摘要

纳呆不仅是脾胃病的常见症状,也可能是郁证的表现之一。凡由情志不遂所引起且从郁论治有效的纳呆即为郁证性纳呆。郁证性纳呆存在两种形态:一是单纯郁证,病机主要有肝气郁结或化火、心脾郁结；二是病郁同存,即同时存在郁证与脾胃病,无论因郁致病还是因病致郁,其病机主要为肝胃不和。现代医学的抑郁症、恶劣心境、神经性厌食、躯体化障碍等精神类疾病,以及精神心理因素引起的功能性消化不良等功能性胃肠病,以食欲不振为主要表现者,可以参照郁证性纳呆进行诊疗。

Abstract

Anorexia is not only a common symptom of spleen and stomach diseases,but also may be one of the manifestations of depression syndrome. Anorexia due to depression syndrome refers to anorexia triggered or aggravated by emotional factors and treated effectively by alleviating depression. There are two patterns of anorexia due to depression syndrome,one is single depression syndrome,which involves several pathogenesis such as stagnation of liver qi,stagnant liver qi transforming into fire and stagnation of heart-spleen；the other is coexistence of diseases and depression syndrome,no matter diseases induced by depression syndrome or depression syndrome induced by diseases,the main pathogenesis is incoordination between liver and stomach. Some mental disorders such as depression,dysthymia,anorexia nervosa,somatization disorder, and functional gastrointestinal disorders such as functional dyspepsia caused by psychological factors and mainly manifests as loss of appetite,can be diagnosed and treated according to “anorexia due to depression syndrome”.
