Differential diagnosis and treatment of Hashimoto thyroiditis based on concept "spleen qi dispersing the essence" concept
"Spleen qi dispersing the essence" refers to the process by which the spleen transforms and disperses the essence from water and grains, which are then transformed into qi, blood, and body fluids, circulating throughout the body. This concept provides a comprehensive understanding of the spleen's role in transportation and transformation and in ascending the clear. It is also central to the proper metabolism of qi, blood, and body fluids. The development of Hashimoto thyroiditis fundamentally results from a disorder in the body's qi, blood, and fluid circulation, often associated with qi stagnation, phlegm accumulation, and blood stasis. Spleen qi deficiency serves as the prerequisite for the onset of Hashimoto thyroiditis, while the obstruction of qi flow initiates the condition, and the intermingling of phlegm and stasis is the final pathological outcome. This pathological progression is consistent with the concept of "spleen failing to disperse the essence" as a core pathogenesis. Based on the correlation between the two, the treatment should focus on "supporting spleen to disperse the essence", which involves a comprehensive strategy of tonifying spleen qi, reinforcing the healthy qi while eliminating pathogenic factors, soothing liver qi to relieve depression, regulating qi movement, resolving phlegm and dampness, activating blood circulation, and removing blood stasis. In this way, the spleen qi can be replenished, the qi movement can be made smooth, phlegm and stasis can be eliminated, and qi, blood and body fluids can be distributed and dispersed following their normal pathways. This paper aims to provide a broader perspective for the diagnosis and treatment of Hashimoto thyroiditis with traditional Chinese medicine. One clinical case is also presented for illustration.
