摘要

目的：观察加味芍药甘草汤联合枸橼酸坦度螺酮治疗中风后小脑性共济失调的临床疗效。方法：将116例中风后小脑性共济失调患者随机分为治疗组和对照组,每组58例。对照组予基础治疗加枸橼酸坦度螺酮口服,治疗组在对照组基础上加服加味芍药甘草汤。两组疗程均为4周,观察临床疗效、Berg平衡量表(BBS)评分、改良巴氏指数(MBI)、小脑后下动脉经颅多普勒超声(TCD)、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分及不良反应情况。结果：①治疗组、对照组总有效率分别为91.38%、72.41%,治疗组临床疗效优于对照组(P＜0.05)；②两组治疗后BBS评分、MBI均较治疗前明显升高(P＜0.05),且治疗组优于对照组(P＜0.05)；③两组治疗后收缩期峰值血流速度(Vs)、舒张期末血流速度(Vd)、平均血流速度(Vm)均较治疗前明显升高(P＜0.05),脉动指数(PI)、阻力反指数(RI)均较治疗前明显降低(P＜0.05),且治疗组均优于对照组(P＜0.05)；④两组治疗后HAMA、HAMD评分均较治疗前明显降低(P＜0.05),且治疗组优于对照组(P＜0.05)；⑤治疗组不良反应发生率明显低于对照组(P＜0.05)。结论：加味芍药甘草汤合枸橼酸坦度螺酮治疗中风后小脑性共济失调疗效显著,可改善患者的平衡能力、日常生活能力、小脑后下动脉TCD水平以及焦虑、抑郁情绪,且安全性高,值得临床推广应用。

Abstract

Objective:To observe the clinical study of modified Shaoyao Gancao decoction combined with tandospirone citrate in treatment of post-stroke cerebellar ataxia. MethodsOne hundred and sixteen patients with post-stroke cerebellar ataxia were randomly divided into treatment group and control group,58 patients in each group,the control group was treated with basic treatment plus tandorone citrate,and the treatment group was treated with modified Shaoyao Gancao decoction on the basis of the control group. The course of treatment in both groups was 4 weeks. The clinical efficacy,Berg Balance Scale(BBS)scores,Modified Barthel Index(MBI)scores, Transcranial Doppler(TCD) ultrasonography of the posterior inferior cerebellar artery,Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)and adverse reactions of the two groups were observed. Results:①The total effective rate of treatment group and control group was 91.38% and 72.41%,and the clinical effect of the treatment group after treatment was better than that of the control group(P<0.05)；②BBS scores and MBI scores of the two groups after treatment were significantly higher than those before treatment(P<0.05),and the treatment group were better than those of the control group(P<0.05)；③TCD in the two groups showed that the peak systolic velocity(Vs),end diastolic velocity(Vd)and mean velocity(Vm)were significantly higher than those before treatment(P<0.05),the levels of Pulsatility Index(PI)and Resistive Index(RI)were significantly lower than those before treatment(P<0.05),and the treatment group were better than those of the control group(P<0.05)； ④The levels of HAMA and HAMD in the two groups after treatment were significantly lower than those before treatment(P<0.05), and the treatment group were better than those of the control group(P<0.05)；⑤Incidence of adverse reactions in the treatment group were lower than those in the control group(P<0.05). Conclusion:Modified Shaoyao Gancao decoction combined with tandospirone citrate has a significant effect on post-stroke cerebellar ataxia, which can improve the balance ability and activity of daily life,obviously improve the TCD level of the posterior inferior cerebellar artery and anxiety and depression,it is safety and worthy of clinical application.
