摘要

目的：探讨迟发性性腺功能减退症中医证型分布情况及临床特征方法：选择迟发性性腺功能减退症患者461例,进行中医辨证分型及老年男子症状量表(AMS)评分,同时测定血清中总睾酮(TT)水平结果：在461例患者中,以肝肾阴虚型占比最高(34.49%),其余依次为肾阳亏虚型、肾虚肝郁型、心脾两虚型和其他型。肝肾阴虚型血管舒缩症状积分高于其他证型,而体能症状积分低于其他证型(P<0.05)；肾阳亏虚型性功能症状积分高于其他证型,而血管舒缩症状积分低于其他证型(P<0.05)；肾虚肝郁型精神心理症状积分高于其他证型(P<0.05)。肾阳亏虚型、心脾两虚型血清总睾酮(TT)水平低于肝肾阴虚型、肾虚肝郁型(P<0.05) 结论：迟发性性腺功能减退症中医证型以肝肾阴虚、肾阳亏虚为主,各证型临床症状和睾酮水平存在一定的差异；肝肾阴虚型以血管舒缩症状为主,肾阳亏虚型以性功能症状为主,肾虚肝郁型以精神心理症状为主,心脾两虚型体能症状较其他证型有更为严重的趋势；肾阳亏虚型、心脾两虚型血清总睾酮(TT)水平低于肝肾阴虚型和肾虚肝郁型。

Abstract

Objective: To observe the TCM syndrome distribution and clinical characteristics of late onset hypogonadism Methods: Four hundred and sixty-one cases with late onset hypogonadism (LOH) were divided into TCM syndrome types. The scales of AMS were surveyed and the total testosterone (TT) levels in serum were detected Results: Among the 461 patients, the deficiency of liver yin and kidney yin (34.49%) were the main two TCM syndrome types of LOH. The rest of them are deficiency of kidney yang type, kidney deficiency and liver depression type, deficiency of heart and spleen type, etc. Symptom scores of angiokinesis in deficiency of liver yin and kidney yin type was higher than those of the other types, while symptom scores of physical ability was lower than those of the other types (P＜0.05); scores of sexual function in deficiency of kidney yang type was higher than those of the other types, but the symptom scores of angiokinesis was lower than those of the other types (P＜0.05); symptom scores of mental-psychological in kidney deficiency and liver depression type was higher that those of the other types(P＜0.05). The testosterone (TT) levels in serum of the deficiency of kidney yang type and deficiency of heart and spleen type were lower than those of the deficiency of liver yin and kidney yin type and kidney deficiency and liver depression type (P＜0.05) Conclusion: The deficiency of liver yin and kidney yin and deficiency of kidney yang type are the main types of LOH. There are significant differences in clinical efficacy and TT level among TCM syndrome types.
