目的:采用自行研制的PDA-1型脉诊仪采集非疾病人群脉图,分析不同健康状态、不同年龄梯度的脉图特征,为中医脉诊学的临床应用研究提供参考。 方法:采集1 720例非疾病人群的基本信息及脉图信息,以自主研制的“H20问卷”进行健康状态判别,分析不同年龄梯度健康人群、不同年龄梯度亚健康人群、同年龄梯度内健康与亚健康人群的脉图差异及特征信息。 结果:①=1\*GB31 720例非疾病人群中,健康人群730例,亚健康人群990例。②健康组与亚健康组比较,h,5,值显著降低(P,<,0.01),t,4,、t,5,、h,3,/h,1,、h,4,/h,1,、t,4,/t,5,、w,1,/t、w,2,/t值显著升高(P,<,0.05,P,<,0.01)。③不同年龄梯度健康人群脉图参数比较,40-49岁年龄梯度与≥50岁年龄梯度脉图参数差异无统计学意义(P,>,0.05),而与,<,30岁年龄梯度、30-39岁年龄梯度有差异,具体表现在h,5,值显著降低(P,<,0.01),h,3,、h,4,、t,4,、h,3,/h,1,、w,1,/t等值显著升高(P,<,0.05,P,<,0.01)。④不同年龄梯度亚健康人群脉图参数比较结果显示,随着年龄的增加,h,5,值显著降低(P,<,0.01),t,1,、t,4,、t,4,/t,5,值显著升高(P,<,0.05,P,<,0.01)。⑤同年龄梯度健康与亚健康人群脉图参数比较结果显示,在,<,30岁、30-39岁年龄梯度内,亚健康人群脉图参数h5值显著降低(P,<,0.05),<,30岁年龄梯度t,4,、h,3,/h,1,、t,4,/t,5,、w,1,/t值显著升高(P,<,0.05,P,<,0.01);30-39岁年龄梯度t,4,、h,4,/h,1,值显著升高(P,<,0.05,P,<,0.01);40-49岁、≥50岁年龄梯度内及两个年龄梯度间比较,健康人群与亚健康人群脉图参数差异无统计学意义(P,>,0.05),较,<,30岁、30-39岁年龄梯度,h,5,逐渐降低。 结论:健康人群与亚健康人群的脉图有差异,相同年龄段不同健康状态人群之间的脉图也有差异,主要表现在h,5,、h,3,/h,1,、w,1,/t等指标;研究结果提示,亚健康状态人群的左心室射血功能和大动脉顺应性降低,动脉血管弹性降低,外周阻力增加,脉象偏弦。脉图可以作为一种评估健康状态的客观依据。
Objective:To analyze the pulse graph characteristics of non-disease population with different health status and age gradient based on self-developed pulse diagnosis instrument (PDA-1),and provide reference for the research on clinical application of Chinese medical pulse diagnosis. MethodsThe basic information and pulse graph of 1 720 non-disease subjects were collected. The health status of each case was evaluated by self-developed H20 questionnaire. The differences on pulse graphs and characteristic information were analyzed in healthy subjects with different age gradient,sub-healthy subjects with different age gradient and healthy and sub-healthy subjects with same age. Results:①Among the 1 720 non-disease cases,there were 730 healthy cases and 990 sub-healthy cases. ②Compared with the healthy group,the value of h,5, was decreased significantly in the sub-healthy group (P,<,0.01),and the values of t,4,t,5,h,3,/h,1,h,4,/h,1,t,4,/t,5,w,1,/t and w,2,/t were increased significantly (P,<,0.05,P,<,0.01). ③In healthy subjects,there were no statistically significant differences on the pulse graph parameters between people of age from 40 to 49 year-old and those equal or more than 50 year-old gradient (P,>,0.05). There were statistically significant differences on the pulse graph parameters between people of age from 40 to 49 year-old and those less than 30 year-old or those of age from 30-39 year-old. To be specific,the value of h,5, was decreased significantly (P,<,0.01) and the values of h,3,h,4,t,4,h,3,/h,1,w,1,/t were increased significantly (P,<,0.05,P,<,0.01) in the pulse graph of subjects with age from 40 to 49 year-old. ④In sub-healthy subjects,with the increasing age,the value of h,5, was decreased significantly (P,<,0.01) and the values of t,1,t,4,t,4,/t,5, were significantly increased (P,<,0.05,P,<,0.01). ⑤For the comparison between the healthy and sub-healthy subjects with the same age gradient,in the age of ,<,30 and 30 to 39 year-old,the value of h5 was decreased significantly in sub-healthy subjects(P,<,0.05);in the age of ,<,30 year-old,the values of t,4,h,3,/h,1,t,4,/t,5, and w,1,/t were increased significantly in sub-healthy subjects (P,<,0.05,P,<,0.01);in the age of 30-39 year-old,the values of t,4, and h,4,/h,1, were significantly increased in sub-healthy subjects (P,<,0.05,P,<,0.01). There were no statistically significant differences on the pulse graph parameters between healthy and sub-healthy subjects at 40 to 49 year-old gradient and equal or more than 50 year-old gradient (P,>,0.05). Compared with the age of ,<,30 and 30-39 year-old,the value of h,5, was gradually decreased in the subjects with age of 40 to 49 and ≥50 year-old. Conclusion:There are differences on the pulse graph between health and sub-healthy subjects,as well as subjects with same age gradient and different health status,which mainly present as the values of h,5,h,3,/h,1,w,1,/t and other indicators. The study results suggest that in sub-healthy subjects,the left ventricular ejection function and the arterial compliance are decreased,the arterial vascular elasticity is reduced,the peripheral resistance is increased and the pulse condition tends to be stringy pulse. The pulse graph can be considered as an objective basis for health status evaluation.
