摘要

分析心房纤维化与气阴两虚、虚风内动的关联机制，结合美国心脏协会（AHA）推荐的心房颤动（简称“房颤”）4阶段管理模式，探讨非瓣膜性房颤的中医分期论治。房颤风险期病机为气阴两虚，房颤阵发期病机为虚风内动，房颤持续期病机为虚风入络，永久性房颤病机为精坏神去。中医治疗采用益气养阴以复脉、熄风定悸以止颤，若房颤持续则用和血通络之法，永久性房颤可予补益安神类方药或针刺调神以缓解症状、控制心室率。

Abstract

This study analyzed the correlation mechanism between atrial fibrosis and the traditional Chinese medicine（TCM） pathogenesis of qi-yin deficiency with internal stirring of deficient wind. Based on the American Heart Association （AHA）'s 4-stage management model for atrial fibrillation （AF）， it explored the stage-based TCM treatment of non-valvular atrial fibrillation. The pathological mechanisms were defined as follows： qi-yin deficiency during the AF risk stage， internal stirring of deficient wind during the paroxysmal stage， deficient wind invading collaterals during the persistent stage， and collapse of essence and spirit during the permanent stage. TCM interventions included replenishing qi and nourishing yin to restore the pulse for the risk stage， extinguishing wind and calming palpitations to stop tremors for the paroxysmal stage， harmonizing blood and unblocking collaterals for the persistent stage， and using tonifying and tranquilizing formulas or acupuncture to regulate spirit for the permanent stage to alleviate symptoms and control ventricular rate.
