摘要

目的：观察活血止痛散联合西药治疗早期糖尿病足的临床疗效。方法：将120例早期糖尿病足患者随机分成治疗组与对照组,每组60例。对照组常规西药治疗,治疗组在对照组治疗措施基础上加服活血止痛散。两组疗程均为20天,观察临床疗效,比较血糖水平、足部踝肱比及趾肱比、深浅感觉、运动及感觉神经传导速度的变化情况。结果：①治疗组、对照组临床总有效率分别为80.00%、55.00%；组间临床疗效比较,治疗组明显优于对照组(P<0.05)。②组间治疗后比较,空腹血糖、餐后2 h血糖水平差异有统计学意义,治疗组明显低于对照组(P<0.05)。③组间治疗后比较,踝肱比、趾肱比水平差异有统计学意义,治疗组明显较对照组升高(P<0.05)。④组间治疗后比较,浅感觉、深感觉水平差异有统计学意义,治疗组感觉功能提高明显优于对照组(P<0.05)。⑤组间治疗后比较,腓总神经、正中神经传导速度差异有统计学意义,治疗组神经传导速度明显快于对照组(P<0.05)。结论：活血止痛散联合西药治疗早期糖尿病足,可明显缓解患者的临床症状,降低血糖水平,改善足部的深浅感觉及神经功能,从而有效控制本病的病情发展。

Abstract

Objective:To observe the clinical efficacy of Huoxue Zhitong Powder combined with western medicine in the treatment of early diabetic foot. Methods120 patients with early diabetic foot were randomly divided into the treatment group and control group,60 cases in each group. The control group was treated with routine therapy of western medicine,and the treatment group was orally treated with Huoxue Zhitong Powder based on the treatment for the control group,with a course of 20 days. The clinical efficacy was observed,the changes of blood sugar level,ankle-to-brachial ratio index(ABI)and toe-to-brachial ratio index(TBI)in foot,deep and superficial sensation and motor sensory nerve conduction velocity were compared. Results:①The clinical total effective rate was 80.00% in the treatment group and 55.00% in the control group. The clinical efficacy of the treatment group was significantly better than that of the control group(P<0.05). ②After treatment,there were statistically significant differences on the levels of fasting plasma glucose(FPG)and 2h postprandial glucose(2h PG)between the two groups,and the levels of the treatment group were obviously lower than those of the control group(P<0.05). ③After treatment,there were statistically significant differences on the ABI and TBI between the two groups,the ratio indexes of the treatment group were obviously higher than those of the control group(P<0.05). ④After treatment,there were statistically significant differences on the levels of deep and superficial sensation between the two groups,and the improvement on sensory function in the treatment group was obviously better than that in the control group(P<0.05). ⑤After treatment,there were statistically significant differences on the conduction velocity of common peroneal nerve and median nerve between the two groups,and the conduction velocity in the treatment group were obviously faster than those in the control group(P<0.05). Conclusion:Huoxue Zhitong Powder combined with western medicine in the treatment of early diabetic foot can obviously alleviate the clinical symptoms of patients,reduce the blood sugar level,improve the deep and superficial sensation and neurological function in foot,and thus effectively control the disease development.
