摘要

目的：探讨强心解郁汤联合常规疗法治疗老年慢性心力衰竭合并抑郁症的临床疗效。方法：将60例老年慢性心力衰竭合并抑郁症患者随机分为对照组和治疗组,每组30例。两组均予心力衰竭常规治疗,对照组同时加服舍曲林,治疗组加服强心解郁汤。两组疗程均为8周,评价抑郁疗效,观察Lee氏心力衰竭评分、HAMD量表总评分及血浆NT-ProBNP、左室射血分数(LVEF)水平变化情况。结果：①=1\*GB3治疗组、对照组抑郁总有效率分别为93.3%、83.3%；组间抑郁疗效比较,差异有统计学意义(P<0.05)。②=2\*GB3与本组治疗前比较,两组治疗后HAMD-24量表总分、Lee氏心力衰竭评分均明显减少(P<0.05),LVEF水平增加(P<0.05),NT-proBNP水平降低(P<0.05)；组间治疗后比较,各指标差异均有统计学意义(P<0.05)。结论：强心解郁汤治疗老年慢性心力衰竭合并抑郁症疗效显著,能有效控制患者的抑郁症状,改善心功能。

Abstract

Objective:To explore the clinical efficacy of Qiangxin Jieyu Decoction combined with conventional therapy in the treatment of chronic heart failure complicated with depression in the elderly. MethodsSixty elderly patients with chronic heart failure complicated with depression were randomly assigned into treatment group and control group, 30 patients in each group. Patients in both groups were given the routine treatment for heart failure. On top of this, patients in the control group were treated with sertraline, and those in the treatment group were given Qiangxin Jieyu Decoction. The treatment duration was 8 weeks in both groups. The treatment effect on depression was evaluated. Lee`s heart failure score, total score on Hamilton Depression Scale (HAMD), plasma NT-proBNP level and left ventricular ejection fraction (LVEF) were observed. Results:①=1\*GB3The total effective rate in depression treatment was 93.3% and 83.3% in the treatment and control groups, respectively; and there was statistically significant difference in the treatment effect on depression between the groups (P<0.05). ②=2\*GB3After treatment, the total HAMD-24 score and Lee`s heart failure score decreased (P<0.05), the LVEF level increased (P<0.05), and the NT-proBNP level decreased (P<0.05) significantly in both groups; and there were statistically significant differences in these indicators between the groups (P<0.05). Conclusion:Qiangxin Jieyu Decoction is effective in the treatment of chronic heart failure complicated with depression in the elderly. It can effectively control the patients’ depressive symptoms and improve heart function.
