摘要:The role and significance of traditional Chinese medicine(TCM) in preventing and treating coronavirus disease 2019(COVID-19) through regulating human immunity was explained from three aspects:the first is the prevention and treatment methods of strengthening body resistance, eliminating pathogenic factors and harmonizing yin and yang, the second is the guiding principles of identifying etiology based on differentiation of symptoms and signs and treating in accordance with pattern identification, and the third is the core connotation of “preventive treatment of diseases”—preventing diseases before the occurrence and preventing changes of existing diseases. It is believed that TCM can effectively prevent and treat COVID-19 due to its unique theory of “healthy qi”, which shares something in common with the connotation of modern immunology.

摘要:Coronavirus disease 2019 (COVID-19) has the characteristics of pathogenic “wind” in its transmission route, onset manifestation and main symptoms and is similar to “lung wind” recorded in Nei Jing (Yellow Emperor’s Inner Canon). Through in-depth study of “lung wind”, it is thought that such formulas as Duhuo powder and Mahuang Xuming decoction for the treatment of “lung wind” can also be used for the treatment of COVID-19. The previous clinical experience of treating “lung wind” conveys two messages:Firstly, for patients with COVID-19, visceral deficiency is the root and pathogenic wind is the branch. When the disease is mild, treat the root and branch simultaneously; When the disease becomes severe and critical, treat the root or branch separately; Secondly, COVID-19 can be treated from the perspective of “wind” in TCM, that is to treat both the external and internal by dispelling pathogenic wind and activating yang qi.

摘要:The theoretic foundation of pathogenesis and treatment of coronavirus disease 2019 (COVID-19) was explored based on Li Gao’s theory of spleen and stomach. It is thought that the COVID-19 mainly attacks the lungs, but spleen deficiency and dampness abundance is the major pathological basis for its pathogenesis. In the treatment, tonifying the middle-jiao and supplementing qi, lifting clear-yang and expelling dampness with bitter-warm ingredients are the predominant methods for patients with deficiency syndrome, and a small amount of medicinals for clearing heat and draining dampness can be used as supplementary.

摘要:Due to the outbreak of a major epidemic, individuals are influenced by pestilential qi and emotional internal injuries, resulting in abnormalities of Shen, which are manifested as physical and mental disharmony. There are "four strategies" for regulating Shen, namely, speech (counseling and guidance), emotion (competitiveness and transfer), essence (conversion and tranquility) and physique (qigong and relaxation), which can be selected according to individual’s physical characteristics and the difference in the degree of virus exposure.

摘要:Based on the characteristics of the pathogenesis and clinical manifestations, locations of COVID-19 are mainly in the pulmonary collaterals, and the core pathogenesis is “toxin impairing pulmonary collaterals”. Treatments of dispelling damp toxin, clearing away heat toxin and resolving stasis toxin should be applied as early as possible to clear the lung. The methods of aromatic resuscitation and clearing collaterals should be used in severe and critical periods. We should pay enough attention to open-close treatments on the depletion syndromes and resolving blood stasis to clear collaterals.During the convalescence, tonifying and activating the pulmonary collaterals, clearing away residual pathogens should be used. Based on “removing toxicity and dredging channels”, differentiation treatments of comprehensive syndromes could effectively relieve clinical symptoms, prevent progression and improve prognosis.

摘要:Based on the research results of lung function rehabilitation with traditional Chinese and modern medicine at home and abroad, the pulmonary function rehabilitation strategy for mechanically ventilated patients with severe/critical coronavirus disease 2019 (COVID-19) was discussed. Mechanical ventilation is an essential supporting method for the treatment of COVID-19, but it may cause respiratory myasthenia, lead to sputum excretion disorder, and further affect the prognosis. For mechanically ventilated patients with severe/critical COVID-19, it is of great significance to take necessary pulmonary function rehabilitation intervention measures with traditional Chinese medicine and modern medicine as early as possible, which not only help to relieve the dyspnea symptom of patients, but also shorten the hospitalization duration and improve the prognosis.

