摘要

目的：借助中医传承辅助系统,探讨刘凤斌教授治疗溃疡性结肠炎的用药经验。方法：收集刘凤斌教授治疗溃疡性结肠炎的门诊处方,运用中医传承辅助系统中的关联规则及熵聚类等方法挖掘其中的组方特点和用药规律。结果：①=1\*GB3本研究共纳入处方169首,涉及药物126种,累计使用频次2 248次。②=2\*GB3使用频次在20次以上的药物共34种,其中使用频次最高(58次以上,为高频使用药物)的12味药物分别为藿香、茯苓、黄连、太子参、木香、白术、甘草、白花蛇舌草、火炭母、山药、乌梅炭、青黛。③=3\*GB3依据方剂数量,设置相关度(药物与药物之间的关联度)为8,惩罚度(减少负面数据信息干扰的参数)为2,进行聚类分析,得到关联系数在0.03以上的药对17个。④=4\*GB3基于复杂系统熵聚类方法,按相关度为8和惩罚度为2的约束条件,演化出3～4味药物的核心组合14个；在此基础上,通过无监督的熵层次聚类方法,进一步组合、演化出7首新处方。结论：总结出12味高频用药,17个常用药对,14个核心药组,7首新处方；刘凤斌教授治疗溃疡性结肠炎具有丰富的临床经验,常以健脾化湿、清热利湿之品为治,切合本病脾虚为本、湿热为标的病机；中医传承辅助系统可用于名老中医经验的研究。

Abstract

Objective:To discuss Professor Liu Fengbin’s medication rules in the treatment of ulcerative colitis based on traditional Chinese medicine(TCM)inheritance support system. MethodsProfessor Liu Fengbin’s prescriptions for ulcerative colitis were collected,the prescription characteristics and medication rules were analyzed by association rule mining and complex system entropy cluster in TCM inheritance support system. Results:①This study totally included 169 prescriptions involving 126 drugs with a cumulative frequency of 2 248. ②There were 34 kinds of drugs with frequency of usage more than 20 times. Among,12 drugs that with the highest frequency of usage(medication frequency more than 58 times as high frequency drugs)were Huoxiang(Agastaches Herba),Fuling(Poria),Huanglian(Coptidis Rhizoma),Taizishen(Pseudostellariae Radix),Muxiang(Aucklandiae Radix),Baizhu(Atractylodis Macrocephalae Rhizoma),Gancao(Glycyrrhizae Radix),Baihua Sheshecao(Hedyotis Herba),Huotanmu(Polygonum chinensis),Shanyao(Rhizoma Dioscoreae),Wumeitan(charred Mume Fructus),Qingdai(Naturalis Indigo). ③According to the number of prescriptions,the correlation (degree of association between drugs and drugs) was set as 8 and the penalty (parameters to reduce the interference of negative data information) was 2,then the cluster analysis was performed. 17 couplet medicines that correlation coefficient was more than 0.03 were obtained. ④Based on the complex system entropy clustering,under the constraints of correlation degree as 8 and penalty as 2,14 core combinations that contained 3-4 drugs were evolved. On this basis,by unsupervised entropy hierarchical clustering,seven new prescriptions were further combined and evolved. Conclusion:12 drugs with high frequency of usage,17 commonly used couplet medicines,14 core drug combinations and 7 new prescriptions are summarized. Professor Liu Fengbin has rich clinical experience in the treatment of ulcerative colitis. The products with effects of invigorating spleen for eliminating dampness and clearing heat and promoting diuresis are often used in the treatment,which aim at the pathogenesis of spleen deficiency as root cause and dampness heat as manifestation. TCM inheritance support system can be used for the study of experience of famous and veteran doctors of TCM.
