摘要

目的：观察蝎蜈胶囊联合川芎平喘合剂治疗痰瘀互结型支气管哮喘发作期的临床疗效。方法：将104例痰瘀互结型支气管哮喘发作期患者随机分为治疗组和对照组,每组52例。治疗组予蝎蜈胶囊联合川芎平喘合剂治疗,对照组予复方甲氧那明胶囊口服及布地奈德粉吸入剂。两组疗程均为7天,观察临床疗效,比较中医证候积分、肺功能指标及相关细胞因子的变化情况。结果：①治疗组、对照组临床总有效率分别为86.5%、75.0%；组间临床疗效比较,治疗组明显优于对照组(P＜0.05)。②治疗前后组内比较,两组中医证候积分差异均有统计学意义(P＜0.05)；组间治疗前后差值比较,差异有统计学意义,治疗组中医证候积分降低幅度明显大于对照组(P＜0.05)。③治疗前后组内比较,两组用力肺活量(FVC)、第1秒用力呼气量(FEV,1,)、最大呼气流速(PEF)水平差异均有统计学意义(P＜0.05)；组间治疗后比较,FEV,1,、PEF水平差异有统计学意义,治疗组改善明显优于对照组(P＜0.05),而FVC差异无统计学意义(P＞0.05)。④治疗前后组内比较,两组血清白细胞介素-4(IL-4)、γ干扰素(IFN-γ)水平差异均有统计学意义(P＜0.05)；组间治疗后比较,IL-4、IFN-γ水平差异无统计学意义(P＞0.05)。结论：蝎蜈胶囊联合川芎平喘合剂治疗痰瘀互结型支气管哮喘发作期的疗效满意,可明显缓解患者临床症状及肺通气功能,其机制可能与纠正Th1/Th2 细胞功能失衡有关。

Abstract

Objective:To observe the clinical efficacy of Xiewu Capsule combined with Chuanxiong Pingchuan Mixture in the treatment of bronchial asthma in attack stage with syndrome of intermingled phlegm and blood stasis. Methods104 patients of bronchial asthma in attack stage with syndrome of intermingled phlegm and blood stasis were randomly divided into the treatment group and control group,52 cases in each group. The treatment group was treated with Xiewu Capsule combined with Chuanxiong Pingchuan Mixture,and the control group was treated with Compound Methoxyphenamine Capsules by oral administration and Budesonide Powder by inhalation,with a course of 7 days. The clinical efficacy was observed,and the changes on TCM syndrome score,pulmonary function indexes and related cytokines were compared. Results:①The clinical total effective rates were 86.5% in the treatment group and 75.0% in the control group,and the clinical efficacy of the treatment group was significantly better than that of the control group (P,<,0.05). ②For the comparison between treatment before and after,there was statistically significant difference on TCM syndrome score in both groups (P,<,0.05). For the comparison of the difference between treatment before and after,there was statistically significant difference between the two groups,and the decrease of TCM syndrome score in the treatment group was significantly greater than that in the control group (P,<,0.05). ③For the comparison between treatment before and after,there were statistically significant differences on the levels of forced vital capacity (FVC),forced expiratory volume in first second (FEV,1,) and peak expiratory flow (PEF) in both groups (P,<,0.05). After treatment,there were statistically significant differences on the levels of FEV,1, and PEF between the two groups,and the improvement in the treatment group was significantly better than that in the control group (P,<,0.05),while there was no statistically significant difference on FVC level between the two groups (P,>,0.05). ④For the comparison between treatment before and after,there were statistically significant differences on the levels of serum interleukin-4 (IL-4) and interferon-γ (IFN-γ) in both groups (P,<,0.05). After treatment,there were no statistically significant differences on the levels of IL-4 and IFN-γ between the two groups (P,>,0.05). Conclusion:Xiewu Capsule combined with Chuanxiong Pingchuan Mixture shows satisfactory efficacy in the treatment of bronchial asthma in attack stage with syndrome of intermingled phlegm and blood stasis,which can obviously alleviate the clinical symptoms and improve the pulmonary ventilation function in patients. Its mechanism may be related to correcting the imbalance of Th1/Th2 cell function.
