摘要

目的：观察养阴化痰息风方治疗反复发作性肝性脑病的临床疗效。方法：将86例反复发作性肝性脑病患者随机分为治疗组与对照组,每组43例。对照组予利福昔明及乳果糖治疗,治疗组予养阴化痰息风方治疗。两组疗程均为6个月,观察治疗期间肝性脑病发生次数、比较数字连接试验及血浆血氨水平的变化情况。结果：①最终完成试验者74例,治疗组37例,对照组37例。②治疗期间,治疗组肝性脑病的平均发作次数明显少于对照组(P,<,0.05)。③治疗前后组内比较,两组NCT-A试验时间、DST试验评分差异均有统计学意义(P,<,0.05)；组间治疗后比较,NCT-A试验时间、DST试验评分差异无统计学意义(P,>,0.05)。④治疗前后组内比较,两组血氨(NH,3,)水平差异均有统计学意义(P,<,0.05)；组间治疗后比较,血氨水平差异有统计学意义,治疗组降低程度大于对照组(P,<,0.05)。结论：养阴化痰息风方治疗肝性脑病,可明显减少本病的发作次数,降低血氨水平。

Abstract

Objective:To observe the clinical effects of “Yangyin Huatan Xifeng Decoction” in the treatment of recurrent hepatic encephalopathy. MethodsEight-six subjects were randomly divided into treatment group and control group, 43 cases in each group. Control group was treated with rifaximin and lactulose, while treatment group was given “Yangyin Huatan Xifeng Decoction”, with a course of 6 months. The occurrence of hepatic encephalopathy was observed and the number-connection test and the plasma level of ammonia were compared during the treatment. Results:①Seventy-four cases completed the whole trials, among which 37 cases were in treatment group and 37 cases were in control group. ②During the course of treatment, the average attack times of hepatic encephalopathy in the treatment group were significantly fewer than that of the control group(P<0.05). ③The differences of NCT-A test time, DST test scores and blood ammonia levels within each group before and after treatment were statistically significant(P<0.05). But there were no significant differences in the NCT-A test time and DST test scores between two groups. Compared with the control group after treatment, the blood ammonia levels in the treatment group were significantly decreased(P<0.05). Conclusion:“Yangyin Huatan Xifeng Decoction” works well on recurrent encephalopathy in reducing the attack frequency and blood ammonia.
