From the aspects of traditional Chinese medicine theory spleen-brain correlation and apoplexy, preliminary clinical practice and research, this article expounded the thinking of treating ischemic stroke from the spleen. The etiology and pathogenesis of ischemic stroke were complex, involving multiple zang-fu organs. “Although five zang-fu organs are related, the spleen is pivotal”. The mechanism of ischemic stroke caused by spleen dysfunction was mainly in pathogen excessiveness and spleen deficiency causing cerebral occlusion; spleen failing to transform properly, phlegm and blood stasis blocking collaterals; spleen failing to manage blood and vessel, brain collaterals stagnation; incoordination between spleen and stomach, qi movement disorder. Therefore, invigorating spleen and strengthening healthy energy should be applied the whole process of ischemic stroke prevention and treatment.
