摘要

目的：观察中西医综合康复方案治疗肝郁气滞型卒中后抑郁的临床疗效。方法：将62例肝郁气滞型卒中后抑郁患者随机分为治疗组和对照组,每组31例。对照组采用西医常规康复训练,治疗组在对照组治疗措施基础上,联合中医综合康复疗法(自拟理气解郁方+针刺+穴位按摩)。两组疗程均为1个月,观察临床疗效,比较汉密尔顿抑郁量表(HAMD)评分、美国国立卫生研究院卒中量表(NIHSS)评分、日常生活活动能力量表(ADL)评分的变化情况。结果：①治疗组、对照组总有效率分别为93.5%、74.2%；组间临床疗效比较,治疗组明显优于对照组(P<0.05)。②治疗前后组内比较,两组HAMD评分、NIHSS评分明显降低(P<0.05),ADL评分明显升高(P<0.05)；组间治疗后比较,治疗组HAMD评分、NIHSS评分明显低于对照组(P<0.05),治疗组ADL评分明显高于对照组(P<0.05)。结论：中西医综合康复方案治疗肝郁气滞型卒中后抑郁,能有效改善患者的抑郁状态,促进其神经功能恢复,提高其生活质量。

Abstract

Objective:To observe the clinical effect of integrated Chinese and western medicine rehabilitation program on post-stroke depression (PSD) of liver qi stagnation pattern. MethodsA total of 62 patients with liver qi stagnation pattern PSD were randomly divided into the treatment group and the control group with 31 cases in each group. The control group was treated with routine rehabilitation training of western medicine, and the treatment group was given the routine rehabilitation training of western medicine combined with the rehabilitation therapy of Chinese medicine (including self-designed Liqi Jieyu (qi-regulating and depression-relieving) formula, acupuncture and acupoint massage). The course of treatment was one month in both groups. The clinical effect was observed and the changes of HAMD, NIHSS and ADL scores were compared. Results:①The total effective rate of the treatment group and the control group was 93.5% and 74.2% respectively, and the clinical effect of the treatment group was significantly better than that of the control group (P<0.05). ②Compared before and after treatment, HAMD scores and NIHSS scores of both groups were significantly lower (P<0.05), and ADL score were significantly higher (P<0.05); after treatment, HAMD scores and NIHSS score of the treatment group were significantly lower than those of the control group (P<0.05), and ADL score of the treatment group was significantly higher than that of the control group (P<0.05). Conclusion:The integrated Chinese and western medicine rehabilitation program for the treatment of liver qi stagnation pattern PSD can effectively improve the depression state of patients, promote the recovery of neurological function and improve the quality of life.
