慢性疼痛是全球性健康难题,开发多学科综合管理模式已成为共识。传统中医推拿“外治”疼痛的疗效确切且理论坚实,而“内应”获效机制不明,这限制了其国际化推广应用。近年来研究发现,躯体痛觉神经传导通路中多分子变化是推拿镇痛的局部“内应”靶点,但这些发现未能体现中医整体观指导下的联动协调机制。现代医学认为,神经系统高度可塑性对慢性疼痛的发生、维持至关重要,这与推拿的整体调节作用存在共性。通过总结“外治内应”理论源流及其现代解释,从神经可塑性角度系统探析推拿镇痛的全局效应机制,发现推拿可调控“脊髓-脑干-大脑”三级疼痛网络中的神经可塑性获效,为推拿镇痛“外治内应”的科学内涵提供了依据。
Chronic pain is a global health issue, and the development of integrated multidisciplinary management models has become widely accepted. The external treatment of pain with traditional Chinese medicine (TCM) tuina has demonstrated definite efficacy and a solid theoretical foundation, while the underlying mechanisms of its "internal response" therapeutic effects remain unclear. This gap has hindered the international promotion and application of tuina. Recent studies have identified molecular changes in somatic pain neural pathways as local "internal response" targets for tuina analgesia. However, these findings do not fully capture the integrative, coordinated mechanism guided by the holistic approach of TCM. Modern medicine acknowledges the significant role of neural plasticity in the initiation and maintenance of chronic pain, which shares common ground with the overall regulatory effects of tuina. By reviewing the origins and modern interpretations of the "external treatment inducing internal response" theory, this paper systematically explores the global mechanism of tuina analgesia from the perspective of neuroplasticity. The results show that tuina can modulate the neuroplasticity within the "spinal cord-brainstem-brain" pain network, providing scientific evidence for the "external treatment inducing internal response" theory in tuina analgesia.
