摘要

目的,2,观察穿戴式踝穴刺激仪联合综合康复训练对痉挛性偏瘫脑性瘫痪患儿足踝功能的影响。,方法,2,将46例痉挛性偏瘫脑性瘫痪患儿随机分为治疗组、对照组，每组23例；对照组给予综合康复训练，治疗组在综合康复训练的基础上加用穿戴式踝穴刺激仪，两组疗程均为12周。比较两组患侧腓肠肌肌张力、踝关节主动背屈活动度（ROM）、胫骨扭转角度的变化情况。,结果,2,①试验过程中，治疗组、对照组分别脱落1例，最终完成试验者44例，治疗组、对照组各22例。②治疗前后组内比较，两组患侧腓肠肌肌张力降低（,P,<,0.05）；组间治疗后比较，治疗组患侧腓肠肌肌张力低于对照组（,P,<,0.05）。③治疗前后组内比较，两组患侧ROM增加（,P,<,0.05）；组间治疗后比较，治疗组患侧ROM大于对照组（,P,<,0.05）。④治疗前后组内比较，两组患侧胫骨扭转角度无明显变化（,P,>,0.05）；组间治疗后比较，患侧胫骨扭转角度差异无统计学意义（,P,>,0.05）。,结论,2,与单纯采用综合康复训练相比，加用穿戴式踝穴刺激仪能更好地改善痉挛性偏瘫脑性瘫痪患儿的足踝关节活动度、缓解肌肉张力。

Abstract

Objective,2,To investigate the effect of the wearable ankle point stimulator combined with comprehensive rehabilitation training on foot and ankle function in children with spastic hemiplegic cerebral palsy （SHCP）.,Methods,2,Forty-six children with SHCP were randomly divided into the treatment group （,n,=23） and the control group （,n,=23）. The control group was given comprehensive rehabilitation training， and the treatment group was given the wearable ankle point stimulator besides the comprehensive rehabilitation training. The course of treatment for both groups was 12 weeks. The changes in gastrocnemius muscle tone， ankle active dorsiflexion range of motion （ROM） and abnormal angle of tibial torsion were compared between the two groups.,Results,2,①During the trial， one case dropped out respectively from treatment group and control group， and 44 cases finally completed the trial， with 22 cases each in treatment group and control group. ②After treatment， the muscle tone of the gastrocnemius muscle of the affected side decreased in both groups （,P,<,0.05）； and the muscle tone of the gastrocnemius muscle of the affected side was lower in the treatment group than that in the control group （,P,<,0.05）. ③After treatment， the ROM on the affected side increased in both groups （,P,<,0.05）； and the ROM on the affected side in the treatment group was higher than that in the control group （,P,<,0.05）. ④After treatment， there was no significant change in the abnormal angle of tibial torsion on the affected side in both groups （,P,>,0.05）； and the difference between the two groups in the abnormal angle of tibial torsion on the affected side was not statistically significant （,P,>,0.05）.,Conclusion,2,The combined use of wearable ankle point stimulator with comprehensive rehabilitation training can better improve foot and ankle ROM and relieve muscle tone than the single use of comprehensive rehabilitation training in children with SHCP.