摘要:Objective:To analyze and summarize rules of traditional Chinese medicine (TCM) prescriptions in the prevention and treatment of non-critical coronavirus disease 2019 (COVID-19) (including mild, moderate and severe types) by data mining through multiple versions of Diagnosis and Treatment protocol for Coronavirus Disease 2019 (hereinafter referred to as “COVID-19”) Prevention and Control Program issued by the State and the health committees of all provinces, autonomous regions and municipalities directly under the Central Government and the Administration of Traditional Chinese Medicine (or medical and health management institutions). MethodsTCM prescriptions in the multiple versions of Diagnosis and Treatment Protocol for COVID-19 issued by the state and 24 provinces, autonomous regions and municipalities directly under the Central Government were retrieved. The prescription database was established after screening the source data, and the data analysis and mining were carried out by using frequency analysis, Apriori algorithm in association rules and complex system entropy clustering algorithm methods embedded in TCMISS (V2.0.1), so as to analyze the core medicinal herbs and the corresponding properties, tastes, tropisms, medication modes and generate new prescriptions.  Results:①A total of 149 formulas and 146 kinds of medicinal herbs were analyzed after screening. It was found that the top 7 medicinal herbs used frequently were Gancao (Glycyrrhizae Radix), Ku Xingren (Armeniacae Semen), Mahuang (Ephedrae Herba), Chenpi (Citri Reticulatae Pericarpium), Shigao (Gypsum Fibrosum), Cangzhu (Atractylodis Rhizoma) and Huoxiang (Agastaches Herba) respectively. Most medicinal herbs have bitter taste, warm property and enter the lung, spleen, stomach and heart meridians. ②According to the preliminary analysis of association rules, the medicinal pairs were ranked from high to low in frequency, and the top three were “Ku Xingren (Armeniacae Semen) & Mahuang (Ephedrae Herba)”, “Gancao (Glycyrrhizae Radix) & Ku Xingren (Armeniacae Semen)”, and “Shigao (Gypsum Fibrosum) & Ku Xingren (Armeniacae Semen)”. ③There were 11 medicinal pairs with correlation coefficient above 0.040, such as “Mahuang (Ephedrae Herba) & Chishao (Paeoniae Radix Rubra)”, “Mahuang (Ephedrae Herba) & Baizhu (Atractylodis macrocephalae Rhizoma)” and “Mahuang (Ephedrae Herba) & Renshen (Ginseng Radix)”, etc. ④According to the analysis of prescriptions in types, San’ao decoction was used as the core formula for the treatment of mild cases together with Huoxiang Zhengqi powder, Dayuan decoction and Fangfeng Tongsheng powder. Maxing Shigan decoction, Dayuan decoction and Yinqiao Powder were used jointly for the treatment of moderate cases. The combination of Maxing Shigan decoction, Xuanbai Chengqi decoction and Sini formulas for heart resurrection was used for the treatment of severe cases. ⑤According to the complex systems entropy cluster algorithm, 14 medicinal groups containing 3 to 4 core medicinal herbs were deduced, such as “Tinglizi (Lepidii Semen), Taoren (Persicae Semen) and Baizhu (Atractylodis macrocephalae Rhizoma)”, “Tinglizi (Lepidii Semen), Dahuang (Rhei Radix et Rhizoma) and Baizhu (Atractylodis macrocephalae Rhizoma)”, “Lianqiao (Forsythiae Fructus), Jinyinhua (Lonicerae Flos) and Lugen (Phragmitis Rhizoma)”, etc; and 7 new prescriptions were generated, such as “Tinglizi (Lepidii Semen), Taoren (Persicae Semen), Baizhu (Atractylodis macrocephalae Rhizoma), Chishao (Paeoniae Radix Rubra) and Gualou (Trichosanthis Fructus)”, “Ku Xingren (Armeniacae Semen), Huangqi (Astragali Radix), Taoren (Persicae Semen), Mahuang (Ephedrae Herba) and Shigao (Gypsum Fibrosum)”, “Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Huoxiang (Agastaches Herba), Fuling (Poria), Dahuang (Rhei Radix et Rhizoma), Shigao (Gypsum Fibrosum) and Gualou (Trichosanthis Fructus)”, etc.  Conclusion:At present, the rules of the recommended prescriptions in multiple versions of Diagnosis and Treatment Protocol for COVID-19 focus on eliminating phlegm, relieving cough and asthma, relieving exterior and regulating qi, clearing heat and resolving dampness, invigorating qi, etc., with the treatment of lung as the key and treatment of the spleen and stomach as the complementation. During the treatment period, the rules of prescriptions for the treatment of three types vary:the mild type focuses on relieving exterior and eliminating dampness, the moderate type focuses on relieving both the exterior and interior, eliminating dampness and clearing heat and the severe type focuses on purging heat and attacking interior, invigorating qi and restoring yang. The statistical analysis results are basically consistent with the consensus of experts, and some new prescription rules with potential clinical value have also been found, which can provide theoretical basis for clinical treatment and new drug development.

