摘要

目的：观察颜氏温阳活血方防治冠脉介入术后再狭窄的临床疗效。方法：将60例冠脉介入术后患者随机分为治疗组与对照组,每组30例。对照组予西药常规治疗,治疗组在对照组治疗措施基础上加用颜氏温阳活血方。两组疗程均为3个月,随访观察心血管事件及再狭窄发生情况,比较心绞痛相关积分、中医血瘀证及胸痹证评分的变化情况。结果：①治疗组、对照组再狭窄发生率分别为3.33%、20.00%,组间再狭窄发生率比较,治疗组明显低于对照组(P<0.05)。②组间治疗后比较,心绞痛发作次数、持续时间、疼痛程度、NTG用量、心绞痛分级积分及总积分差异有统计学意义,治疗组改善心绞痛症状明显优于对照组(P<0.05)。③组间治疗后比较,胸痛、舌质紫暗瘀斑、口唇齿龈紫暗、舌下脉络曲张评分及总评分差异有统计学意义,治疗组对血瘀证的整体缓解程度明显优于对照组(P<0.05)。④组间治疗后比较,胸闷、心悸、畏寒肢冷、气短乏力、面白唇淡评分及总评分差异有统计学意义,治疗组对胸痹证的整体缓解程度明显优于对照组(P<0.05)。结论：颜氏温阳活血方防治冠脉介入术后再狭窄,可明显降低再狭窄发生率,改善患者临床症状。

Abstract

Objective:To observe the clinical efficacy of Yan’s Wenyang Huoxue Recipe in the prevention and treatment of restenosis after percutaneous coronary intervention. Methods 60 patients who had received the percutaneous coronary intervention were randomly divided into the treatment group and control group,30 cases in group. The control group was treated with the routine therapy of western medicine, the treatment group was treated with Yan’s Wenyang Huoxue Recipe based on the treatment for the control group,with a course of 3 months. During the follow-up,the cardiovascular events and the restenosis occurrence were observed,the angina-related scores, TCM syndrome scores of blood stasis and chest painful impediment were compared. Results: ①The incidence rate of restenosis in the treatment group and the control group was 3.33 % and 20.00 %, respectively. The incidence rate of restenosis in the treatment group was significantly lower than that in the control group(P<0.05). ②For the comparison between the two groups after treatment,the differences on the occurrence frequency, duration, pain degree, NTG dosage, grading score of angina and the total score were statistically significant,the improvement on angina symptoms in the treatment group was better than that in the control group(P<0.05). ③For the comparison between the two groups after treatment,the differences on the syndrome scores of chest pain, dark purple tongue with ecchymosis, dark purple lips and gums, sublingual varicose vein and the total score were statistically significant,the improvement on blood stasis syndrome in the treatment group was better than that in the control group(P<0.05). ④For the comparison between the two groups after treatment,the differences on the syndrome scores of oppression in chest,palpitation,fear of cold and cold in limbs,breath shortness and weakness,white complexion and pale lips and the total score were statistically significant,the improvement on syndrome of chest painful impediment was better than that in the control group(P<0.05). Conclusion: Yan’s Wenyang Huoxue Recipe can be used in the prevention and treatment of restenosis after percutaneous coronary intervention,which significantly decrease the incidence rate of restenosis and improve the clinical symptoms of patients.
