regulated the mRNA expression of AMPK (P<0.05); Western blot showed that compared with the model group, the expressions of CCL2 and mTOR were down regulated and the expression of AMPK was up-regulated in the TCM group (P<0.001). ②Animal experiments showed that Yiqi Fuzheng Prescription could inhibit the growth of Lewis lung cancer transplanted tumor in mice. The flow cytometry results of tumor cells showed that Yiqi Fuzheng Prescription could inhibit the expression of M2 TAMs, up regulate the expression of M1 TAMs in tumor tissues, and the difference was statistically significant (P<0.05).ConclusionYiqi Fuzheng Prescription can inhibit the transformation of TAMs into the M2 phenotype, and it plays a role in regulating the expression of each signaling molecule in CCL2/AMPK/mTOR, an important signaling pathway of TAMs polarization, suggesting that Yiqi Fuzheng Prescription may intervene in the TAMs polarizationto inhibit tumor metastasis through regulating the signaling pathway CCL2/AMPK/mTOR.

摘要:ObjectiveTo establish UPLC fingerprint of Dahuang Zhechong Pill, as well as pattern recognition, and provide ideas and references for the establishment of corresponding quality evaluation system.MethodsWaters Acquity UPLC HSS T3 column (2.1 mm×100 mm, 1.8 μm) was taken, and acetonitrile-0.2% formic acid solution with gradient elution was used as mobile phase. The detection wavelength switching procedure was optimized according to the peak sequence of the main components of various medicinal materials. The column temperature was 35 ℃,and the flow rate was 0.3 mL/min. The TCM chromatographic fingerprint similarity evaluation system (version 2012.1) was used to establish the control fingerprint of Dahuang Zhechong Pill and evaluate the similarity, and then ChemPattern chemometrics software (version 2017) was used for cluster analysis, principal component analysis (PCA), partial least squares regression analysis (PLS) and pattern recognition.ResultsIn the fingerprint, 30 common peaks were confirmed. According to the chromatographic peak behavior, reference ion fragments in UPLC-ESI-QTOF-MS and literature information, 13 known components were identified and other common peaks were attributed. The similarity of samples from B, C, F and G manufacturers was more than 0.90. PLS analysis could clearly distinguish the differences of samples from different manufacturers. Amygdalin and several main components of Scutellariae Radix and Rhei Radix et Rhizoma were selected as strong characteristic peaks. The results of cluster analysis, PCA and PLS were mutually verified.ConclusionThe UPLC fingerprint of Dahuang Zhechong Pill shows good stability and repeatability. Combined with chemical pattern recognition method, it can quickly and accurately reflect the overall quality of Dahuang Zhechong Pill, which can provide the basis for its quality control and evaluation.

摘要:This article reviews the resistance mechanism of epidermal growth factor receptor (EGFR) targeted therapy for colorectal cancer and the research progress of traditional Chinese herbal medicines (TCHMs). Molecular mutation of the EGFR signaling pathway and abnormal activation of the compensatory signaling pathway are the main mechanisms for the resistance to anti-EGFR therapy. TCHMs have shown an obvious advantage in reversing tumor drug resistance owing to their multi-targeting effects. A variety of traditional Chinese herbal medicines and their active components not only have direct anti-tumor effects, but also can enhance the sensitivity of anti-tumor drugs and inhibit tumor drug resistance when used in combination with targeted drugs, which therefore are worthy of further in-depth study.

摘要:This review summarized current studies on how traditional Chinese medicine influenced epigenetic modifications in women with polycystic ovary syndrome (PCOS) during gestation. Studies suggested hyperandrogenic intrauterine environment associate with altered fetal epigenetic modification and placental structure and functions, which in turn caused changes of fetal programming and influences offspring’s health in their adulthoods. Traditional Chinese medicine provided a new target for disease prevention and treatment through reversing altered epigenetic modification induced by hyperandrogenic intrauterine environment in women with PCOS.