摘要:Objective:To design a safe, effective, simple and easy-to-learn Kangyi Qiangshen Gong (anti-epidemic and body-strengthening exercise) for clinical application to combat epidemic and strengthen body function through a retrospective analysis of 308 coronavirus disease 2019 (COVID-19) cases in Huangshi, Hubei province and with the combination of core elements and research evidence of traditional Chinese medicine (TCM) Qigong exercises. MethodsThe medical records of 308 COVID-19 cases (classified into four types:mild, moderate, severe and critical) admitted to Huangshi Hospital of Traditional Chinese Medicine from January 25,2020 to February 20,2020 were collected, and the clinical data, auxiliary examinations and treatment plans of each type were analyzed statistically. The clinical data included each patient's general information (gender, age), admission signs, degree of illness, TCM syndrome differentiation and classification, main clinical symptoms, time required for symptom improvement, residual symptoms at discharge, CT image changes of the lungs, and treatment outcomes, etc.  Results:①On admission, the total proportion of patients with moderate and mild COVID-19 reached 61%, and with the continuous recovery of severe cases, the proportion of moderate and mild cases further increased. With the aggravation of the disease from mild to critical, the average hospitalization duration of patients was continuously prolonged. ②The average age of the dead patients with critical COVID-19 was 74.1 years old, which was significantly different from that of dead patients with severe COVID-19. The age of dead patients of critical type was older (P<0.05), and before death the average hospitalization time was 9.2 days which showed a rapid progress of disease. The death cases were all patients with severe or critical COVID-19, and all of these patients were diagnosed with basic diseases before the confirmation of infection. ③The mortality rate of 308 cases in this study reached 5.19%, which was higher than the national average mortality rate of 3.83% issued by the National Center for Disease Control and Prevention. The mortality rate of critical cases was 57.14%,while the mortality rate of severe cases was 4.82%. The number of death cases of mild and moderate type was 0 and no case changed to severe type, but there were 4 death cases of critical type changing from severe type. ④The TCM syndromes of 308 patients were mainly cold-dampness stagnating in the lungs, accounting for 82%. There were also 13% of wind-heat invading the lungs syndrome and 5% of dampness-pathogen stagnating in the lungs syndrome, which were syndromes unmentioned in the Diagnosis and Treatment Protocol for COVID-19 issued by the National Health Committee. ⑤Cough, fever, chest tightness and fatigue were the most important and common symptoms. Among them, fever and diarrhea relatively had faster improvement with shorter duration and less residual symptoms at discharge. Cough, chest tightness and fatigue took longer to improve with longer persistence of symptoms at discharge. Cough, in particular, had the longest persistence at discharge compared with other symptoms and the difference was statistically significant (P<0.05). ⑥In typical cases, lung CT showed slightly reduced scope of infection foci and reduced density after treatment, but both lungs still showed ground glass-like changes suggesting that the lung function of patients would decrease in the later stage and there would be chest tightness and dyspnea. ⑦By reviewing ancient and modern literature of TCM Qigong exercises and methods, and combining the clinical characteristics of COVID-19 cases in Hubei province, TCM Qigong exercises can be respectively designed for different types of patients, ranging from moderate type, mild type to discharged ones.  Conclusion:Based on the clinical characteristics of COVID-19 cases in Hubei province, the designed Kangyi Qiangshen Gong (anti-epidemic and body-strengthening exercise) are expected to relieve the main clinical symptoms of patients with COVID-19, especially those persistent symptoms at discharge such as cough, chest tightness and fatigue, thus promoting the early recovery of patients.

