摘要

目的：观察补肾活血方对薄型子宫内膜排卵障碍患者克罗米芬促排卵周期中子宫内膜容受性及临床妊娠率的影响。方法：将60例薄型子宫内膜排卵障碍患者随机分为对照组与治疗组,每组30例。两组均予克罗米芬促排卵治疗,对照组予戊酸雌二醇口服,治疗组在对照组治疗措施基础上加用补肾活血方。两组疗程均为1个月经周期,3个月经周期后随访,观察临床妊娠率,比较排卵日子宫动脉厚度及类型、子宫动脉搏动指数(PI)、子宫内膜阻力指数(RI)的变化情况。结果：①组间治疗后比较,治疗组对子宫内膜厚度、A型子宫内膜比例的改善明显优于对照组(P＜0.05)。②组间治疗后比较,治疗组PI、RI降低程度明显强于对照组(P＜0.05)。③3个月经周期后,治疗组、对照组临床妊娠率分别为43.3%、26.7%；组间临床妊娠率比较,差异有统计学意义,治疗组明显高于对照组(P＜0.05)。结论：在克罗米芬促排卵周期中,补肾活血方联合戊酸雌二醇可增加薄型子宫内膜患者子宫内膜容受性,从而提高妊娠率。

Abstract

Objective:To observe the effects of Bushen Huoxue Recipe on endometrial receptivity and clinical pregnancy rate of patients with thin endometrial ovulation disorder in clomiphene promoted ovulation cycle. Methods60 patients with thin endometrial ovulation disorder were randomly divided into the control group and treatment group, 30 cases in each group. Both groups were treated with clomiphene to promote ovulation. The control group was orally treated with estradiol valerate, the treatment group was treated with Bushen Huoxue Recipe based on the treatment for the control group. The treatment course was one menstrual cycle in both groups with follow-up after 3 menstrual cycles. The clinical pregnancy rate was observed. The changes on endometrial thickness and type, uterine attery pulsatility index (PI) and uterine attery resistance index (RI) on the day of ovulation were compared between the two groups. Results:①After treatment,the improvement on endometrial thickness and proportion of A type endometrium in the treatment group was significantly better than that in the control group(P＜0.05). ②After treatment,the decrease of PI and RI in the treatment group were greater than those in the control group(P＜0.05). ③After 3 menstrual cycles,the clinical pregnancy rates of the treatment group and the control group were 43.3% and 26.7% respectively. There was statistically significant difference on the clinical pregnancy rate between the two groups, and the clinical pregnancy rate of the treatment group was significantly higher than that of the control group(P＜0.05). Conclusion: In clomiphene promoted ovulation cycle, Bushen Huoxue Recipe combined with estradiol valerate can increase the endometrial receptivity of patients with thin endometrium, and thus improve the pregnancy rate.
