摘要

目的：观察腕踝针配合中药熏蒸治疗血瘀脉络型糖尿病周围神经病变的临床疗效。方法：采用随机对照的方法,将64例患者分为治疗组(32例)和对照组(32例),分别予腕踝针配合中药熏蒸治疗和甲钴胺肌肉注射治疗。两组疗程均为3个月,观察临床疗效及临床症状积分、血液流变学指标及神经病变积分变化情况。结果：①治疗组的临床疗效(总有效率为87.5%)明显优于对照组(总有效率为59.4%),差异有统计学意义(P<0.05)。②治疗后,两组临床症状积分、神经病变积分均明显减少(P<0.05),且治疗组低于对照组(P<0.05)。③治疗后,两组全血高切黏度、全血低切黏度、血浆黏度、纤维蛋白原水平显著降低(P<0.05),且治疗组低于对照组(P<0.05)。结论：腕踝针配合中药熏蒸及肌肉注射甲钴胺均可改善血瘀脉络型糖尿病周围神经病变症状,腕踝针配合中药熏蒸治疗效果优于肌肉注射甲钴胺。

Abstract

Objective:To observe the clinical efficacy of wrist-ankle acupuncture combined with fumigation and steaming therapy of Chinese medicine in the treatment of diabetic peripheral neuropathy with syndrome of blood stasis in channels and collaterals. MethodsBased on the random and controlled trial,64 patients were divided into the treatment group and control group,32 cases in each group. The treatment group was treated with wrist-ankle acupuncture combined with fumigation and steaming therapy of Chinese medicine and the control group was treated with mecobalamin by intramuscular injection,with a course of 3 months. The clinical efficacy was observed,and the changes on clinical symptom score,hemorheological indexes and neuropathy scores were compared before and after treatment. Results:①The total effective rates were 87.5% in the treatment group and 59.4% in the control group,and the clinical efficacy of the treatment group was significantly better than that of the control group,with statistical difference(P<0.05). ②After treatment,the clinical symptom score and neuropathy scores were obviously decreased in both groups(P<0.05),and the scores of the treatment group were lower than those of the control group(P<0.05). ③After treatment,the levels of whole blood high and low shear viscosity,plasma viscosity and fibrinogen were significantly decreased in both groups(P<0.05),and the levels of the treatment group were lower than those of the control group(P<0.05). Conclusion:Both intramuscular injection of mecobalamin and wrist-ankle acupuncture combined with fumigation and steaming therapy of Chinese medicine could improve the symptoms of diabetic peripheral neuropathy with syndrome of blood stasis in channels and collaterals. The efficacy of wrist-ankle acupuncture combined with fumigation and steaming therapy of Chinese medicine is better than that of intramuscular injection of mecobalamin.