摘要:Objective:To analyze the distribution characteristics of TCM syndromes in convalescent patients with coronavirus disease 2019 (COVID-19) in Shanghai. MethodsSixty-six convalescent patients with COVID-19 who had been treated in Shanghai Public Health Clinical Center were included. Data collection was completed within 24 hours after the patient was discharged from hospital, including basic information (gender, age, past medical history, smoking history, epidemiological data and medication during hospitalization), TCM syndrome information, tongue image characteristics and course of disease, and analysis and differentiation were carried out according to the “COVID-19 Diagnosis and Treatment Program (Trial Implementation of the Sixth Edition)”.  Results:①The main symptoms of convalescent patients with COVID-19 were short yellow urine, thirst, sweating, bitter mouth, fatigue, chest tightness and shortness of breath with red or dark red tongue (77.3%) and greasy coating (53.0%); The proportion of patients with dual deficiency of qi and yin was 81.8%, and that of patients with deficiency of lung and spleen qi was 18.2%. ②There were 20 cases (37.0%) with damp-heat syndrome in patients with dual deficiency of qi and yin and 10 cases (83.3%) with phlegm-dampness syndrome in patients with lung and spleen qi deficiency (P<0.05). The incidence rate of dampness syndrome in patients with lung and spleen qi deficiency was higher than that in patients with dual deficiency of qi and yin (P<0.05). ③The disease course in patients with dual deficiency of qi and yin was longer than that in patients with lung and spleen qi deficiency (P<0.05), but there was no significant correlation between the course of disease and whether the dampness syndrome was combined (P>0.05).  Conclusion:Dual deficiency of qi and yin is the main syndrome of patients with COVID-19 in the recovery period with a longer course of disease. Patients with deficiency of lung and spleen qi are more prone to be complicated with dampness syndrome.

摘要:Based on two cured cases of lung pestilence with high fever and lung pestilence with sudden onset of severe cough, TCM clinical thinking pattern, syndrome differentiation and medication methods for the treatment of corona virus disease 2019 (COVID-19) were introduced. It is believed that COVID-19 can be treated according to the treatment of lung pestilence, and severe patients should be treated with traditional Chinese medicine as early as possible. Syndromes should be differentiated based on the individual, climatic and geographic factors, as well as the results of modern medicine examinations.

摘要:Several common problems in the diagnosis and treatment of coronavirus disease 2019 (COVID-19) with integrated Chinese and western medicine were discussed based on one case record. It is believed that digestive tract symptoms are also prominent in patients with COVID-19 besides respiratory tract manifestations. Early diagnosis and treatment evaluation include comprehensive analysis of CT examination of the lungs and virus nucleic acid examination. At the middle stage, emphasis is placed on the toxic phlegm and blood stasis, and prevention of liver damage during medication. At the advanced stage the lungs and intestines should be treated simultaneously. During the recovery period it is appropriate to adopt the methods of strengthening the spleen and tonifying the lung, activating blood circulation and removing blood stasis.

摘要:This article discusses the TCM diagnosis and treatment of functional dyspepsia based on the theory of “stagnation due to deficiency”. It is considered that functional dyspepsia is caused by weakness of the spleen and stomach, with “stagnation due to deficiency” as the main pathogenesis and phlegm-dampness blocking collaterals due to qi stagnation as the turning point of the pathogenesis. Eventually, the result of phlegm-qi stagnation and blood stasis obstructing collaterals leads to persistency of the disease. In clinical practice, doctors should pay close attention to the pathogenesis of “stagnation due to deficiency” and causes of the disease. When treating the disease, doctors should know the principle of treatment and keep in mind the methods such as strengthening the body to dispel pathogenic factors and dispelling pathogenic factors without harming the healthy qi. When prescribing a prescription, doctors should first use Chinese medicines to mildly supplement the spleen and stomach, and apply Chinese medicines to clear away dampness and turbidity, dredge and descend qi and slowly eliminate stasis.

摘要:Shandougen Radix Sophorae Tonkin, RST was recorded as “toxic” in the Chinese Pharmacopoeia (ChP). According to the literature, matrine and oxymatrine mainly contribute to its toxicity, and the former is more poisonous. A retrospective study was carried out on the relationship between its drying and concocting processing methods, usage and toxicity, and the idea of directly slicing fresh RST into decoction pieces was put forward and practiced. Fresh RST was quickly cleaned after harvest, the surface moisture was quickly dried up, and RST was quickly sliced into 5 mm pieces. After a complete second fast-drying, the matrine content was 0.031%. The whole process of RST played a role in controlling and reducing matrine content to lower toxicity to a certain degree.

