目的:探究补肾健脾方对人肝癌高转移原位移植瘤裸鼠切除术后肿瘤复发的影响以及对索拉非尼的增效减毒作用 方法:剥取人肝癌高转移原位移植裸鼠模型(HCCLM3)肝癌瘤体,切1 mm×1 mm×1 mm大小,原位植入实验裸鼠肝脏内,术后常规无菌饲养。术后第15天开腹行新生肿块切除术,术后同前饲养。将实验裸鼠分为补肾健脾方组(A组)、索拉非尼组(B组)、联合用药组(C组)、模型对照组(D组),每组7只;于第2次手术后第3天开始,根据裸鼠体质量给予相应干预,均灌胃给药,连续给药4周,每天1次。记录给药前1天、给药后第7天、第14天、第21天、第28天(D0、7、14、21、28)裸鼠体质量以及给药后每天饲料消耗量,共28 d。最后一次给药结束48 h后处死裸鼠,采集血液、瘤体标本,测量瘤重、瘤体体积,并检测血常规指标,包括白细胞(WBC)、红细胞(RBC)、血红蛋白(Hb)、血小板(PLT) 结果:①瘤重和瘤体体积:与D组比较,A、B、C组均能显著减轻复发瘤体重量并缩小体积(P<0.001);C组比A组更能降低瘤重及其体积(P<0.05)。②鼠重:与A组比较,B、C、D组差异均有统计学意义(P<0.05);与D0比较,D7、D14、D21、D28各组裸鼠体质量均有不同程度的降低,差异均有统计学意义(P<0.05)。裸鼠体质量下降值从小到大依次为A组(0.60±1.75)g,C组(1.29±1.63)g,D组(1.57±1.84)g,B组(2.14±2.40)g。③饲料消耗量:A组、B组、C组和D组每组裸鼠饲料消耗总量组间比较差异显著(P<0.001);与A组比较,B、C、D组差异有统计学意义(P<0.001);D组与C组比较差异有统计学意义(P<0.05)。术后A组裸鼠饮食量呈上升趋势,C组呈缓慢上升后稳步下降趋势,B和D组饲料消耗量减少最为明显。④血常规:各项指标4组组间比较均有统计学差异(P<0.05)。血象4个指标可见,A组与D组比较差异无统计学意义(P>0.05),WBC、RBC、HGB水平A组均略高于D组,A组与B、C两组比较,差异有统计学意义(P<0.05);各项指标B组水平最低,相较于其余3组,差异均有统计学意义(P<0.05),C组指标水平均略高于B组(P>0.05) 结论:补肾健脾方能够抑制肝癌复发瘤体生长,增强机体对索拉非尼的耐受程度,减轻其对血液系统的毒性反应,稳定机体日常饮食摄取和体质量,对索拉非尼有增效减毒作用。
Objective:To explore the effect of “Bushen Jianpi Decoction” on tumor recurrence inhibition of high metastasis models of human hepatocellular carcinoma (HCC) after curative in nude mice and its synergies and toxicity-reducing effect on Sorafenib Methods:Decorticate and reset the tumor in HCCLM3 model nude mice with a size of 1 mm×1 mm×1 mm. Transplant the tumor into the liver of experimental nude mice orthotopicly. On postoperative day 15,do laparotomy to resct the new tumor. Divide experimental nude mice into group A,B,C and D,6 in each group. From the 3rd day of the 2nd surgery,according to the nude mice weight,they received gavage in a dosage of 0.1ml/10 g each day,for 4 weeks in all. Group A were given “Bushen Jianpi Decoction”. Group B received Sorafenib suspension. Group C were treated with both two medicines. Group D were administered 0.9%NS. Record the weight of nude mice pre-dose,the 7th,14th,21st and 28th day(D0,7, 14,1, 28)of the course as well as the fodder consumption every day of each group. After treatment,the tumor weight and size were measured and Blood Routine Index(eg.WBC,RBC,Hb,PLT) were tested Results:① Tumor weight and size:In Group A,B and C,Tumor weight(P<0.001) were lower and size(p=0.001) smaller than in Group D; Both weight and size in Group C decreased more than Group A(P<0.05). ② Mice weight:Compared Group A with Group B,C,D, there was statistical difference(P<0.05). Mice weight on D7、14、21、28 were all lower than D0(P<0.05).Falling range from smaller to larger was Group A(0.60±1.75),C(1.29±1.63),D(1.57±1.84)and B(2.14±2.40). ③ Fodder consumption:Compared Group A with Group B,C,D, there was statistical difference(P<0.001);Group C and D had statistical difference(P<0.05). Group A ate more than the other groups while Group C’s fodder consumption first slowly raised then decreased. Fodder consumption of Group B and D reduced more obviously. ④ Blood Routine Index(WBC, RBC, HGB, PLT):Four indexes of Group A and D had no statistical difference(P>0.05),but WBC,RBC and HGB count of Group A were higher than D. Compared Group A with Group B and C,there were statistical difference(P<0.05). Group B was the lowest of all indexes(P<0.05). Group C was a little bit more than B,while Group B and C had no statistical difference(P>0.05) Conclusion:“Bushen Jianpi Decoction” could inhibit recurrent tumor growth,improve the body resistance against Sorafenib,decrease its toxic reaction to the Blood System,and stablize the body dietary and weight. The Decoction could restrain HCC recurrence after hepatectomy,and it had synergies and toxicity-reducing effect on Sorafenib.
