摘要

目的：观察加味痹证“三两三”方联合甲氨蝶呤治疗寒湿瘀阻型类风湿关节炎的临床疗效方法：将120例寒湿瘀阻型类风湿关节炎患者随机分为治疗组(80例)与对照组(40例)。对照组予甲氨蝶呤口服,治疗组在对照组治疗基础上加用加味痹证“三两三”方。两组疗程均为8周,观察临床疗效及安全性,比较临床症状与体征(TJC28、SJC28、VAS评分)、实验室指标(ESR、CRP)、病情评估指数(PGA、MDGA、CDAI)的变化情况结果：①治疗组、对照组总有效率分别为86.25%和67.50%,组间临床疗效比较差异有统计学意义(P<0.05)。②治疗前后组内比较,两组TJC28、SJC28、VAS评分差异均有统计学意义(P<0.05)；组间治疗后比较,TJC28、SJC28、VAS评分差异有统计学意义(P<0.05)。③治疗前后组内比较,两组ESR、CRP水平差异有统计学意义(P<0.05),治疗组Hb水平差异有统计学意义(P<0.05),对照组Hb水平差异无统计学意义(P>0.05)；组间治疗后比较,ESR、CRP水平差异无统计学意义(P>0.05),Hb水平差异有统计学意义(P<0.05)。④治疗前后组内比较,两组PGA、MDGA、CDAI指数差异均有统计学意义(P<0.05)；组间治疗后比较,PGA、MDGA、CDAI指数差异有统计学意义(P<0.05)。⑤两组不良反应发生率比较,差异有统计学意义(P<0.05) 结论：加味痹证“三两三”方联合甲氨蝶呤治疗寒湿瘀阻型类风湿关节炎,可缓解疼痛,降低疾病活动度,且不良反应较少。

Abstract

Objective:To observe the clinical efficacy of Jiawei arthromyodynia “San Liang San Decoction” combined with methotrexate in treatment of cold dampness and blood stasis rheumatoid arthritis Methods:120 cases of patients with cold dampness and blood stasis rheumatoid arthritis were randomly divided into treatment group (80 cases) and control group (40 cases). The control group was treated with methotrexate orally; the treatment group was treated with Jiawei arthromyodynia “San Liang San Decoction” based on the treatment of the control group. Two groups were treated for 8 weeks. The clinical efficacy and safety were observed. The changes of linical symptoms and signs (TJC28, SJC28, VAS score), laboratory indexes (ESR, CRP), the condition assessment index (PGA, MDGA, CDAI) were compared Results:①The total efficiency of the treatment group and the control group were 86.25% and 67.50% respectively. The difference of the clinical efficacy between the two groups was statistically significant (P<0.05). ② Compared before and after the treatment, the difference of TJC28, SJC28, VAS score of the two groups were statistically significant (P<0.05); after the treatment, the difference of TJC28, SJC28, VAS score between the two groups were statistically significant (P<0.05). ③Compared before and after the treatment, the differences of the levels of ESR, CRP of the two groups were statistically significant (P<0.05); the difference of HGB level in the treatment group was statistically significant (P<0.05), while had no statistically significant difference in HGB level in the control group (P>0.05). After the treatment, there was no statistically significant differences in ESR and CRP level (P>0.05) while the difference of HGB level was statistically significant (P<0.05). ④ Compared before and after the treatment, the difference of PGA, MDGA, CDAI index level were statistically significant (P<0.05); after the treatment, the difference of PGA, MDGA, CDAI index level was statistical significance (P<0.05). ⑤ The occurrence rate of adverse reactions of the two groups were compared, the difference was statistically significant (P<0.05) Conclusion:Jiawei arthromyodynia “San Liang San Decoction” combined with methotrexate in treatment of cold dampness and blood stasis rheumatoid arthritis can relieve pain, reduce the disease activity, and has less adverse reactions.