摘要:Objective:To evaluate and observe the clinical effect on the treatment of incarcerated hemorrhoids combined with Gu’s fumigating, washing and plastering methods at different operation timing. MethodsSeventy patients with incarcerated hemorrhoids were randomly divided into the emergency operation group (35 cases) and selective operation group (35 cases). Patients in the emergency operation group underwent PPH combined with external hemorrhoid dissection on the day of admission or the next day, and were treated with Gu’s fumigating, washing and plastering methods for 21 days after operation. The selective operation group was first treated with Gu’s fumigating washing and plastering methods for 3 to 7 days before operation, and continued to be treated with Gu’s fumigating, washing and plastering methods for 21 days after operation. The hospitalization days and wound healing time were observed, and the changes of main symptoms and signs (pain, edema, exudation, hemorrhage) and quality of life scores between the two groups were compared.  Results:①During the experiment, one case dropped out and one case was eliminated in both groups. There were 66 cases finally completed the experiment with 33 cases in each group. ②=2\*GB3On day 1,3 and 7 after operation, the VAS pain score in the elective operation group was lower than that in the emergency operation group (P < 0.05), and the degree of edema was milder than that in the emergency operation group (P < 0.05); On day 3 after operation, the bleeding amount in the elective operation group was less than that in the emergency operation group (P < 0.05). ③The hospitalization time of the selective operation group was longer than that of the emergency operation group. (P< 0.05). There was no significant difference in postoperative exudation, quality of life score and wound healing time between the two groups (P> 0.05). ④During the trial, no obvious adverse reactions occurred in both groups.  Conclusion:To reduce the related post-operation complications (pain, hemorrhage and edema) at the initial stage, selective operation should be considered in the first place for patients with incarcerated hemorrhoids.

摘要:Objective:To investigate the effect of Wenyang Sanyu Pingchuan recipe on the levels of nuclear factor-κB (NF-κB) p65, signal transducers and activators of transcription 6 (STAT6) and related inflammatory factors in asthmatic mice. MethodsForty male BALB/c mice were randomly divided into blank control group, asthma model group, dexamethasone group and Wenyang Sanyu Pingchuan recipe group, with 10 mice in each group. Except the blank control group, mice in other groups established the asthma model, and were given corresponding drug intervention. HE staining was used to observe the histopathological changes of the lung, the levels of Interleukin 4 (IL-4), Interleukin 13 (IL-13), interferon-γ (IFN-γ) and Immunoglobulin E (IgE) were determined by ELISA, and the protein levels of p65 and STAT6 were detected by Western blot after 28 days.  Results:Compared with the blank control group, the levels of IL-4, IL-13 and p65, STAT6 in the model group increased significantly (P<0.01), and the level of IFN-γ significantly decreased (P<0.01). Compared with model group, the levels of IL-4 and IL-13 in dexamethasone group and Wenyang Sanyu Pingchuan recipe group, the levels of p65 and STAT6 in dexamethasone group and the level of STAT6 in Wenyang Sanyu Pingchuan recipe group significantly reduced (P<0.01). Compared with model group, the levels of IFN-γ in two drug delivery groups significantly elevated (P<0.01), and the level of p65 in Wenyang Sanyu Pingchuan recipe group reduced (P<0.05). Compared with the dexamethasone group, the level of IL-4 in Wenyang Sanyu Pingchuan recipe group increased (P<0.05), the level of IL-13 significantly increased (P<0.01), and the level of IFN-γ significantly decreased (P<0.01 ), but there was no significant difference in the levels of p65 and STAT6 between dexamethasone group and Wenyang Sanyu Pingchuan recipe group (P>0.05).  Conclusion:Wenyang Sanyu Pingchuan Recipe could reduce the levels of p65 and STAT6 in lung tissue, thus affecting the secretion of inflammatory factors IL-4, IL-13 and IFN-γ in lung. Wenyang Sanyu Pingchuan recipe may relieve airway inflammation in asthma model mice by regulating NF-κB and Janus kinase/signal transduction and transcription activator (JAK/STAT) signaling pathways.

摘要:Lumbar disc herniation(LDH)is a common clinical disease, which can be treated by surgery and conservative treatment.Because of the risks of anesthesia and dural tearing during the course of operation, the conservative treatment should be the first choice in clinic. In order to provide the theoretical evidence for clinical conservative treatment, this article summarizes the mechanism of reabsorption after LDH and the relative clinical research. The mechanism of reabsorption after LDH is closely related to vascularization factor, autoimmunity, inflammatory response, tissue dehydration, apoptosis, etc.; in clinic, the reabsorption degree of intervertebral disc is related to age, gender, disease course, etc.,meanwhile is also related to the following factors:the size and type of protrusion, posterior longitudinal ligament,Modic change,“bull’s eye sign”,etc.; in the treatment, the advantage of conservative treatment should be played to promote the reabsorption of LDH,and the combination of drug and non-drug therapy, and the complementation of modern physical factor and traditional Chinese medicine therapy should be applied.
